[Assessment of long follow-up results after surgical treatment of stress urinary incontinence using tvt and pubovaginal short skin flap sling].

The aims of present study were to evaluate the quality of women's life (QoL) after surgical treatment of stress urinary incontinence (SUI) and to compare long follow-up results of two different procedures. Quality of life questionnaire represents a useful and cheap tool in the investigation of outcomes of the surgical treatment of stress incontinence. By providing an objective measure of the impact of results on women's lives, quality of life questionnaires allow more comprehensive understanding of disease severity and its effect on psychosocial function. In addition, they have an increasingly important role in clinical research, allow comparing two and more different methods of treatment. QoL of patients after TVT was significantly higher, then after pubovaginal sling using short skin flap (subjective cure 80% versus 66% after 48 months follow up). These benefits in QoL, coupled with the minimally invasive nature of the procedure, suggest that TVT could become a favored treatment option in women with SUI.

[Tavanik effectiveness and safety in the treatment of urinary tract infection complications].

20 patients with complicated urinary infection (CUI) have been treated with levofloxacine (tavanik) for 10 days in a single daily dose 250 mg. They were compared with 23 acute pyelonephritis patients given ciprofloxacin in a dose 1 g/day. None of the patients had purulent inflammation or was treated with antibacterial drugs. The response to tavanik and its safety were good or very good in 95% patients. With ciprofloxacin the same results were achieved in 88% patients. 3 patients failed this treatment. 2 patients developed purulent pyelonephritis and were operated. The number and intensity of side effects were higher were less in the tavanik group. Thus, tavanik, a new antibacterial drug from fluoroquinolone group, is effective and low-toxic in complicated infection of the urinary tracts.

[Experience with the use of ciprofloxacin in patients with acute uncomplicated cystitis].

The efficacy of ciprofloxacin was studied in the treatment of 50 women (the average age of 41.6 years) with acute noncomplicated cystitis. The drug was administered in a dose of 100 mg twice a day for 3 days. The reference group included 15 women with the same disease subjected to the routine therapy with cotrimoxazol in a dose of 8 mg and pipemidic acid in a dose of 100 mg administered twice a day. The positive results evident from the subjective clinical improvement and no veritable bacteriuria were stated in 46 patients (92 per cent). The effect was at the average observed in 36 hours in the ciprofloxacin group while in the reference group it was at the average stated in 81.2 hours from the treatment start.

[Experience in the treatment of 152 women with bilateral injuries of the ureter].

Data on the treatment of 152 women with bilateral injuries to the ureters are presented. Many-year experience gained in the treatment of women with such injuries permitted the authors to single out their main causes and to find the most rational methods of derivation of the urine and develop the treatment policy depending on the type of injury to the ureters, complications of these injuries, and anatomo-functional status of the urinary system. Plastic surgery brought good results in 84.4% of patients.

[Use of driptan (oxybutinin) in patients with urgent forms of urination disorders].

Imperative micturition occurs rather frequently. The data of different authors favour driptan (oxibutinin) in this condition. In the urological clinic of the Medical Institute oxibutinin hydrochloride was tried since 1991. A total of 354 patients were treated (328 females, 12 males and 14 adolescents). The indications to the treatment were the following: instability of the detrusor, urethra or both, isolated forms of pollakiuria, some forms of enuresis, irritation in benign prostatic hyperplasia, correction of detrusor instability in enuresis before operation. Oxibutinine hydrochloride produced positive results in various forms of imperative micturition with symptoms of urgent incontinence (urgent enuresis) or without them. The drug produced a response in many socially maladapted individuals considered incurable.

[Parameters of PSA fractions in early diagnosis of prostatic cancer in healthy males over 50].

292 symptomless males aged 50-85 years were examined for blood prostate-specific antigen (PSA) for early diagnosis of prostatic cancer. Males with total PSA (TPSA) higher than age-specific standard underwent free PSA (FPSA) assay to estimate FPSA/TPSA and transperineal biopsy of the prostate. TPSA ranged from 0.3 to 112.0 ng/ml and exceeded the age standard in 102 examinees (35%). Prognostically unfavorable FPSA/TPSA (under 15%) was registered in 17(5.8%) examinees. Biopsy of the prostate has revealed cancer in 7 of them. Among those whose TPSA was above the standard, but FPSA/TPSA > 15%, morphologically verified prostatic cancer was detected in 5 males. It is thought valid to include the method of PSA fractions estimation in the program of screening for males at risk to develop prostatic cancer as it is simple, cost effective and highly informative.

[Application of resectoscope of Marberger type with cutting and vaporizing loops in the treatment of benign prostatic hyperplasia].

One of the latest achievement in the design of tools for TUR is introduction of a new resectoscope constructed by M. Marberger. Its oval loops can remove 30% more tissue than it is made by a standard resectoscope with semicurcular loops. 203 patients with benign prostatic hyperplasia (BPH) were operated using Marberger resectoscope, 54 BPH patients were operated using a standard resectoscope. 12-month follow up results of both TURs did not differ much. The advantage of the new tool is quicker removal of the tissue.

[Safety ov benign prostatic hyperplasia treatment with alpha-adrenoblocker dalphas].

The study was made of dalphas-retard safety and effectiveness in 940 patients with benign prostatic hyperplasia and relevant urination problems. The drug was given in a dose 5-10 mg/day for 12 weeks. The results were assessed in 881 patients who had completed the treatment course. 18 patients (1.9%) discontinued the treatment because of side effects. 38 patients (4%) had side effects once or more times.

[Effectiveness of various alfuzosin schedules in patients with benign prostatic hyperplasia (BPH)].

238 patients with initial BPH (mean age 64.07 years) were treated with alfusosine in a standard dose 5 mg twice a day for 6 months. After that 117 patients continued the above scheme while 121 patients continued to receive alfusosine in the same dose each other day. The results assessed by IPSS, QOL, maximal urine flow rate, levels of prostate-specific antigen in the serum appeared identical in both groups. Thus, the split course of alfusosine after 6-month standard therapy is perspective as it is more cost effective than the standard scheme being equally beneficial.

[Pharmacological and cost effectiveness bases of the use of categel and categel S [correction of F] in urological practice].

Preparations catedgel and catedgel S made in Austria (Montavit) was tried in Moscow hospital N 50. Categel is a sterile gel of methylcellulose with 2% lidocain and 0.05% chlorhexidine, catedgel S contains the same components but lidocain. Categel significantly reduces the risk of infectious-inflammatory complications after endourological manipulations, improves endoscopic diagnosis and makes some manipulations less painful. Comparative pharmacological cost-effect assessment of categel S and glycerine effects in prostatic transurethral resection. Categel was found 2.11 times more effective. It also improves quality of life of the patients. Categel can be recommended for wide use in urology.

[Bicalutamide monotherapy of patients with disseminated forms of prostatic cancer].

Combined antiandrogen therapy (complete androgen blocking) implies surgical or chemical castration in combination with antiandrogen therapy. Both sources of androgens are thus blocked in patients with locally disseminated prostatic cancer. The purpose of this study was to evaluate the efficiency of bikalutamide (150 mg) monotherapy. The study was carried out in 58 patients with prostatic cancer (stages T3-T4) divided into 2 groups: 1) bikalutamide monotherapy (150 mg daily) and 2) bikalutamide (50 mg daily) + bilateral orchidectomy. All patients were examined before and 3, 6, 12, 18, and 24 months after the beginning of therapy. The efficiency of non-steroid antiandrogen was higher in group 1. The mean level of prostate-specific antigen in this group decreased to 10.6 ng/ml by 24 months, while in group 2 to 25.3 ng/ml. Hence, bicalutamide was effective, safe, and well tolerated in our study, and is therefore recommended as monotherapy for patients with disseminated forms of prostatic cancer.

[Assisted reproductive technologies in urology].

Since 1996 the authors have examined and treated 42 infertile married couples (the husbands had azoospermia). To obtain spermatozoa directly from the testes or epididymis and subsequent ICSI, 42 patients underwent 53 interventions: TESE (n = 38), PESA (n = 10), MESA (n = 5). Spermatozoa were obtained from 32(76.1%) of 42 patients, fertilization occurred in 21(50%) cases, 10(23.8%) wives got pregnant. Assisted reproductive technologies expand potentialities of correcting severe forms of male infertility including azoospermia. The armory of the operative procedures in male infertility was replenished with MESA, TESE, PESA.

[Comparative evaluation of the effectiveness and safety of combined drug therapy of patients with benign prostatic hyperplasia with finasteride and alfuzozine].

A finasteride + alfuzozine combination was used in 3-year treatment of 138 patients with initial benign prostatic hyperplasia (BPH). The principle of the combination is in parallel administration of 5 alpha-reductase inhibitors which inhibit cell proliferation at the hormonal level and alpha-adrenoblockers affecting the smooth muscle component of prostatic stroma and detruzor blood supply. Patients participating in the study had enlanged prostate (at least 60 cm3) and pronounced symptoms (IPSS over 13 scores). Patients of two control groups received alfuzozine and finasteride monotherapy, respectively. Better micturition and relief of BPH symptoms were seen in 96% patients of the study group, 84 and 74% controls, respectively. Thus, compared to monotherapy, finasteride and alfuzozine were more effective in combination which is pathogenetically valid and perspective in BPH chemotherapy.

[Ovestin in preparation for transvaginal surgeries].

The trial of the efficiency of the drug estriol-ovestin in preoperative preparation of patients to transvaginal operations included 118 patients (mean age 48.6 years). 82 patients received ovestin as a vaginal suppository and creams in a daily dose of 500 mu for 2-3 weeks before the operation. 36 control patients did not receive ovestin in their preoperative preparation. Effectiveness of ovestin was assessed by duration of pain symptom in the postoperative period, duration of the vaginal postoperative discharge, duration of dysuria after removal of a urethral catheter, the time of renewal of sexual intercourses. The results of the study proved effectiveness of ovestin in preoperative preparation of patients for vaginal surgery.

[Yohimbine in the treatment of erectile dysfunction].

Iochimbin hydrochloride was given to 153 men with erectile dysfunction. The results are available for 140 of them. The ability of iochimbin in a single dose of 5-10 mg to enhance arterial blood inflow to cavernous bodies of the penis was confirmed by dynamic angiopenoscintigraphy and Doppler ultrasonography. Iochimbin hydrochloride in a mean daily dose of 15-20 mg proved effective in erectile dysfunction--38 to 84% responders depending on the type of erectile dysfunction. Occasional side effects can be relieved by reducing the drug dose.
