The surgical and health care innovator: patents and marketing guidelines.

Many physicians and health professionals have had ideas for new devices but are reluctant to pursue the idea to production. Some abandon a sound innovation because of unfamiliarity with the intricate processes from concept to final marketing. The subject of surgical or medical innovation is not taught in medical schools or in residency training and few articles have appeared in the medical literature. This presentation is meant to guide the potential inventor from the concept stage through protection measures and the patent process to the final manufacture of a new device. It is hoped that this article will provide the innovator with a sound footing and stimulate him to contribute to medical technology and medical progress. Other disciplines (scientific, academic, and industry) might also be interested in these guidelines on patents and marketing strategies.

Breast self-examination: a new training method.

Breast self-examination is an important, well-established method of cancer detection. Some authors disagree as to its value and regard physical examination by a physician as superior. Many women, however, do not have an annual physical examination, and the majority (80%) discover their own breast cancer. Proponents find that monthly breast self-examination leads to earlier diagnosis and treatment, and to improved prognosis. Only one third of women are performing monthly breast self-examination, and most do so with inadequate technique and poor comprehension. They are unable to distinguish normal from abnormal findings, and corrective measures are necessary. Health professionals, the best source of patient instruction, should periodically recheck the patient's performance. The technique taught should be uniform and uncomplicated. A simple way to demonstrate common benign and malignant lesions is presented to improve the patient's understanding, skill, and confidence.

Vermiform appendix located within the cecal wall. Anomalies and bizarre locations.

The unusual finding of an acute appendix located within the posterior wall of the cecum, beneath the serosa, and its surgical removal is described. In order to remove the appendix, the cecal wall had to be incised. The appendix had no serosal covering, and had the gross appearance of a peeled seedless grape. The anatomy and embryology of the vermiform appendix, including this aberration, are presented briefly. A discussion of the difficult-to-find appendix is presented. The possibility exists that some of the reported cases of agenesis of the appendix could have been an overlooked intracecal appendix.

Improved triple lumen all purpose drains and their care and management.

A triple lumen all purpose drain has become well established over the past decade as an all purpose drain. It permits either overflow or suction (closed or sump) drainage, aspiration for the taking of cultures or analysis of fluid, irrigation either continuous or intermittent, and the instillation of medications. Further improvements have been made in the drain in the quest for the ideal surgical drain. It is now made of silicone, has a radiopaque stripe, a suture sleeve, a triple lumen molded connector with a drainage tube, irrigation port, bacterial filter, and stoppers. The advantages of these additions are presented. In addition, the drain is being made in two sizes; the smaller one is indicated in pediatric, breast, plastic, head and neck, and other procedures. The care and management of these drains is discussed.

Excision and delayed closure of pilonidal sinuses.

An alternative drainage procedure, a technique of excision and delayed closure for the treatment of pilonidal sinuses, was used upon 78 patients. The procedure compared favorably with marsupialization as well as the excision and semiprimary closure technique with regard to healing time and low recurrence rates.

A suture-tightening technic for delayed wound closure.

A method is described that allows the tightening of standard suture materials for skin approximation of open wounds. The need for wire sutures in such circumstances is therefore unnecessary. The technic has been applied in delayed primary or secondary wound closure, and in the tightening of loosened ties or retention sutures.

Delayed mastectomy after outpatient breast biopsy. Long-term survival study.

Forty-one patients who had outpatient biopsy and delayed mastectomy (most within 3 days) were subjected to long-term survival study. The ten year and longer survival studies revealed no deleterious effects from this procedure. The improved statistics were believed due to operation on lesions considered clinically benign, the short period of delay, and a low incidence of axillary metastasis.
