Chronic renal failure challenges both patients and families and the nurses who assist them to learn to manage a complex illness treatment program. The Chronic Renal Failure Evaluation and Teaching Tool described here is used to assess and teach inpatients being treated medically with or without dialysis. This article describes the format, content, and use of this tool.
This was a collaborative project between education and service that focused on teaching patients with COPD and resulted in the development and testing of an evaluation and teaching tool. The process is described and analyzed using an adaptation of Lancaster's framework for successful collaboration.

An evaluation and teaching tool for patients with chronic obstructive pulmonary disease was found to be helpful in preparing patients for self-care at home. Similar tools could be useful for patients with other chronic illnesses.
The increasing burden of documentation on nurses and the advent of improved computer technology prompted the development of a bedside documentation system. The time required for documentation of the nursing assessment and the number of observations recorded by the computer were compared with handwritten documentation. The time required for computer documentation using default charting of nursing assessment was significantly less than the time required for handwritten documentation. The number of observations recorded by computer was significantly greater than the number of observations recorded by hand.
