Prevalence of HIV among childbearing women and women having termination of pregnancy: multidisciplinary steering group study.

OBJECTIVE: To determine the prevalence of HIV among pregnant women, in particular those whose behaviour or that of their partners put them at "low risk" of infection. DESIGN: Voluntary named or anonymous HIV testing of pregnant women during 21 months (November 1988 to July 1990). SUBJECTS AND SETTING: All women who planned to continue their pregnancy and attended clinics serving the antenatal populations of Edinburgh and Dundee. All women admitted for termination of pregnancy to gynaecology wards serving the pregnant populations of Dundee and outlying rural areas. MAIN OUTCOME MEASURES: Period prevalence of HIV antibody positivity. RESULTS: 91% of antenatal clinic attenders and 97% of women having termination of pregnancy agreed to HIV testing on a named or anonymous basis. HIV period prevalences for antenatal clinic attenders and women having termination of pregnancy tested in Dundee were 0.13% and 0.85% respectively, and for antenatal clinic attenders tested in Edinburgh 0.26%. For those at "low risk" rates for antenatal clinic attenders and women having termination of pregnancy in Dundee were 0.11% and 0.13%, and for antenatal clinic attenders in Edinburgh 0.02%. In Dundee HIV prevalence among women having a termination of pregnancy (0.85%) was significantly greater than that among antenatal clinic attenders (0.13%). CONCLUSIONS: HIV infection is undoubtedly occurring among women at "low risk," and it is clear that a policy of selective testing of those at only "high risk" is inadequate for pregnant women living in areas of high prevalence such as Edinburgh and Dundee. Moreover, when studying pregnant populations in such areas there is the need to include those having a termination of pregnancy.

Rapid effects of laminin on the growth cone.

To gain insight into how laminin promotes neurite growth, high resolution video microscopy was used to determine the rapid effects of laminin on growth cone structure. Sympathetic growth cones in serum-free medium on polylysine substrate displayed extensive motility and protrusive activity and often had large lamellipodia. However, their neurites grew slowly because membranous organelles from the central region advanced into the lamellipodium only slowly. Acute addition of laminin accelerated growth severalfold and had visible effects on the growth cone within minutes. Laminin dramatically accelerated the advance of membranous organelles, which, with microtubules, rapidly filled the lamellipodium. Retraction of individual protrusions (filopodia and veils) was rapidly reduced. These effects of laminin are important in accelerating growth and suggest a mechanism for pathway selection by growing neurites.

Estimates of HIV infection among injecting drug users in Glasgow, 1985-1990.

OBJECTIVE: To use research and surveillance studies in Glasgow (Scotland, UK) to estimate the number of current injectors infected with HIV, the total number of injectors infected up to the end of 1990 and the recent incidence of infection. DESIGN: (A) Prevalence of injecting drug use was estimated using log-linear modelling. (B) Prevalence of HIV infection was determined from voluntary testing of a community-wide sample of injectors. (C) The number of infected current injectors was predicted by combining the distributions generated by (A) and (B). (D) Data on known HIV-positive injectors were used in conjunction with (C) to forecast the cumulative number of infected injectors. RESULTS: The number of current injectors was estimated to be 9400; the prevalence of HIV infection among 447 injectors recruited to the HIV prevalence study during 1990 was 1.1%. From these data, the number of HIV-positive current injectors in 1990 was estimated to be between 52 and 138. Between 1985 and 1990, 110 known HIV-positive injectors were registered or received treatment in Glasgow for HIV-related diseases; the total number of cases estimated to have occurred during this period was between 110 and 300. The incidence of infection in Glasgow during 1990 was likely to have been low in light of the finding that only one case in the prevalence study had not previously been diagnosed HIV-positive. CONCLUSIONS: Linkage of datasets from a variety of sources and studies has enabled the substantial refinement of estimates of the number of injectors and the proportion infected with HIV in Glasgow up to 1990.

Q-switched ruby laser treatment of nevus of Ota.

Emission of 694-nm laser energy from a Q-switched ruby laser causes photodestruction of cutaneous pigment. The 40-nanosecond pulse duration of Q-switched ruby laser light initiates specific damage to melanosomes thus allowing selective treatment of benign pigmented lesions. Nevus of Ota is a benign facial oculocutaneous melanosis that has melanosomes lying deeply within the dermis. We report the successful use of the Q-switched ruby laser in the treatment of two patients with the nevus of Ota.

Microtubule-based filopodium-like protrusions form after axotomy.

The growth cone at the front of a growing neurite often has F-actin-rich structures--digitate filopodia and sheet-like veils and lamellipodia--whose protrusion advances the leading edge. Microtubules and other cytoplasmic constituents later fill the protruded area, transforming it into new neuritic length. Growth can be initiated from an axon by transecting it. We have used video-enhanced contrast-differential interference contrast microscopy to observe the early events following transection of Aplysia axons in culture. Many filopodium-like protrusions (FLPs) grew rapidly (average instantaneous velocity of 1.6 microns/sec) from the sides and end of the axon stump within minutes of transection. Some of these displayed bidirectional transport of swellings, at a rate similar to fast axonal transport. Dihydrocytochalasin B, which blocks actin polymerization, only halved the number of FLPs that formed within 10 min of transection, and actually increased the number of transporting FLPs. Nocodazole, a microtubule-specific drug, also halved the number of FLPs, but none of them displayed transport of swellings. No FLPs formed in the presence of both drugs. In transected axons that had not been exposed to either drug, removal of the plasma membrane revealed fibers in many of the FLPs; immunofluorescence showed these fibers to be microtubules. Thus, a substantial number of the FLPs that form soon after axotomy are microtubule based, rather than actin based, underscoring the potential of microtubules to drive the rapid extension of neuritic precursors.

A system for surveillance of voluntary HIV testing: results of the first 2 years, 1989-1990.

OBJECTIVE: In 1988, in order to improve the already established surveillance of AIDS and HIV-infected cases in Scotland, UK, the Communicable Diseases (Scotland) Unit [CD(S)U], in collaboration with microbiologists responsible for HIV testing, instituted a routine system for the epidemiological surveillance of all voluntary HIV tests. METHODS: To facilitate information capture, a standardized HIV request form was introduced for use throughout the country by clinicians requesting an HIV test. In addition, a network of laboratory computers, operated by clerical personnel, was established for the collection and collation of data. RESULTS: In 1989, of 9483 individuals tested for HIV, 129 (1.4%) were HIV-antibody-positive compared with 130 (1.2%) of 11,111 tested in 1990. A comparison of 1989 with 1990 rates of HIV among specific population groups, including injecting drug users (4.1%, 1989; 2.7%, 1990), homosexual men (5.2%, 1989; 4.5%, 1990), heterosexual men and women with high-risk partners (1.6%, 1989; 1.8%, 1990) and heterosexual men and women with lesser risk (0.3%, 1989; 0.5%, 1990), revealed no statistically significance differences at a 95% level of confidence. CONCLUSION: We believe that this surveillance system is the first of its kind to be implemented on a nationwide basis. The first 2 years' findings suggest a degree of stability in new transmissions of HIV occurring in Scotland. However, the increasing numbers of those known to be HIV-infected continue to cause considerable concern: 1943 individuals were reported to CD(S)U as HIV-antibody-positive by December 1991.

Substrate-bound factors stimulate engorgement of growth cone lamellipodia during neurite elongation.

The surfaces on which neurons grow greatly affect neurite elongation, but it is unclear how substrates influence the events within the growth cone that bring about elongation. Neurite elongation by Aplysia californica neurons in culture occurs through a series of transformations of the structures of the growth cone (Goldberg and Burmeister, J. Cell Biol., 103:1921-1931, 1986). The growth cone produces actin-rich protrusions, veils, and lamellipodia, which can then mature into the central body of the growth cone through the net advance of microtubules and membranous organelles from contiguous central regions, a process called "engorgement." Aplysia neurons form growth cones on poly-l-lysine-treated substrates, but their rate of neurite elongation is greatly enhanced on substrates additionally exposed to Aplysia hemolymph. The acute application of hemolymph to slowly growing neurites brings about a rapid acceleration of neurite elongation and engorgement. The enhancement of engorgement was effected with material eluted from hemolymph-treated substrates and was not seen when hemolymph was added to neurons cultured on hemolymph-treated substrates inactivated by exposure to UV radiation. Thus, we conclude that the rapid acceleration of engorgement caused by hemolymph is, in large part, a substrate-mediated effect. We propose that extracellular substrate molecules can modulate the rate of neurite growth through the regulation of the engorgement of lamellipodia. The microtubule disrupters colcemid and nocodazole inhibit the advance of vesicular elements into the lamellipodia following hemolymph treatment, but taxol, which promotes the polymerization and stabilization of microtubules, does not itself enhance engorgement. The microfilament disrupter cytochalasin B, however, stimulates engorgement. Our results suggest that regulating the resistance of the peripheral actin meshwork to penetration by microtubules and vesicles may be a mechanism by which substrate-attached molecules regulate neurite advance.

Cytochrome oxidase activity as a measure of synaptogenesis by multifunctional neuron L7 of Aplysia.

Neuron L7 of the marine mollusc, Aplysia californica, is unique in that it innervates five different target tissues in the animal. We show that when L7 is grown in vitro with two of these targets, that is, muscle cells isolated from the auricle or the gill vein, newly formed L7 neurites contact the muscle cells. Chemical synapses are formed since intracellular stimulation of L7 elicits contraction of individual muscle cells. Interestingly, auricle muscles are also innervated by neuron RBhe and co-cultures of RBhe and auricle muscle cells also exhibit synapse formation. To explore the molecular basis for synaptogenesis between L7 and its targets, it would be useful to quantify the extent of synapse formation in vitro, that is, to determine how many muscle cells can be innervated by a single L7. We show that this can be attained by staining for cytochrome oxidase activity. Cultures of auricle and gill vein muscles were exposed to the appropriate neurotransmitter in order to elicit contraction. The cells were then fixed and stained. In both cases, only cells that contracted were stained and electron microscopy showed reaction product associated with the cristae of mitochondria. When this procedure was applied to cultures of L7 and muscle cells, 38 +/- 2.8% (S.E.M.; n = 7) of the cells on the neurites were stained and therefore responded to L7 stimulation. Thus, part of the L7-RBhe circuit can be assembled in vitro and the extent of synaptogenesis can be accurately quantitated.

Urological manifestations of HIV-related disease. A case of AIDS-associated testicular seminoma, Kaposi's sarcoma and possible intracranial lymphoma.

A wide variety of pathologies afflicting the genitourinary tract can be displayed by patients infected with the human immunodeficiency virus (HIV), including both infective and neoplastic conditions. Treatment can often be offered for such conditions using a combination of therapeutic approaches. These conditions should be looked for specifically in patients with HIV infection as treatment may improve their quality of life. A case is presented of a testicular seminoma (appearing in association with an intracranial mass and Kaposi's sarcoma) in a male patient with the acquired immune deficiency syndrome (AIDS); he made a good response to orchiectomy and radiotherapy for his seminoma. A short review is also given of the range of HIV-associated genitourinary pathologies which have been described to date.

Pyogenic granuloma in children. Treatment with the flashlamp-pumped pulsed dye laser.

Pyogenic granuloma, a rapidly developing, benign vascular nodule often arising in children at sites of trauma in the skin or mucous membrane, was treated successfully with flashlamp-pumped pulsed dye laser in three pediatric patients. Excellent cosmetic results were obtained. The authors suggest that the flashlamp-pumped pulsed dye laser be considered as yet another effective modality in the treatment of pyogenic granuloma.

Hemangioma

Squamous cell carcinoma arising in a site of chronic osteomyelitis. Treatment with Mohs micrographic surgery.

Squamous cell carcinoma (SCC) arising in a focus of chronic osteomyelitis is a rare but potentially fatal disease. Because of the possibility of metastases, amputation proximal to the site of the tumor is the current standard of therapy. We report the first case of SCC arising in chronic osteomyelitis treated by Mohs micrographic surgery as an alternative method of treatment.
