Olsalazine in the treatment of active ulcerative colitis: a placebo controlled clinical trial and assessment of drug disposition.

The efficacy, safety and disposition of olsalazine was assessed in patients with left-sided ulcerative colitis or proctitis in a double-blind placebo controlled trial. Thirty patients with a mild-to-moderate attack of ulcerative colitis were randomly allocated to olsalazine capsules, 1 g b.d., or placebo for 6 weeks. Good clinical response was found in six patients receiving olsalazine and in two receiving placebo. Improvement in sigmoidoscopic findings and histological appearance of rectal biopsies was also seen more often in olsalazine-treated patients. Plasma concentrations of olsalazine were significantly higher in patients who improved. Olsalazine showed an advantage over placebo which needs to be confirmed by further studies; it was safe in sulphasalazine-sensitive patients but appeared to cause watery diarrhoea in two patients.

Breed differences in the frequency of bovine lymphocyte antigens.

Lymphocytes from 1,564 cattle of 18 breeds and cross-bred groups in Australia were tested for major histocompatibility system class 1 antigens. Gene frequencies were calculated for the Angus, Belmont Red, Brahman, Hereford and Holstein-Friesian breeds. There were substantial differences among these breeds in antigen and gene frequency. There were striking differences among all 18 breeds in the presence or absence of certain antigens. Two antigens, CA13 and CA36, were strongly associated in Hereford cattle but occurred independently of each other in the other breeds.

Long term management of hemorrhagic esophagitis with cimetidine and omeprazole.

We report a patient whose severe recurrent anemia was due to overt and occult hemorrhage from peptic esophagitis. Cimetidine treatment was successful for seven years but dose reduction was followed by intractable hemorrhage which did not respond to H2-receptor antagonists. Omeprazole therapy produced dramatic remission of symptoms and anemia.

Comparison of lamb production from indigenous and exotic x indigenous ewes in Indonesia.

Lamb production in relation to total feed consumption was recorded in Javanese thin tail (JTT) ewes and in first cross Suffolk x JTT (SX), Wiltshire Horn x JTT (WX) and Poll Dorset x JTT (DX) ewes over three successive lambings or for 21/2 years after joining with rams of the same genotypes in a continuous mating system. For the JTT, SX, WX and DX genotypes respectively, total numbers of lambs weaned per ewe joined were 3.77, 2.50, 2.08 and 1.33, mean lamb body weights at weaning were 12.8, 18.8, 18.6 and 20.9 kg, total weights of lamb weaned per ewe joined were 48.4, 46.9, 38.8 and 27.9 kg, and average weights of feed consumed by the whole production system per kg of lamb weaned were 16.4, 21.9, 23.5 and 32.8 kg. Thus cross-bred ewes required more feed to produce fewer though heavier lambs without any real increase in total productivity.

Altered solid and liquid gastric emptying in patients with duodenal ulcer disease.

Alteration in gastric emptying has been implicated in duodenal ulcer disease. The precise abnormalities remain controversial. We have used a radionuclide technique to assess solid and liquid gastric emptying in 14 patients with endoscopically proven duodenal ulcer and 22 healthy controls. Solid gastric emptying values for the patient group fell within the normal range. The median time taken for 50% (T50) of the liquid marker to empty from the stomach was 12 minutes (range 6-23 minutes) which was significantly faster (p less than .005) than controls (median 18 minutes, range 11-35). In 10 of the 14 patients, however, the rate of liquid emptying was within the normal range. There was no significant difference in the T50 for gastric emptying of solids between the groups, but in duodenal ulcer patients food left the stomach significantly earlier than in controls (p less than .05). After this, however, the linear rate at which duodenal ulcer patients emptied solid food from the stomach was a median 0.75%/minutes (range 0.5-1.4 minutes), which was slower (p less than .0005) than controls, median 1.25/minutes (range 0.7-2.3). These results show that the pattern of gastric emptying of digestible solids and liquids in patients with duodenal ulcer disease, as a group, is significantly altered.

Fulminant hepatic failure.

Fulminant hepatic failure is an uncommon but dramatic illness with a high mortality. Viral hepatitis or drug toxicity is the usual cause. In the severe forms of the illness the patients, often young and previously healthy, rapidly become profoundly unwell due to extensive hepatic necrosis. Coma is accompanied by a disastrous disturbance of all the synthetic, metabolic and excretory functions of the liver. Management is largely supportive and multisystem complications may demand all the resources of the intensive care unit to allow survival. Systems for temporary hepatic support have aroused interest, based on the assumption that the hepatic lesion is potentially reversible. A number of heroic techniques have been tried and abandoned. Encouraging reports of charcoal column haemoperfusion have yet to be confirmed by controlled clinical trial.

Comparative performance of intensively managed Khaki Campbell and native Indonesian ducks.

Egg Production traits of Khaki Campbell, Alabio, Bali and Tegal ducks were measured to 68 weeks of age under intensive management. Khaki Campbell ducks laid significantly more eggs and required less feed per egg or per gram of egg produced than the Indonesian breeds. Appreciable numbers of local ducks were observed to moult on two occasions during the trial. It was thought that this may have been a response to seasonal factors such as rainfall patterns. Tegal ducks laid significantly fewer eggs and were less efficient than Alabio and Bali ducks. Egg specific gravity was consistently highest in the Alabio and Tegal breeds. The poor production of Tegal ducks compared with that in other studies may have been related to their early onset of lay under the 16 h daylength used in the trial or to a sampling effect.

The effect of collection method and housing system on semen production and fertility of Alabio drakes.

Three methods of semen collection from Alabio drakes were compared: the use of an artificial vagina (AV), electro ejaculation (EE) and manual massage (MM). For the latter two methods, drakes were housed in individual cages or in groups in floor pens, while for the AV method all drakes were kept in individual cages. Training success rates of drakes for the three collection methods were similar. Drakes housed in cages produced significantly more semen with a higher sperm concentration, which maintained fertility longer than semen from floor-penned drakes. The average semen volume per ejaculate, sperm concentration and total number of sperm per ejaculate were highest for the AV method while the other two methods did not differ. The average duration of fertility was significantly longer for the AV method (8.8 d) than the EE (6.5 d) or MM methods (5.1 d). The proportion of fertile eggs for 7 d following the insemination of 2 X 10(8) spermatozoa was also highest for the AV method. Although greater skill was required from the operator, the AV method is recommended for general use because of its significantly higher semen yield and longer duration of fertility.

Omeprazole inhibition of nocturnal gastric secretion in patients with duodenal ulcer.

We studied the effect of single 08.00 h doses of omeprazole or placebo on gastric acid secretion during the following night, 14 to 23 h after administration, in seven male subjects with duodenal ulcer. The drug was given orally, double-blind, in randomized order. Omeprazole 20 mg, 40 mg and 80 mg reduced mean total overnight acid output by 43%, 73% and 91% respectively and median pH increased from 1.4 with placebo to 1.6, 3.1 and 7.0 respectively. The inhibitory effect was maintained throughout the study period. No clinical side effects or abnormalities of laboratory screening tests were seen. Omeprazole is well tolerated and administration at 08.00 h produces prolonged dose related inhibition of acid output during the following night.

The effect of omeprazole on gastric emptying in patients with duodenal ulcer disease.

The effect of a single oral dose of omeprazole (90 mg) on gastric emptying was assessed with a dual isotope scintigraphic technique in eight patients with a history of duodenal ulcer disease. Omeprazole had no significant effect on solid or liquid gastric emptying. The bioavailability (AUC0-2 h) of omeprazole in all patients was greater than that required for total suppression of acid secretion. No adverse clinical or laboratory effects were observed.

Symptomatic and asymptomatic ulcer recurrence following highly selective vagotomy.

An assessment of outcome has been made after highly selective vagotomy performed by two surgeons on 174 patients. The median time to followup was 36 months (range 3-94). The group comprised 74% males and 26% females (median age 45 years, range 19-75). Assessment was by review of medical history, endoscopy (irrespective of presence or absence of symptoms) and questionnaire. Symptomatic ulcer recurrence had already been diagnosed in 16 patients (9%). There were 2 unassociated deaths and 14 patients remained untraced. The remaining 142 patients were assessed by questionnaire and asked to undergo an endoscopy. One hundred and nineteen agreed to endoscopy and ulcers were found in 11 patients (9%). Five were sited in the duodenum and 6 in the stomach. Duodenitis was found in 30% of patients. Patients with ulcers in the pyloric region were at significantly greater risk than the general group for developing a recurrence. Patients who agreed to endoscopy (119) underwent Visick grading prior to endoscopy. A poor correlation was found between ulcer recurrence and Visick score. The "asymptomatic" recurrence rate of 9% following highly selective vagotomy should be considered when assessing the outcome of the surgery.

Time relationships between oviposition, ovulation and egg formation in Khaki Campbell ducks.

1. Ten-month-old Khaki Campbell ducks were killed between 5 min and 15 h after oviposition. Time of oviposition and interval between eggs were recorded prior to killing. 2. Oviposition generally occurred between 04.00 and 06.00 h, 7 to 9 h after the onset of the dark period the previous day. Ninety-seven percent of eggs were laid by 07.00 h. 3. The mean +/- SD time interval between consecutive ovipositions was 24.0 +/- 0.3 h, with a range from 23.5 to 24.5 h. 4. It was estimated that ovulation occurred on average 10 min after oviposition, and the ovum spent 15 to 30 min in the infundibulum, 2.5 to 3 h in the magnum, 2 to 2.5 h in the isthmus and 18.6 h in the shell gland.

The influence of smoking on the healing of duodenal ulcer treated with oxmetidine or cimetidine.

The influence of smoking on duodenal ulcer healing was examined during a double blind study of 83 patients randomly allocated to oxmetidine or cimetidine treatment. Smoking habits were recorded but patients were not advised to change these. Smokers and nonsmokers were similar clinically and did not differ in compliance with medication. Both H2-receptor antagonists were equally effective and after four weeks of treatment ulcers were healed in 76% of patients. Ulcer healing occurred significantly less frequently in smokers (69%) than nonsmokers (89%). Smokers with healed ulcers consumed fewer cigarettes per day (mean +/- SE: 15.8 +/- 1.4) than those whose ulcers did not heal (22.2 +/- 2.7). When smokers were grouped according to daily cigarette consumption, a direct relationship was found between increasing cigarette consumption and decreasing frequency of ulcer healing. No reduction in ulcer healing was apparent in patients who smoked nine cigarettes a day, or less.

Is there an HLA antigen association with duodenal ulcer?

To assess the influence of ethnic origin on the incidence of HLA antigens in duodenal ulcer, we typed 160 white Australian patients (including 22 born in Greece or Italy) and 320 blood donor controls for the HLA-A and -B loci. 5 of the 22 (22.7%) southern European patients, but only 19 of the 138 (13.8%) of the remaining duodenal ulcer patients were found to have antigen B5 compared with 27 (8.4%) of the controls. HLA-B12 was found in 55 (34.4%) duodenal ulcer patients (including 6 of the 22 southern European patients) and 117 (36.6%) controls. No significant association of duodenal ulcer with HLA-B5 or HLA-B12 was found when differences in racial origin of patients were taken into account.

Long-term treatment of duodenal ulcer with cimetidine. Intermittent or continuous therapy?

Forty-eight patients with chronic duodenal ulcers which were healed with cimetidine were allocated at random into two equal groups to assess different ways of using cimetidine during one year of treatment. Twenty-four patients received intermittent six-week courses of cimetidine for each relapse, and 24 patients were treated with maintenance administration of cimetidine (400 mg twice a day) continuously. Only one patient in the group receiving continuous therapy suffered clinical recurrence, but asymptomatic ulceration was found in four others. The group of patients who were receiving intermittent therapy suffered a total of 36 clinical recurrences. Three of these patients required prolonged treatment to heal their ulcers, and seven developed asymptomatic ulcer. The number of relapses varied from none to five. No way of predicting the individual prognosis was found. Intermittent treatment was an acceptable alternative in approximately half of the patients treated in this way, and was a failure in one-quarter of the group.
