[Dupuytren's disease. Review of 326 surgically treated patients].

The COC (Orthopedic Society of Charleroi) has presented an analysis of operated cases. We have taken 326 files into account. The pre-operative evaluation is based upon the classification by Tubiana, Michon and Thomine. The analysis pays special attention to the post-operative results in terms of several factors: the age, the stage of illness, the extent of illness and the improvement. For this final summing up, we put forward criteria which for us are better in the evaluation of the final result.

Biochemical investigations after burning injury: complement system, protease-antiprotease balance and acute-phase reactants.

Seventeen burned patients were investigated--Group I (n=10) with a mean burned area expressed as unit burn standard (UBS) of 69 +/- 24 and Group II (n = 7) with a mean UBS of 23 +/- 8. Blood samples were collected immediately after admission, 6-12 h after injury, during the morning and evening of day 1, and then daily for 2 weeks. This prospective study demonstrated complement activation in vivo in all burned patients, measured by C3d/C3 ratio index which was not related to the extent of the burned surface. A significant protease-antiprotease imbalance, correlated to the severity of burns, was found, leukocyte elastase was increased throughout the observation period, alpha 2-macroglobulin drastically decreased in severely burned patients, and alpha 1-proteinase inhibitor promptly decreased below the normal level in patients with more than 40 UBS. Finally, there was a delayed but then persistent acute-phase reactant protein response involving C-reactive protein, haptoglobin and alpha 1-acid glycoprotein, the concentrations of which reached a plateau on days 6 or 7.

[Cancer of the prostate: current status of chemotherapy].

Life expectancy of patients with prostatic cancer is varying from one case to another and, sometimes, these patients have a very long life expectancy. Less than a third of this patients are dying from their cancer. This notion implies to be very careful in the choice of aggressive treatments. Isolated or combined chemotherapy assays permitted to select cyclophosphamide, methotrexate, cis-platinum or estramustine phosphate as active drugs in the treatment of prostatic cancer. But the randomized studies, in Europe or in U.S.A., are not demonstrative enough to propose these drugs for a routine treatment. Cancer of the prostate is usually represented by cellular groups that are more or less sensitive to the different kind of available treatments. Perhaps in the future the best results of treatment will be the association of chemotherapy-radiotherapy and hormonotherapy.

[Facial injuries. Classification].

The present paper was intended as an introduction to the symposium organized in Paris on 4 October 1990 by the Royal Belgian Society for Surgery under the heading "Eurosurgery 90". The anatomical and functional properties of the facial bones and soft tissues were recalled. The face is the support of the main social functions of Man. The classification makes a distinction between fractures of the middle third and of the mandible. Among the formers, classical fractures of Le Fort I, II and III, are now less frequent than asymmetrical or complex fractures with multiple soft tissue lacerations. The main types of mandibular fractures are described. Part of these may show important displacements, due to the insertion of strong muscles. The aims of treatment are defined and to-day technology briefly described. Tendency is to shorten the duration of intermaxillary fixation thanks to the more recent procedures of internal fixation.

[Facio-craniostenosis].

Facio-craniostenosis. Facio-craniostenosis is associated with premature stenosis of one or several cranial sutures. The clinical variety is defined by the affected suture. In the complex forms, major disturbances of the anterior fossa of the cranial base are observed. Intracranial hypertension sometimes observed may lead to impairment of the function of the brain or the eyes. The authors have observed 73 children with craniostenosis and have operated upon 49 of them. Three types of methods have been used. The stress is made on the latest one combining advancement of the orbito-frontal "bandeau" and the regularization of the vault. Operating early is essential for prevention of functional sequellae. The optimal age for surgical treatment seems to be between 3 and 6 months. Complications have been remarkable benign.

Age Factors

[Role of a surgical emergency department in the management of a disaster bringing a great number of patients with burns].

Definition of a disaster would follow prescriptions from the "International Trauma Foundation" (Brighton 1981, document A). This disaster may be: simple (where the structure of community remains intact), compound (where the structure of community and function of the community are disrupted). The classification of disasters is realized: by cause, by number of casualties, by nature of pathology, by time while cause operating, by time of rescue procedures at site, by affected area, by rural or urban location. Belgian legislation about disaster is described. Disaster related to major technological risks will be characterized by high probability of traumatic and burns injuries. The authors prepare medical responses to this eventually, defining the role of a "base hospital", the role of a medical team at site and the management of different groups of burned patients. Propositions are made to bring together actors of "disaster plan".
