[Operative treatment of osteochondritis dissecans of the knee joint].

39 Patients out of 51 who have been operated on because of osteochondrosis dissecans of the knee-joint had a follow-up after a mean observation period of 8 years. In 17 patients the removal of the dissecate was the only operative procedure, in 11 patients a mouse-bed smoothing was simultaneously performed. The excision of the dissecate out of the mouse-bed with a simultaneous smoothing of the margin was performed in 11 cases. Postoperatively 28 patients were absolutely free of symptoms and the weight-bearing was not restricted. The follow-up revealed low-grade symptoms dependent on weight-bearing in 7 patients. A connection between the modifications of the operative procedure and the result of the operation could not be established. Signs of an arthrosis, already preoperatively detectable, and excessively spread osteonecrotic foci had an unfavourable influence on the result.

[Value of the orthopedic examination of newborn infants with regard to the diagnosis of hip dislocation].

Out of a total of 10,617 new-born babies examined, 11.1 percent required supervision as a result of clinical signs pointing to a congenital dislocated hip joint, and 6 percent as a result of anamnestic indications. X-ray follow-up examinations after four months showed pathological hip joint findings in 27.7 percent of the patients under supervision. Out of a total of 1,587 new-born babies without pathological findings (both clinical and anamnestic), 1.2 percent showed pathological hip findings in an X-ray examination performed at the age of four months. The overall incidence of congenital dislocated hip joints was found to be 5.8 percent. The results point to the importance of regular and thorough hip examinations in child-welfare centers even after earlier checks on new-born babies which revealed no pathological findings.

Follow-Up Studies

[Arthrodesis of the knee joint (author's transl)].

Arthrodesis of the knee joint was performed in 75 patients. Solid fusion was achieved in 74 out of them. Average time for union was basically dependent upon type of pathological process and not upon fusion technique employed. In the last years compression arthrodesis with internal fixation by plates was used in all cases of degenerative, posttraumatic and rheumatoid arthrosis; in cases of tuberculosis and other infections of the knee joint external osteosynthesis by fixateur externe was the preferred procedure.

Adult

[Current viewpoints on osteoarticular tuberculosis].

Nowadays the osteoarticular tuberculosis is a disease of adult and senile age. Among my own patients were most frequently affected the spinal column (55.7%), the hip-joint (10.1%) and the knee-joint (9.4%). If other diagnostic methods fail, to the interest of an early ascertainment of the diagnosis the performance of a test excision should not be delayed. On modern conditions a conservative treatment is sufficient in most cases and achieves good results. Here, the chemotherapy follows the usual directives, in which cases on account of the lacking proof of causative organisms frequently must be issued from the probable position of resistance. Excluding early diagnosed, purely synovial tuberculoses of the joint in young patients, even nowadays every tuberculous skeletal focus needs immobilisation. In case that conservative therapy does not lead to an improvement of the findings, operative measures are indicated. Here, the removal of the focus is the first place. A final solution of the problem of the osteoarticular tuberculosis is to be expected above all within the general combat against tuberculosis.
