Pressurization and acute mountain sickness.

Numerous cases of acute mountain sickness (AMS) during trekking were reported to have been successfully treated with portable pressure chambers. The effect of early pressurization during acute altitude exposure in the Alps had not been previously studied. In order to test the hypothesis that an early pressurization of unacclimatized subjects for 3 h could prevent or delay the appearance of symptoms of AMS, 51 previously healthy subjects climbed from 1,030 to 4,360 m within 12 h. Upon arrival at 4,360 m, AMS scores (Lake Louise Consensus Questionnaire '91), oxygen saturation (SaO2), and heart rate (HR) were determined at rest. The subjects were then randomly divided in two groups; one group was pressurized to 200 mBar for 3 h while the other rested. AMS score, HR, and SaO2 were similar in both groups before treatment. AMS score had decreased (from 2.44 +/- 0.41 (S.E.) to 0.89 +/- 0.26, p < 0.05) and SaO2 had increased (from 75.22 +/- 1.32% to 79.07 +/- 1.27%, p < 0.05) in the treatment group 15 min after leaving the pressure chamber whereas the control group had unchanged AMS score (2.50 +/- 0.40 vs. 2.40 +/- 0.40, N.S.) and SaO2 (77.83 +/- 1.41 vs. 76.67 +/- 1.24, N.S.). The next morning, however, AMS score, HR, and SaO2 were similar for both groups. It is concluded that during acute ascent in the Alps, an early 3-h pressurization of unacclimatized subjects does slightly delay the onset of AMS but does not prevent the illness nor does it attenuate its severity upon appearance.

Stellar corneal rupture and secondary glaucoma after squash trauma in a keratotomized eye.

We report an unusual dramatic case of stellar ocular rupture after squash blunt in a recently keratotomized eye through weakened keratotomy scars. The early secondary glaucoma, the tension generated by the surgical sutures of the wounds and the use of systemic and topical steroids could explain the reopening of two incisions initially unperforated and the delayed healing procedure. We also stress the benefits of wearing protective eye glasses when playing squash, as well as a strict legislation in Belgium in such an eye-threatening sport.

Hyperthyroidism, ophthalmoplegia and unilateral mydriasis.

Clinical description of a 33 years-old woman, with hyperthyroidism, admitted to the emergency unit with external bilateral ophthalmoplegia and left mydriasis, unreactive to light. The external ophthalmoplegia is proven to be due to myasthenia. The left mydriatic pupil demonstrates features typical for Adie's tonic pupil. The association of a tonic pupil with an auto-immune disease is infrequent. The axiom that internal ophthalmoplegia in a patient with external ophthalmoplegia excludes myasthenia gravis should be reevaluated.

[Somatostatin in a case of neonatal hyperinsulinism caused by focal adenomatosis of the head of pancreas].

The use of somatostatin in the treatment of a newborn infant with hyperinsulinism is reported. When administered alone, somatostatin was only able to prevent hypoglycemia for a short period of time, whereas the addition of a constant infusion of glucagon allowed successful control of the patient's hypoglycemia. As hyperinsulinism relapsed after a subtotal pancreatectomy, a trial was carried out with a somatostatin analog, which has an expected longer duration of action. It led to a significant rise in the blood glucose level but failed to prevent safely hypoglycemia even when 4 injections were performed daily.

[Diagnosis of neonatal Streptococcus B infection by the latex test].

The detection of group B streptococcal soluble antigens was performed in 139 newborn infants suspected of infection. A qualitative agglutination test of sensitized latex particles (Wellcogen Strep B) was performed in the urine. The diagnosis of group B streptococcal infection was assessed by retrospective analysis of patient files using previously defined criteria. The correlation between the agglutination test result and the presence or absence of streptococcal infection was statistically significant; the test had a sensitivity of 81%, specificity of 97%, positive predictive value of 76% and negative predictive value of 98%. This study confirms the interest of such a test in the early diagnosis of group B streptococcal infection in infants.

[Complications of varicella as a function of the terrain. Apropos of 103 cases].

The complications of varicella have been studied in 103 cases of hospitalized children. The patients have been divided into two groups: group I comprises 94 previously healthy children; group II is made up of 9 children who have been treated by corticotherapy for a long time. The frequency of the complications is not statistically different between the two groups. On the other hand, all the fatal complications are observed in group II (3 deaths out of 9 patients). This investigation is in accordance with published data which allows us to characterize a high risk population for life threatening varicella: patients with congenital or acquired cell immunity deficiency. This group requires special therapy: vaccination, specific immunoglobulins and acyclovir.

[Listeria monocytogenes nosocomial infection in the maternity ward].

Nosocomial infection with Listeria monocytogenes 4b occurred in January 1990 in a maternity hospital in Grenoble. The 3 patients involved were born within a 24 hour-interval. The premature newborn responsible for contamination was asymptomatic. Two other newborns without any perinatal infectious risk presented with meningitis, one on the 5th day of life in the maternity hospital, the other one on the 11th day while already at home. The 3 strains of Listeria had the same serovar and lysovar. Epidemiologic investigations led to suspect a contamination in the delivery room and during the care of the children. Strict respect of hygiene orders is imperative to avoid nosocomial infections.

[Pseudo precocious puberty in the McCune-Albright syndrome].

McCune Albright Syndrome is characterized by the triad of polyostostic fibrous dysplasia of bones, skin pigmentation (café au lait-spots) and sexual precocity. The authors report a case of a 7 years girl with this syndrome in whom the sexual precocity is due to an autonomous hyperfunctioning of luteinized follicular cysts of ovary. The pathogenic mechanisms of this autonomy are debate.

[The effect of the formulation of a skin cream with a procyanidin polymer base and its effect on skin lesions caused by herpesviruses].

Procyanidin polymers are reported to be virostatic and virucidal in vitro. In this experiment we showed, on a hairless mouse model, biological effects of these agents, already demonstrated in vitro on cell cultures. Several dermic cream compositions have been tested and compared for reduction of HSV1 cutaneous lesions. In this study, it appeared clearly that active principle dosage form is as important as the biological effect of the agent. Composition and water content influence permeation speed, contact time with surface skin layers and consequently the pharmacological effect on herpetic skin injuries.
