Light-shift-induced quantum gates for ions in thermal motion.

An effective interaction between trapped ions in thermal motion can be generated by illuminating them simultaneously with a single laser resonant with the ionic carrier frequency. The ac Stark shift induces simultaneous "virtual" two-phonon transitions via several motional modes. Within a certain laser intensity range these transitions can interfere constructively, resulting in a relatively fast, heating-resistant two-qubit logic gate.

Cervical osteophytes: their significance in ENT practice (ankylosing vertebral hyperostosis--Forestier's disease).

The condition known as ankylosing vertebral hyperostosis or Forestier's disease is described. Eight cases are reviewed where the radiological findings warranted this diagnosis. Their presenting symptoms are summarized as are their responses to treatment. The relevance of this diagnosis is discussed with emphasis being placed on the importance of thorough investigation of these patients. In those for whom no other cause has been found for their symptoms, the various therapeutic options are outlined with the possible complications.

The predictive value of eustachian tube function (measured with sonotubometry) in the successful outcome of myringoplasty.

Sonotubometry, as a technique for assessing Eustachian tube function, does not rely on an intact tympanic membrane. This device has been used to study 25 patients having myringoplasty who all had preoperative testing and post-operative testing at 4 months. The overall success rate for grafting was 76%. In those who had normal preoperative tubal function the success rate was 86%, as opposed to 67% in those whose preoperative tubal function was poor. This difference was not statistically significant. In all those patients where poor preoperative function ended with successful grafting, post-operative tubal function reverted to normal. Other factors also taken into account, such as the patient's age, size of the perforation and the operative difficulty, had no bearing on the outcome of surgery.

Allergic aspergillus sinusitis--an overlooked diagnosis?

Aspergillus infection of the nose and sinuses has been recognized for nearly 100 years but a variant, allergic aspergillus sinusitis, has recently been recognized. This non-invasive form causes chronic sinusitis characterized by thick green inspissated mucus in which eosinophils and Charcot-Leyden crystals are found. Fungal hyphae are scanty and are identified with special stains. Consequently the diagnosis may frequently be overlooked. It has many similarities with allergic broncho-pulmonary aspergillosis and is treated by surgical debridement and aeration with or without cortico steroids, either topical or systemic.

Lipoid proteinosis: Urbach-Wiethe disease.

An otherwise healthy patient with Urbach-Wiethe disease required surgical removal of two 3rd molar teeth. In this multisystem disorder infiltration of the buccal, pharyngeal and laryngeal mucosa may cause difficulties with tracheal intubation and increase the likelihood of trauma. The anaesthetic implications and management are described.
