Simulation of organismic morphology and behavior by synthetic poly-alpha-amino acids.

Experiments imitating spontaneous geothermal occurrences have yielded most of the amino acids found in protein. All of the amino acids found in protein are simultaneously condensed, by heating in a range of appropriate conditions, to polymers which have many of the properties of proteins. These properties include molecular weights of many thousand, digestibility by proteolytic enzymes, and catalytic activities. One of the other properties is the tendency to form structured units; these units have many of the attributes of biocells. The processes indicated, and others, comprise a conceptual continuum which, according to accumulated information, must have occurred under the conditions existing in regions of the primitive Earth.

A statewide consortium initiative to establish an undergraduate clinical internship program.

The amount of time and the type of experience needed by undergraduate students in the clinical setting to actually practice nursing has been the subject of many debates between nurses in academe and service. Rather than blaming each other for the culture shock and transition problems of neophyte nurses, members of the academic and practice community in Rhode Island decided to address the problem together. The development of the collaborative program described in this article grew out of a research study and was coordinated through the efforts of nurses from service and academe. The central outcome of this statewide consortium's project was to increase the students' level of nursing competencies. "Turf" issues that related to the colleges and the hospitals involved became secondary. Everyone involved believed that students from different collegiate programs working together presented exciting opportunities to share and learn from one another. In addition, other outcomes included increased recruitment and retention of these baccalaureate-prepared nurses in the hospitals, and greater ease in their orientation and transition. In addition, a cluster of three research studies were conducted by members of the consortium. This ongoing program, in which nurses in education and service are working together in a statewide consortium to break down the turf issues that separated them, is achieving outcomes that could not have been accomplished had it been undertaken independently. Subunit structure of a potent platelet-activating glycoprotein isolated from the venom of Crotalus durissus cascavella.

The potent platelet-activating factor isolated from the venom of Crotalus durissus cascavella is an acid-soluble multisubunit glycoprotein of Mr 72,000 built up of two types of subunits, alpha and beta, linked by disulphide bonds. The mean apparent Mr of the reduced complex was around 12,000 by gel filtration under denaturating conditions. The Mrs of the alpha and beta subunits, with an apparent ratio of 1/1, were 12,600 and 13,580 by SDS-PAGE respectively. The Mr 72,000 glycoprotein is thought to be an alpha 3 beta 3 complex. The urea dissociated glycoprotein (Mr 72,000) retained its platelet-stimulating activity. It is concluded that the Mr 300,000 form isolated at acidic pH under native conditions, and showing a rosette - like, ring-shaped structure in the electron microscope as well as the Mr 144,000 form isolated at physiological pH under native conditions and active on platelets were the tetrameric and dimeric states of the molecule respectively.

Comparison of arbutamine stress 99mTc-labeled sestamibi single-photon emission computed tomographic imaging and echocardiography for detection of the extent and severity of coronary artery disease and inducible ischemia.

BACKGROUND: Arbutamine is a new synthetic catecholamine developed specifically for pharmacologic stress testing. METHODS AND RESULTS: We investigated 39 patients undergoing coronary arteriography to compare arbutamine stress (99m)Tc-labeled sestamibi single-photon emission computed tomographic imaging and echocardiography for detection of the extent and severity of coronary artery disease and inducible ischemia. Rest and stress studies were analyzed blindly according to a 12-segment left ventricular model for both techniques. Each segment was graded according to severity of wall thickening abnormality and perfusion defect (1 = normal to 4 = severe). Total perfusion defect and wall thickening scores were calculated at peak stress and the difference in scores between stress and rest (delta perfusion defect; delta wall thickening) were used as indexes of inducible ischemia. Twenty-one patients had multivessel disease, nine had single-vessel disease, and nine had normal coronary arteries. Diagnostic accuracies for the detection of coronary artery disease for single-photon emission computed tomographic imaging and echocardiography were 95% and 92%, respectively. Extent and severity of coronary artery disease indicated by a peak stress perfusion defect score of 26 +/- 6.4 and wall thickening score of 25.1 +/- 8.4 were similar, and there was no significant difference in the delta perfusion defect and delta wall thickening scores of 8.7 +/- 5.5 and 10.4 +/- 7.1, respectively. Segmental concordance rates for the detection of coronary artery disease and inducible ischemia were 74% (K = 0.47; confidence interval 0.39 to 0.55) and 74% (kappa = 0.42; confidence interval 0.34 to 0.51), respectively. Regional concordance for coronary artery disease was 84% (kappa = 0.68; confidence interval 0.51 to 0.84). Where discordance was present, there was a greater prevalence of perfusion abnormality compared with wall thickening abnormality. CONCLUSION: Arbutamine stress single-photon emission computed tomographic imaging and echocardiography provide largely equivalent and accurate pathophysiologic information for the evaluation of coronary artery disease and inducible ischemia.

Fulminant cutaneous T-cell lymphoma and Sézary syndrome.

The term cutaneous T-cell lymphoma (CTCL) has recently been coined to describe a group of rare lymphoproliferative T-cell disorders classically beginning in the skin and pursuing a chronic, progressive, and indolent course eventually followed by visceral involvement. This term encompasses the spectrum of the traditional mycosis fungoides and Sézary syndrome. A unique case of CTCL that developed a fulminant course leading to death within a few months following the initial symptoms is presented. The clinical course bears a striking similarity to a group of acute T-cell malignancies, while the cytologic-pathologic findings favor the diagnosis of Sézary syndrome. Regardless of the exact classifications, this entity appears to be a highly malignant T-cell disorder, resistant to chemotherapy.
