Solitary ulcer syndrome of the rectum--a histopathologic characterisation of 33 biopsies.

Histological analysis of 33 rectal biopsies from 26 patients of solitary ulcer syndrome (SUS) of rectum was undertaken. Fibrous obliteration and smooth muscle extension into the lamina propria were the characteristic and most constant findings. The limitation of the biopsy procedure in procuring deeper tissue made the detection of colitis cystica profunda rare in our study. A combination of fibrosis of the lamina propria, superficial mucosal ulceration, muscularis mucosae hypertrophy and submucosal fibrosis, along with the clinical suspicion should clinch the diagnosis of SUS.

Relative merits of various rapid biopsy urease tests for diagnosis of Helicobacter pylori (Campylobacter pylori).

Three rapid urease tests, i.e., liquid urea broth containing phenol red as indicator, liquid urea broth containing bromothymol blue as indicator and CLO gel were compared in 109 patients of dyspepsia for the diagnosis of Campylobacter pylori (Helicobacter pylori) infection. Mean time taken for positive reaction in liquid broth with phenol was 3 minutes (range 0.6 to 5.3 minutes) with bromothymol blue was 3.5 minutes (range 0.4 to 5.5 minutes) while with CLO gel it was 101 minutes (range 11-261 minutes). There was no difference in results of liquid urea broth containing phenol red and bromothymol blue. The difference in timing of urea broth containing phenol red and bromothymol blue was statistically significant as compared to CLO gel (p less than 0.05). Rapid urease tests employing liquid urea broth are quick, simple and reliable for the diagnosis of Helicobacer pylori infection.

Endoscopic management of chronic organoaxial volvulus of the stomach.

Endoscopic correction of the chronic organoaxial volvulus of the stomach was attempted in seven cases of primary and three cases of secondary volvulus. Endoscopic correction was successful in six cases of primary volvulus and one case of volvulus secondary to duodenal carcinoma. This paper describes the details of the technique of endoscopic correction of gastric volvulus, and documentation of correction of the volvulus by barium meal study with a follow-up of 5-26 months.

Duodeno--jejunal vascular malformation.

Vascular malformations are rare but an important cause of obscure upper gastrointestinal bleeding. A patient with a large vascular malformation involving the duodenum and proximal jejunum, presenting with a severe bout of melaena, is described. The diagnosis was suspected on endoscopy and confirmed by angiography and surgery.

Endoscopic removal of impacted Sengstaken-Blakemore tube.

In developing countries, the Sengstaken-Blakemore tube (SB tube) is one of the common treatment modalities for controlling hemorrhages from bleeding esophageal varices. Its use is accompanied by numerous complications. An unusual complication is impaction of a SB tube and its endoscopic removal is described.

Chloroquine phosphate induced haematemesis.

Two cases who developed haematemesis, 30 and 36 hours after ingestion of 4 tablets (600 mg base) of chloroquine phosphate are described. Upper gastrointestinal endoscopy revealed erosions in the stomach and duodenum.

Pseudomyxoma peritonei. A report of three cases.

Pseudomyxoma peritonei is an infrequently encountered clinical entity which presents mainly with abdominal distension. We present here our experience with three such cases and emphasize on the aggressive combined operative and chemotherapeutic approach in this disease.

The emergence of colonic diverticulosis in urbanised Indians. A report of 23 cases.

Of a total of 969 barium enemas performed over a three year period, 23 (2.4%) showed colonic diverticulosis. The mean age of the patients was 58 years and the commonest site involved was the sigmoid colon. All the patients were urbanised and were obligatory vegetarians. Six (25%) of the patients had a complication of the disease and three of them required surgery. The possible relationship of the disease with socio-economic changes is highlighted.

Delta hepatitis infection in north India. A preliminary report.

There is a paucity of data on occurrence of delta hepatitis in India. We here present our preliminary data showing that delta coinfection and superinfection occur quite frequently in our set up. The coinfection rate was found to be 15% and superinfection rate 26.6% in the initial screening of 69 patients of acute and chronic liver disease.
