[Metabolic effects of maternal hyperventilation in obstetrical anaesthesia induction (author's transl)].

Apparative hyperventilation before intubation brings about a favourable blood gas situation, but also a bicarbonate shortage, causing metabolic acidosis with increase of blood lactate and pyruvate. In obstetrics a mother-fetus infusion of lactate can occur, which can produce fetal distress. Because of the relative immaturity of the enzyme systems in the newborn, degradation of the infused lactate proceeds very slowly. These children recover only gradually over a period of several hours. For this reason it appears advisable to hyperventilate only briefly before a quick obstetrical intubation.

Early primary diagnosis of ovarian cancer and detection of recurrence by serum cystine aminopeptidase assay.

The aim of this study was to correlate the relationship between serum cystine aminopeptidase (CAP) activity in normal patients and that in patients with primary ovarian cancer of recurrence. Sera obtained from 63 patients and evaluated for CAP activity by the method of Babuna and Yenen were grouped as follows: in 20 normal, nonpregnant patients, the mean activity was 25; in 14 patients with fibromas of the uterus, it was 38; in two patients with ovarian sarcoma, the activities were "normal" (24 to 27); and in 27 patients with primary ovarian adenocarcinoma or recurrence, the activity corresponded to the size of the tumors. Assay of serum CAP activity is a simple procedure which can be performed in any laboratory. It is a quick method for qualitative, and even quantitative, assessment of an ovarian adenocarcinoma in the nonpregnant patient. It may prove to be especially valuable in the follow-up of patients with ovarian cancer.

[The treatment of female stress incontinence of urine combined with uterine and vaginal prolapse by vaginal hysterectomy, anterior and posterior colporrhaphy in 1,003 cases (author's transl)].

The type and intensity of pre-operative symptoms and the long term follow-up in 1,003 vaginal hysterectomies with anterior and posterior colporrhaphies are reported. The operative technique corresponds to that of the school of Vienna with emphasis on plication of the urethra with joining of the ischio-carverrosus muscles below the urethra. The pre-operative symptoms disappeared in almost all cases. Constipation and backache improved. Early complications were treated and remained without sequelae. Our method of treatment showed good results and cures the social handicap of stress incontinence. In addition a good prophylaxis against unterine cancer and excellent permanent contraception is provided.
