Autotransplantation of splenic tissue.

Autotransplantation of splenic fragments has already been carried out in humans. The optimal size of the particles and amount of tissue required for this procedure has yet to be found. In normal young pigs and miniature piglets, autologous splenic tissue was transplanted into the greater omentum. The regenerated splenic mass, splenic blood flow, and histology were studied six months later. Implanting small splenic particles produced comparable results to implanting thin slices of splenic tissue. The mass of regenerated splenic tissue was only 3.1 g after implanting the whole spleen and 4.5 g after transplanting half of the spleen, which means 5.3% and 7.8% respectively of the weight of control spleens. The blood flow per gram in the regenerated splenic tissue was much lower than in the normal spleen. The blood flow in the whole of the splenic tissue is important for the clearance function of the spleen. Six months after transplanting the whole or half of the spleen, the blood flow to the regenerated splenic tissue was only 1% of that in the control minipigs. When half of the spleen was left in situ, as a model for a partial splenectomy, and the other half transplanted, the regenerated mass was only 3.4% of all splenic tissue and the blood flow 1.5% of the total splenic blood flow. In this model the regenerated splenic mass was independent of the size of the implants and the mass of implanted tissue. The extremely low blood flow indicates an inadequate clearance function and thus the protective function would probably be negligible.

Optiz trigonocephaly syndrome: report of two cases.

We report two patients with Opitz trigonocephaly syndrome. Both children showed the pattern of abnormal findings characteristic of this syndrome, including trigonocephaly, upslanted palpebral fissures, inner epicanthic folds, broad alveolar ridges, small chin, short neck with loose skin, muscular hypotonia and cardiac defect. An 8-week-old girl had multiple gingival frenula, brachydactyly, syndactyly of toes and anal stenosis in addition, while a boy who died at 28 h from cardiac failure showed multiple joint contractures, cryptorchidism and renal cortical cysts.

Splenic regeneration and blood flow after ligation of the splenic artery or partial splenectomy.

In normal, young miniature piglets, either the major arteries and the veins were ligated or three quarters of the spleen was resected using three different techniques. These techniques served as an animal model for splenic salvage in human subjects. After arterial ligation, no signs of infarction could be seen 1 and 4 weeks later, and after 6 months, normal sized spleens with enlarged vessels in the gastrolienal ligaments were found. When the splenic remnant was left at the main vessels, no obvious growth could be demonstrated after 6 months. The splenic tissue left at the smaller vessels, however, increased in size. Measurements of splenic blood flow in unanesthetized piglets 6 months after operation resulted in greater differences in perfusion per gram among the experimental groups. Not only splenic mass, but also blood flow to the whole splenic tissue seems to be an important factor in the protective function of the spleen.

[Ceco-ureterocele].

A case of caeco-ureterocele is presented, which is a rare special form of ectopic ureterocele and which was first described by Stephens. Its differential diagnostic characteristics compared with the prolapse of ureterocele are described. The article also discusses possible embryological causes which might explain this peculiar type of malformation.

Androgens as promoters of colon carcinogenesis.

The effects of androgens on colon carcinogenesis were studied in 90 male Fisher-344 rats, given 10 weekly subcutaneous (SC) injections of 7.5 mg azoxymethane (AOM)/kg body weight, starting at 12 weeks of age. Thirty rats received the AOM course only (AOM only). Thirty rats were gonadectomized at 10 weeks of age and underwent the AOM course (castrated). In 30 additional castrated rats, hormone substitution was achieved by sc injection of 1 mg dehydrotestosterone (DHT) three times a week for 12 weeks. AOM was given concommitantly, starting at 12 weeks of age (castrated and hormone substituted). Animals were sacrificed 25 weeks after first AOM injection. As assessed by weekly body weight data and periodic serum determinations of carcinoembryonic antigen (CEA), castrated animals showed a delayed onset of colonic carcinogenesis, compared to AOM-only-treated and hormone-substituted animals. Castration resulted in a decrease of mean tumor size as well as overall incidence of colonic tumors, compared to that observed AOM-only and castrated and hormone-substituted group, respectively (27 vs 47 vs 53%). Castration also resulted in lower mean serum CEA levels at the end of experiment than in the AOM-only and castrated and hormone-substituted group. These findings suggest a promoting effect of androgens in colon carcinogenesis.

Replacement of the ureter by an ileal loop. Quantitative aspects of long-term morphological alterations in minipigs.

In minipigs 1 ureter was replaced by a loop of the terminal ileum and the contralateral kidney removed. After 2.5 to 3 years the morphology of the replaced ureter was compared with the normal ileum. Independent of the 3 different operative techniques used, in about half of the pigs there was loss or flattening of the villi. In the other pigs the morphometrically determined number of intraepithelial lymphocytes, the cell density in the lamina propria and the length of the villi did not differ significantly compared with the normal ileum. There was no increase in goblet cells in the crypts. The transitional epithelium covered only a short distance at the anastomotic junctions. Peyer's patches of normal age-related size were found in the replaced ureter. Despite the long-term contact with urine instead of gut contents, in many pigs a normal amount of lymphocytes remained in the "ileal ureter".

[Vascular changes in the kidney after blunt abdominal trauma in a child].

A genuine trauma, which had been overlooked at first, resulted in macrohaematuria. Infusion urography showed an enlarged left kidney with delayed and incomplete visualisation of the renal pelvis. Angiography showed swelling of the left kidney and small arterial ruptures with pseudoaneurysmatic contrast medium deposits in the parenchyma. Three months later, the angiogram showed largely normalised vessels.

[Clinical and immunologic findings in BCG osteomyelitis].

BCG-osteomyelitis is a rare but serious late complication of BCG-immunisation. Be present clinical and immunological findings in 2 children aged 9 months and 5 years, who developed BCG-osteomyelitis, following immunisation with BCG-vaccine. The affected bones were the first metacarpal bone in one and the 6th rib in the other patient. Both children remained free of recurrences after surgical removal of the granulomatous tissue. Immunological investigations in both cases demonstrated normal humoral immunity (Complement CH50, C3, C4, immunoglobulins). Except for a diminished proportion of circulating B-cells, determination of lymphocyte subpopulations and in-vitro lymphocyte stimulation with Phytohemagglutinin and PPD were normal. In both children spontaneous cell mediated cytotoxicity (SCMC) against the tumor cell line L1210, in one patient also against tumor cell line K562 was decreased. Antibody dependent Cytotoxicity (ADCC) was normal.

[Juvenile polyp in the small bowel (author's transl)].

A single case of a solitary juvenile polyp of the small bowel is presented in a 10 month old infant which produced a jejuno-jejunal intussusception. A review of the literature failed to show a similar case. The pathology of the polyp is discussed.

[Therapy of large postpneumonic tension cavities (pneumatoceles) (author's transl)].

A postpneumonic tension cavity (pneumatocele) is defined as subpleurally trapped air originating from interstitial emphysema following necrobiotic changes of lung tissue. We recommend for the treatment of large pneumatoceles intercostal drainage and continuous suctioning for several days to avoid tension pneumothorax and development of persistent fibrous pseudocysts.

[Struma. Results of surgical management].

Case report about 3620 struma operations and comparison of the results with the publications of other authors. The situations of malignant strumas are considered separately. Postoperative complications, causes of death and mortality are also considered. The necessity of postoperative hormon-substitution is pointed out.
