Renal handling of sodium, potassium, calcium and phosphorus was investigated in 9 patients with cystic fibrosis (CF). Both under baseline conditions and during saline-induced diuresis, urinary excretion rate of sodium (UNaV), potassium, phosphorus and calcium did not differ significantly from control values although the fractional excretion of sodium was significantly higher in patients with CF during saline diuresis. When distally acting diuretics were administered, this difference between CF patients and controls was magnified and in addition, UNaV in CF patients was also significantly higher than in controls. Following saline loading the increment in glomerular filtration rate (GFR) in patients with CF was significantly lower than that of controls. We conclude that patients with CF when subjected to volume expansion demonstrate a lower tubular reabsorptive capacity of sodium as well as a reduced ability to increase their GFR. The defect in sodium reabsorption is probably located in the proximal tubule.
Hemoglobin A1 (HbA1) blood levels were examined in 34 cystic fibrosis (CF) patients aged 1 1/2 to 20 1/2 yr. The mean (+/- SD) HbA1 concentration in the 34 patients was 7.97 +/- 1.16--significantly higher than that in 150 normal children (6.8 +/- 0.8). Oral glucose tolerance tests (OGTT) were performed in 11 patients and blood glucose and insulin concentrations were measured. There was no difference in the fasting blood glucose values between the two groups. Eight children had a normal OGTT, and three showed an impaired OGTT with a delayed peak response in insulin levels. There was a decreased insulin response in all the CF patients. No correlation was found between HbA1 concentration and the age of the patients, nor between HbA1 and the clinical status (Shwachman score). We conclude that insulinopenia in children with CF is not always accompanied by an increment in HbA1, although serial determinations of its level may reveal deterioration in glucose tolerance.

Fifteen children presented with persistent cough as the sole complaint. Physical examination was entirely within normal limits. Personal and family histories of atopy and blood eosinophilia suggested an allergic diathesis. Chest radiographs showed hyperinflation in many and spirometry showed bronchospasm in all the children studied. The added observation that theophylline and salbutamol were effective suggests that the condition is a manifestation of asthma.

The ability to discriminate between cystic fibrosis (CF) patients, carriers and control subjects, by determining the degree of hear inactivation of certain plasma enzymes has recently been reported. Material from Israel on 39 patients, 68 carriers and 65 control subjects, was tested under the originally described conditions with concordant results and distinct separation of the three genotypes. No carriers were detected in the 65 control subjects, perhaps because of the dilutional effect of those ethnic groups in Israel among whom the frequency of the CF gene is known to be rare. Biochemically, no evidence of heterogeneity of CF was found among the various ethnic groups.
