[Urinary incontinence in the elderly: what management for which patient?].

THERAPEUTIC OPTIONS: Appropriate management of urinary incontinence in the elderly basically depends on the patient's medical status and degree of dependence and the type of incontinence. For outpatients, secondary effects of drugs limit their use, in favor of behavioral intervention and pelvic floor training, with good success. Recently proposed surgical approaches offer a promising alternative for the aged population. TEAM MANAGEMENT: Assessment and management of institutional urinary incontinence should be systematically elaborated by the medical care team. Potentially reversible conditions and precipitating factors may be causing or contributing to the incontinence. Their correction and bladder training are the principal items of the management scheme.

Anti-Semitism in the clinical setting: transference and countertransference dimensions.

Despite the salient presence of Jews in the history of psychoanalysis, literature on the subject of anti-Semitism in the clinical setting is surprisingly sparse. This paper attempts to comprehend the reasons for the dearth of literature on this important topic. A clinical section then breaks the silence surrounding expressions of anti-Semitism in the consulting room. The major focus is on transference and countertransference reactions that arise with regard to anti-Semitism in the clinical setting. Since the first section is concerned with silence in the psychoanalytic community, its focus is primarily on countertransference issues that may hinder the analyst's understanding and use of anti-Semitic material. The second, clinical section focuses on the ways both transference and countertransference reactions combine and influence one another and how they may, when properly attended to, serve as catalytic tools for advancing therapeutic goals.

Transitional space in the treatment of immigrants.

This paper illustrates how the dimension of space is an apt arena for immigrant patients to play out their separation problems. A continuum is proposed in which the extreme ends represent pathological attitudes toward space. Two cases are presented to demonstrate how these differential approaches to space result in unique ways of dealing with separation and immigration. Treatment of immigrants involves the transformation of the therapeutic milieu into a transitional space to form a bridge between the patient's native country and current home and a potential space to allow patients to create new ways to view themselves and their worlds.

The primal scene: variations on a theme.

The purpose of this paper is to clarify and enrich the concept of the primal scene. A critical review of the literature highlights the major questions and controversies that have arisen in relation to the concept. The authors propose that multiple and shifting identificatory positions can be discerned in primal scene configurations. Primal scene experiences and fantasies are viewed as a blueprint for internalized object relationships. Both traumatic and adaptive consequences of primal scene fantasies and exposure are noted. A case illustration exemplifies primal scene trauma resulting from chronic exposure and illustrates the analysand's internalized multiple and shifting identifications which became embedded in the transference relationship.

Egon Schiele and Frida Kahlo: the self-portrait as mirror.

This article explores the psychoanalytic significance of the self-portraits of Egon Schiele and Frida Kahlo. Both artists employed the self-portrait as a central means of artistic expression. Their fanatic and prolific focus on artistic self-representation exemplifies the manner in which they dealt creatively with narcissistic issues. The therapeutic function of the self-portrait as mirror is examined as well as the part it plays in self-consolidation and identity maintenance.
