Lumbar isokinetic strength and the influence of age, bodyweight and testing velocity in patients with chronic low back pain in comparison with persons without pain. Lumbar isokinetic strength parameters of 80 patients with chronic low back pain and 70 persons without pain were compared and the influence of age, bodyweight and testing velocity was evaluated. The patients with chronic low back pain showed less strength than the persons without pain. All parameters of extension discriminated between the two groups whereas only some of the flexion parameters did. The isokinetic strength of the lumbal extension muscles was higher than the strength of the flexion muscles. In patients with chronic low back pain, isokinetic strength of lumbar extension muscles was more reduced than the strength of flexion muscles in comparison with persons without pain. At 90 degrees/sec in comparison to 60 degrees/sec, lower extension forces, higher flexion forces and changed ratios of flexion and extension muscles were measured. Age had an influence only on women. There were no changes in ratios of flexion and extension muscles with increasing age. Bodyweight showed weak correlations with isokinetic flexion forces. The influence of different factors on isokinetic force varies between patients with chronic low back patients and healthy subjects.
The present paper reports our first results after pars plana vitrectomy in patients with diabetic retinopathy and hemodialysis with a follow-up of 6 to 24 months. Between January 1992 and October 1994 we performed vitreoretinal surgery with silicone oil tamponade in nine eyes of seven patients with diabetic nephropathy on hemodialysis. All patients had had type I diabetes for 19-32 years. Over the observation period the retina was completely attached in eight eyes. Final visual acuity of 0.1-0.7 was attained in four eyes, 0.06 two, hand movements in one eye. Two eyes had no useful final visual acuity because of redetachment of the retina or secondary glaucoma with rubeosis iridis. The small number of complications shows that pars plana vitrectomy can be done in diabetic patients with nephropathy on hemodialysis. This significantly improves their quality of life.
To elucidate the pathogenic mechanisms of acute allograft dysfunction that is not caused by acute cellular rejection, we have studied the clinical and immunopathologic characteristics of 11 heart transplant recipients who had acute allograft dysfunction in the absence of interstitial mononuclear cell infiltrates on endomyocardial biopsy samples. Six of eleven patients (54%) had a striking increase in levels of anti-HLA antibodies in close temporal proximity with the episode of acute allograft dysfunction. Cardiac allograft function improved in all patients with intensification of immunosuppression.
