[Cytostatic therapy of GI carcinoma based on assessment of cell kinetics (author's transl)].

Flow cytometry affords an insight into the cell cycle of gastrointestinal tumors. In comparison with the normal GI mucosa, dedifferentiated carcinomas show a large SG2M peak expressing an intense proliferation. The histological examination considers normally only a small part of the tumor, whereas the flow cytometric histogram is based upon a larger amount of cells. Therefore some tumors are regarded as well differentiated by the pathologist, although they have a large SG2M peak. Considering the functioning of cytostatic drugs, all GI tumors should be treated which have a large SG2M peak in flow cytometry.

[Wound-healing disorders and mortality in one- and multi-stage sigma and rectum resections].

From 1958 to 1978, the Surgical Clinic of the Tübingen University carried out 427 sigmoidal and rectal resections with a one-, two-, or three-stage proceeding. The comparison proves that, as was to be expected, the one-stage resection more often shows an anastomotic insufficiency. On the other hand, there occur less infections of the abdominal wall because of the absence of a smear-infection deriving from the temporary intestinal fistula. The primary lethality through anastomotical complications does not differ in the one-, two-, or three-stage proceeding if in the latter you effect an only mechanical cleaning of the inactive colon leg. The irrigation of the inactivated colon with an antibiotic solution before the tumor resection leads, compared to the cleaning with saline solution, to much less infections of the abdominal wall, less anastomotic insufficiencies and to the reduction of the primary lethality. This three-stage proceeding is, with a lethality of 1,5%, still the most secure action for the patient.

[Combined use of iso and aniso-peristaltic jejunal loops to reestablish continuity after total gastrectomy (author's transl)].

Following experiments in animals, a new procedure for gastric replacement after total gastrectomy has been employed since 1972. Digestive tract continuity is reestablished by interposing two jejunal segments, on iso- and the other aniso-peristaltic, between the esophagus and the duodenum. The proximal isoperistaltic segment should be between 30 and 40 cm in length, while the distal anisoperistaltic segment should measure 10 cm. The latter segment replaces the pylorus and produces delayed intermittent emptying of the proximal iso-peristaltic segment. Their progressive dilatation results in the development of a new reservoir which reaches its maximum volume after 6 to 12 months. Radiological, cinematographic, and endoscopic examination in 58 patients in whom this reconstruction had been performed demonstrated that function was excellent. Results of metabolic studies and tests for clinical function confirmed the value of the method, which has definite advantages over previous techniques employed to reestablish continuity. For this reason, the method has become a routine procedure in patients requiring extensive total gastrectomies.

[Control mechanisms of the ribonucleotide reduction in mammalian tissue (author's transl)].

The ribonucleoside diphosphate reductase (E.C.1.17.4.1) was partially purified from Ehrlich ascites carcinoma and regenerating rat liver. The specific activity of the two enzyme preparations did not vary with regard to the reduction of UDP, ADP, CDP and GDP. Similarly the regulation of the enzyme system by deoxyribonucleoside triphosphate (dNTP) is almost identical. Through the application of deoxyribonucleosides (10(-3) or 2 x 10(-3)M) and measurement of the dNTP content it was found in Ehrlich and Yoshida ascites tumours that these control mechanisms are transmissible to whole cells. dATP inhibits the reduction of all four nucleoside diphosphates. dTTP stimulates the reduction of GDP, dCTP that of UDP and dGTP that of ADP.

[Early complications and early mortality of various operative methods of gastrectomy (author's transl)].

From 1959-1976, 183 gastrectomies were performed in the Chirurgische Klinik of the University of Tübingen, for gastric carcinoma. The average mortality for the operations was 19.1%, postoperative complications occurred in 41% of the cases. The most frequent cause of death was failure of the oesophageal anastomoses. The two methods most commonly used, Longmire-Gütgemann's interposition of small intestine and oesophagojejunostomy with Braun's anastomosis, differ considerably in the complication rate and mortality. The interposition of small intestine, with almost indentical preoperative and intraoperative risk factors, an operative mortality of 4.4% and distinctly less danger of anastomotic failure was shown to be superior to the technically simple oesophagojejunostomy. The formation of an "artificial stomach" in gastrectomy is therefore to be preferred to all other surgical methods of reconstruction of the intestinal passage, not only because of the better late results but also because of the better early results.

[Studies on the thymidine-triphosphate synthesis in malignant tumors. I. Effects of thymidine on deoxyribonucleoside triphosphate pools and deoxyribonucleic acid synthesis (author's transl)].

Measurements of the deoxyribonucleoside triphosphate (dNTP) contents, the [14C] thymidine and deoxyuridine incorporation and the "key enzymes" of the thymidine triphosphate (dTTP) synthesis, thymidine kinase and ribonucleotide reductase, in diploid Ehrlich-ascites carcinoma, in Yoshida sarcoma-ascites cells and to a smaller extent in surgically removed malignant human tumours show 1. A distinctly increased dTTP content compared with the remaining dNTP is not a characteristic of tumour cells generally but a peculiarity of sarcoma and a sign of differentiation of a malignant tumour. 2. With simultaneous linear deoxyribonucleoside incorporation the dTTP content and the mix-proportion of [14C] dTTP to total dTTP in ascites tumour cells in short-term in-vitro incubation (120 min) remain constant. 3. Thymidine addition to the medium leads to a distinct rise of dTTP concentration even at a dosage of 3 X 10(-5) M. 4. The dNTP contents of ascites tumour cells are within the range of the endproduct-inhibiting concentrations of thymidine kinase and ribonucleotide reductase.

[Studies on the thymidine-triphosphate synthesis in malignant tumors. II. Effect of hyperthermia, Vitamin K and Cytotoxic agents (author's transl)].

Measurements of the deoxyribonucleoside triphosphate (dNTP) contents, the [14C] thymidine and deoxyuridine incorporation and the "key enzymes" of the thymidine triphosphate (dTTP) synthesis, thymidine kinase and ribonucleotide reductase, in diploid Ehrlich-ascites carcinoma, under the application of hyperthermia, vitamin K and cytocidal agents show: The effect of hyperthermia and menadion (the basic substance of the K vitamins) on the above parameters of dNTP synthesis can explain the labile effects of hyperthemia and vitamin K therapy on cancer growth. Alterations of the dNTP concentrations and demonstrable or absent inhibition of the ribonucleotide reduction with application of fluoruracil, amethopterine, cytosine arabinoside, hydroxyurea, trisethylen iminobenzochinone and daunomycin confirm and supplement our knowledge of the cytostatic action mechanism of these substances. They show moreover by the example of fluoruracil and amethopterine medication that the dTTP concentration estimation after in-vitro incubation of tumour cells with the addition of FU or methotrexat is a better measurement of the therapeutic in-vivo responsiveness of malignant tumours than the previously performed test methods.
