Widespread lichen planus in association with Turner's syndrome and multiple endocrinopathies.

A 40-year-old white woman with a history of diabetes mellitus, hypothyroidism, irritable bowel syndrome, and Turner's syndrome presented with widespread lichen planus. She also had extensive erosions of her oral and vaginal mucosa, along with diffuse noncicatricial alopecia. The patient's condition was refractory to most topical therapies, and improved with oral psoralen/ultraviolet A. Previous associations of lichen planus with other medical disorders have been described, yet its occurrence with Turner's syndrome is yet unrecognized.

Pyoderma gangrenosum as a cutaneous manifestation of diverticular disease.

We have described a case of pyoderma gangrenosum due to diverticular disease. The patient was an elderly woman with a 3-month history of cutaneous disease without associated gastrointestinal complaints. There was no history of inflammatory bowel disease, malignancy, or seropositive arthritis. Evaluation revealed a retroperitoneal abscess due to a ruptured diverticulum. Surgical resection of the affected colon led to rapid and complete resolution of all symptoms. Pathologic examination of resected tissue showed diverticulitis without evidence of chronic inflammatory bowel disease.

Fibrous mastocytoma in a patient with generalized cutaneous mastocytosis.

A 57-year-old woman with cutaneous mastocytosis of 23 years duration developed a hyperpigmented abdominal plaque composed of confluent indurated papules that enlarged for a period of 1 year to 12 x 8 cm. Biopsy showed dermal infiltration by closely packed spindle-shaped mast cells, fibroblasts, collagen, and scattered lymphocytes, predominantly T-suppressor cells. Electron microscopy showed close contact between mast cells, fibroblasts, and lymphocytes. Piecemeal mast cell degranulation and extrusion of mast cell granules was seen, with rare mast cell granules in fibroblasts, and collagen fibers in peripheral and perinuclear endoplasmic reticulum of mast cells. the term Fibrous mastocytoma is suggested for this tumor-like dermal fibrosis, possibly induced by lymphokines.

Biopsy

Circumoral dermatitis and cheilitis caused by tartar control dentifrices.

Twenty women had similar symptoms and clinical findings related to the use of tartar control toothpaste. Each had burning and itching around the corners of the mouth, followed by pruritic perioral patches of erythema 4 to 14 days later. All patients had begun using tartar control toothpaste 1 to 2 weeks before onset and had brushed their teeth at least three times per day. The typical findings included a moderately severe perioral erythema with fissuring of the angles of the mouth; scaling erythematous patches, separated by normal skin lateral to the initial erythema, were also present. No vesiculation or crusting was noted, and involvement of the gingiva and buccal mucosa was noted in only one case. On discontinuation of the tartar control toothpaste, marked improvement was seen within 1 to 6 weeks with, in most instances, either no treatment or 1% hydrocortisone cream.
