Defibrillation with the sequential pulse technique: reproducibility with repeated shocks.

The development of the automatic implantable defibrillator has created the need to assess the effects of interventions on defibrillation success. However, first it is important to determine the spontaneous variability of defibrillation threshold (DFT) over time. We repeatedly determined DFT over a maximum of 2-1/2 hours in open-chested, halothane-anesthetized pigs. Ten seconds after induction of fibrillation, defibrillation was attempted by passing a sequential pulse shock through an indwelling catheter and patch electrodes. Ninety-seven fibrillation episodes (FEs) were induced in eight pigs, with a maximum of 30 shocks in an animal. DFT remained stable and fitted a flat least-squares regression equation (y = 6.35 + 0.0055x, where x is the ventricular FE number and y is the DFT, r = 0.0115, p = NS). The mean DFT over time for the eight animals was 7.6 +/- 1.9 J (range 4.8 to 16.3 J). The inter-animal variability for DFT was 3.7 J and the mean intra-animal DFT variability over time was 3.6 +/- 2.3 J (range 0 to 6.8 J). We conclude that, using our methodology, DFT is reproducible and consistent over at least 2 hours. This model provides the basis to assess the effects of acute interventions on the ability to defibrillate.

Microcomputer use in an oral biopsy service.

The need for rapid and accurate retrieval of the data generated by an oral biopsy service and the adjacent medical center was met with the purchase and programming of a microcomputer and hard disk drive. The planning phase involved an assessment of the needs of the department, creation of an ideal form to be displayed on the video screen that can be easily used to enter the information, selection of coding systems, and selection of compatible hardware and software. Customized in-house programming using a commercially available database management system has created an entry form and menu-driven information retrieval system tailored to the needs of the department.

The relationship between oestradiol metabolism and adrenal steroids in the endometrium of postmenopausal women with and without endometrial cancer.

The aim of the present study was to investigate the hypothesis that adrenal androgens are contributory to the development of endometrial cancer either by the oestrogenic action of 5-androstene-3 beta, 17 beta-diol (androstenediol) or through the inhibition of oestradiol metabolism. Concentrations of androstenediol, dehydroepiandrosterone (DHA), DHA sulphate (DHAS), oestrone and oestradiol were measured in plasma and endometrium from postmenopausal women with and without endometrial cancer. There was no difference between normal postmenopausal women and endometrial cancer patients with respect to either tissue or plasma adrenal androgens although there was a tendency for plasma DHAS levels to be increased in cancer patients (normal women: 640 +/- 156 ng/ml; cancer patients: 808 +/- 159 ng/ml). There was a positive correlation between endometrial tissue concentrations of androstenediol and DHA in both normal women (P less than 0.05) and cancer patients (P less than 0.01) but for DHAS the relationship was only significant for non-malignant tissue (androstenediol: DHAS, P less than 0.05; DHA: DHAS, P less than 0.02). A significant positive correlation was found between all three plasma adrenal androgens for both groups. In cancer patients there was a trend towards an inverse correlation between endometrial tissue concentrations of DHAS and the enzyme 17 beta-hydroxysteroid dehydrogenase (17OHSD) although the relationship was not significant (r = 0.49). In endometrium, oestradiol was present in significantly higher concentrations than oestrone whereas in plasma the reverse was the case. There was also a tendency for plasma oestradiol levels to be elevated in the cancer subjects. These data do not support a substantial role for adrenal androgens in endometrial cancer but suggest that a relationship may exist between DHAS and 17OHSD and that an imbalance between sulphatase and sulphotransferase activities may be involved.

Cardiovascular and drinking responses elicited by central administration of angiotensin II: differential effects of GABA injected into the ventral tegmental area and spiperone injected into the nucleus accumbens.

The ventral tegmental area and its dopamine projections to the nucleus accumbens have been shown to be involved in goal-directed behavior. This study investigated the contribution of GABAergic input to the ventral tegmental area and dopaminergic input to the nucleus accumbens to the drinking and cardiovascular responses elicited by central administration of angiotensin II. Injections of 25 ng of angiotensin II into a lateral cerebral ventricle of the conscious rat elicited water intakes averaging 8 ml in 15 min with latencies usually less than 3 min. Pretreatment of the nucleus accumbens with spiperone, a dopamine antagonist, or the ventral tegmental area with gamma-amino butyric acid (GABA) produced dose-dependent reductions in water intake and number of laps taken while increasing the latency to drink. Injections of spiperone into the nucleus accumbens in anesthetized animals failed to alter the cardiovascular response elicited following the central administration of angiotensin II. However, administration of GABA into the ventral tegmental of urethane anaesthetized animals attenuated the pressor response normally elicited by central administration of angiotensin II. These findings suggest that GABA input to the ventral tegmental area is involved in both the cardiovascular and drinking responses elicited following central administration of angiotensin II. The dopamine projections to the nucleus accumbens appear only to be involved in the drinking response elicited by central injections of angiotensin II. Divergence of the signals separately accessing the skeletal motor behavioral component and the cardiovascular component elicited by central administration of angiotensin II must occur before the involvement of these dopamine pathways.

The use of nonradiolabelled steroid infusions to investigate the origin of oestrone sulphate in postmenopausal women.

Infusion of nonradiolabelled dehydroepiandrosterone sulphate (DHA-S) has been used to investigate the possible formation of oestrone sulphate via a sulphated conjugate of androstenedione. The metabolic clearance rate (MCR) of DHA-S also was measured and the mean value (25 1/24h) was similar to values reported using isotopic techniques. Although conversion of DHA-S to 5-androstenediol, a steroid with oestrogenic properties, was detected during infusion of DHA-S, there were no significant increases in plasma levels of conjugated androstenedione or oestrone sulphate. The MCR's oestrone sulphate measured using infusion of nonradiolabelled steroid in two menopausal women were 99 1/24h and 121 1/24h. For one woman, the production rate of oestrone sulphate, calculated from the conversion of oestrone and oestradiol to oestrone sulphate (151 nmol/day) was similar to the measured production rate of oestrone sulphate (144 nmol/day). It is concluded that in menopausal women, oestrone sulphate is derived from conversion of oestrone and oestradiol with no formation occurring via conjugated androstenedione.

Internal cardiac defibrillation threshold: effects of acute ischemia.

The influence of myocardial ischemia on defibrillation success was studied using two different lead orientations in halothane-anesthetized pigs. Ischemia was induced by ligating the left anterior descending artery in its distal third. Controls had loosely tied ligatures placed around the artery at the same site. Ventricular fibrillation was induced by electrical stimulation 30 minutes after coronary artery ligation. Defibrillation used a single truncated pulse of approximately 6 ms duration passed to either: a transvenous electrode catheter (Medtronic, 6880) with the cathode in the apex of the right ventricle and the anode in the superior vena cava-atrial junction region, or the cathode in the apex of the right ventricle and a mesh plaque on the epicardium of the basal lateral left ventricle as anode. Ten seconds after the onset of ventricular fibrillation, defibrillation was attempted with increasing incremental energies until defibrillation was achieved. Fibrillation episodes were repeated at 15-minute intervals until the minimum first shock was successful in defibrillating the animal (i.e., defibrillation threshold). The number of animals successfully defibrillated with a minimum energy above or below 30 J was not different between normal and ischemic animals for either electrode configuration (i.e., 3 out of 20 vs 1 out of 13 for the catheter and 5 out of 6 vs 6 out of 7 for the epicardial plaque, respectively). Also, the cumulative percent success as a function of defibrillation energy was similar in both the normal and ischemic groups. There was a significant reduction in the minimum energy necessary for defibrillation when passing current between the right ventricular apex and the left ventricular epicardial plaque. The present results indicate that, despite differences in lead orientations, acute ischemia in the anesthetized pig does not appear to influence defibrillation success.

Evaluation of laboratory tests for detection of methicillin-resistant Staphylococcus aureus and Staphylococcus epidermidis.

Few studies evaluating susceptibility testing of methicillin-resistant staphylococci have included isolates of Staphylococcus epidermidis, a known pathogen in many types of serious infections. We tested 175 S. epidermidis and 95 Staphylococcus aureus isolates to determine the most sensitive procedures for detecting methicillin-resistant staphylococci. Reference procedures included agar dilution with methicillin and 4% NaCl in the agar and broth microdilution with methicillin and 2% NaCl in cation-supplemented Mueller-Hinton broth. After 24 h of incubation, the results from both methods correlated well and were within 1 log2 dilution for all isolates tested. Only one-half of all resistant isolates (92 of 183) were detected at 18 h by using the standard disk diffusion technique with 5-micrograms methicillin disks, and even fewer were detected with 10-micrograms methicillin disks and newly recommended zone-size criteria. However, the standard disk diffusion method with 4% NaCl in the agar increased the sensitivity and specificity for identification of the proper phenotype to greater than 92%. The spread plate and new spot techniques, both using agar with 4% NaCl, were also sensitive methods. Of 47 S. epidermidis isolates tested against oxacillin, 6 (13%) were oxacillin susceptible but methicillin resistant. Two automated systems, the Automicrobic system (Vitek Systems) and MicroScan (American MicroScan), as well as two broth screening systems available from Remel and Austin Biological Laboratories, failed to detect several resistant isolates, depending on the species.

Central integration of cardiovascular and drinking responses elicited by central administration of angiotensin II: divergence of regulation by the ventral tegmental area and nucleus accumbens.

Previous studies had implicated the involvement of the ventral tegmental area and its dopamine projections to the nucleus accumbens in goal-directed behavior. This study investigated whether or not the GABAergic inputs to the ventral tegmental area and, in turn, dopaminergic input to the nucleus accumbens from the ventral tegmental area modify drinking and cardiovascular responses elicited by central administration of angiotensin II. Injections of 25 ng of angiotensin II into a lateral cerebral ventricle of the rat elicited water intakes averaging 7-8 mL in 15 min with latencies usually less than 3 min. Pretreatment of the nucleus accumbens with spiperone, a dopamine antagonist, or the ventral tegmental area with gamma-amino butyric acid (GABA) produced dose-dependent reductions in water intake and number of laps taken while increasing the latency to drink. The spiperone injection did not alter the pressor response. On the other hand, the GABA injections attenuated the pressor responses to central angiotensin II administration. These findings suggest that GABA input to the ventral tegmental area modifies both the cardiovascular and drinking responses elicited following central administration of angiotensin II. However, the dopamine projections to the nucleus accumbens appear to be involved only in the drinking responses elicited by central injections of angiotensin II. Divergence for the coordination of the skeletal motor behavioral component and the cardiovascular component elicited by central administration of angiotensin II must occur before the involvement of these dopamine pathways.

Internal cardiac defibrillation in man: pronounced improvement with sequential pulse delivery to two different lead orientations.

Wider applicability of an implantable automatic defibrillator depends on achieving internal cardiac defibrillation consistently with the lowest possible energy. In animal studies, we have found that the cardiac defibrillation threshold could be reduced when sequential shocks separated in time and spacially arranged were delivered to the heart. We compared internal cardiac defibrillation using a single pulse shock delivered through an intravascular catheter with this new method for internal cardiac defibrillation in patients undergoing cardiac surgery for the correction of arrhythmias. For the single pulse shock and the first pulse of the sequential pulse shock, current was passed through an intravascular catheter with the catheter cathode at the apex of the right ventricle and the anode at the superior vena cava-atrial junction region. The second pulse of the sequential pulse countershock was delivered between the catheter cathode in the right ventricular apex and an oval plaque electrode secured on the laterobasal left ventricular epicardium as anode. With the single pulse alone for shock delivery, 12 patients could be defibrillated with an average of 20.1 +/- 16.8 J, with a corresponding leading-edge peak voltage and current of 836 +/- 319 V and 9.4 +/- 4.5 A, respectively. However, two of the patients could not be defibrillated with energies below 50 J. With the sequential pulse shock delivery, a significant reduction in all values were recorded. Mean total energy for defibrillation averaged 7.7 +/- 6.0 J. Leading-edge peak voltage and current from the catheter averaged 430 +/- 148 V and 5.0 +/- 2.8 A, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Surgery for Wolff-Parkinson-White syndrome: further experience with an epicardial approach.

We have previously reported the use of an epicardial approach for ablation of left ventricular free wall accessory atrioventricular pathways. The technique involves mobilization of the atrioventricular fat pad and exposure and cryoablation of the atrioventricular junction at the site of the accessory pathway on the normothermic beating heart. Here we describe our further experience with left ventricular free wall accessory pathways and right ventricular free wall accessory pathways. Our experience is based on 53 consecutive patients. There were 35 male and 18 female subjects, 6 to 52 (mean 41.4) years old. Forty-eight patients had a left ventricular free wall accessory pathway, and five had a right ventricular free wall accessory pathway. Two patients had an associated anterior septal accessory pathway. Five patients had associated cardiac abnormalities, including atrial septal defect, aortic insufficiency, mitral valve prolapse, Ebstein's anomaly, and cardiomyopathy. The accessory pathway was ablated in 52 patients who remain arrhythmia free without medication after a mean follow-up period of 12 months. The accessory pathway was permanently modified in one patient. There were no postoperative complications. This epicardial approach can be performed with normothermic cardiopulmonary bypass or without bypass. It does not require cross-clamping of the aorta, allowing a greater margin of safety when this is required for concomitant procedures.

Personality and demographic characteristics of dropouts and completers in a nonhospital residential alcohol treatment program.

This study investigated the relationship of scores on the Psychopathic Deviate, Denial, Dependency, and MacAndrew Scales of the MMPI; intelligence; age; income; and education to continuance in a residential alcohol treatment program. Dropouts scored higher on the Psychopathic Deviate Scale than completers did. For completers, Psychopathic Deviate scores were correlated with age and Denial scores were correlated with intelligence. These relationships did not occur for dropouts. Findings were compared to research done in hospital settings.

Surgical ablation of posterior septal accessory pathways in the Wolff-Parkinson-White syndrome by a closed heart technique.

The conventional technique for surgical ablation of posterior septal accessory pathways in the Wolff-Parkinson-White syndrome has been associated with a significant incidence of inadvertent permanent atrioventricular block. We report our experience with the ablation of posterior septal accessory pathways by a closed heart technique that combines mobilization of the posterior septal atrioventricular fat pad and exposure and cryoablation of the atrioventricular junction. The operation is performed on the normothermic beating heart. Consequently, atrioventricular node-His bundle conduction and accessory pathway conduction can be continuously monitored to avoid inadvertent injury to the atrioventricular node-His bundle system. This technique for ablation of posterior septal accessory pathways was used in 13 patients (four female and nine male patients, aged 14 to 59 years). The heart was exposed via a median sternotomy. Epicardial mapping was used to determine the insertion of the accessory pathway either to the left ventricular process or the immediately adjacent right or left ventricular free wall. Normothermic cardiopulmonary bypass was used in nine patients and omitted in four. Accessory pathway conduction disappeared in the course of dissecting the fat pad from the atrial wall and atrioventricular sulcus in all patients. Cryosurgical lesions were then applied to the atrioventricular sulcus in the area of interest (while monitoring atrioventricular conduction) to ensure transmural fibrosis of the atrioventricular ring. All patients tolerated the procedure well. There were no complications and, specifically, not a single instance of atrioventricular block. All patients remain arrhythmia free after a follow-up period of 10 months. This closed heart approach allows the ablation of posterior septal accessory pathways while the electrocardiogram is being monitored. It obviates the need for aortic cross-clamping and minimize the risk of inadvertent heart block.

Improved internal defibrillation with twin pulse sequential energy delivery to different lead orientations in pigs.

Internal cardiac defibrillation with an intravascular catheter was compared with a new method for internal cardiac defibrillation using 2 pulses delivered in sequence directly to the myocardium. For the sequential pulses, the first pulse was passed through an intravascular catheter (Medtronic 6880), between the anode in the superior vena cava-atrial junction region and the cathode in the apex of the right ventricle. The second pulse was delivered between the catheter tip in the right ventricular apex as cathode and an oval plaque electrode (Medtronic TX-7) secured on the epicardium of the left ventricular free wall as anode. Defibrillation pulses were of truncated, trapezoidal waveform (65% tilt), separated by 1, 10 and 100 ms. Using the catheter alone, 36 normal pig hearts could be defibrillated by 44 J. However, 22 pig hearts (60%) could not be defibrillated with energies below 35 J. Defibrillation threshold was improved with sequential twin pulses, the improvement being dependent on pulse separation (42, 34 and 19 J, at 100-, 10- and 1-ms separation, respectively; F = 14.6, df = 2.29, p less than 0.01). In conclusion, sequential twin pulse defibrillation provides a considerable reduction in energy necessary for defibrillation in comparison to single pulses using the catheter alone. In this study, the optimal separation was 1 ms.

The identification, quantification and possible origin of non-polar conjugates in human plasma.

The existence and quantification of non-polar conjugates of pregnenolone, dehydroepiandrosterone (DHA) and androstenediol in human plasma is described. The plasma level of non-polar pregnenolone conjugate is 200% higher than that of pregnenolone but the conjugates of DHA and androstenediol are 10 and 5-10% respectively of the plasma levels of the unconjugated steroid. Non-polar pregnenolone conjugate concentrations were found to be highly elevated in the plasma of one pregnant subject, and elevated in the plasma of patients with acne and breast cancer. Non-polar DHA conjugate levels were significantly elevated in hirsute patients and were approaching significance for patients with acne. A subject taking the combined oral contraceptive pill had very low plasma DHA conjugate levels. No significant alterations in the plasma levels of the androstenediol conjugates were found. A role for the non-polar conjugates in the aetiology of hirsutism and acne is proposed.

The significance of the ratio in follicle-stimulating hormone and luteinizing hormone in induction of multiple follicular growth.

Among the patients enrolled in the Norfolk In Vitro Fertilization Program there were 32 who had been stimulated according to the basic stimulation protocol using two ampules of human menopausal gonadotropin (hMG) daily. Because of their inadequate response, 23 of these 32 patients were stimulated subsequently with a combination of two ampules of "pure" follicle-stimulating hormone (FSH) and two ampules of hMG on cycle days 3 and 4. The remaining nine patients received four ampules of "pure" FSH only on cycle days 3 and 4. Stimulation was continued with hMG in both FSH regimens. Ten thousand units of human chorionic gonadotropin was used for final maturation. Parallel with the increase in the ratio of exogenous FSH to luteinizing hormone, an increase in oocyte recovery was observed, as well as an improvement in transfer and pregnancy rates. It was concluded that FSH enrichment had a beneficial effect in these patients.

Controlled cryothermal injury to the AV node: feasibility for AV nodal modification.

Elective subtotal injury to the AV node-His bundle region may create a negative dromotropic effect to provide a therapeutic advantage in some patients with supraventricular tachycardia without creating complete AV block. We examined the effects of cryosurgery to the AV nodal region, varying temperature and time using a 15 mm circular cryoprobe applied directly to the canine AV node-His bundle region. Twelve dogs were anesthetized and the heart was exposed through a right thoracotomy. Electrophysiological data obtained included conduction intervals, incremental pacing, and extrastimulus testing. Under inflow occlusion, the cryoprobe was positioned over the AV node-His bundle region using anatomical landmarks and a single freeze was applied (-15 degrees C to -60 degrees C, 15 to 60 seconds). Dogs were allowed to recover for 1 month, after which time electrophysiological testing was repeated under similar conditions; then the animals were sacrificed. With probe temperatures of -60 degrees C for 15 to 60 seconds, five of six dogs experienced complete heart block with dense fibrosis observed in the AV nodal-His bundle region. After freezing with higher temperatures, the remaining seven dogs had return of atrioventricular conduction postoperatively with prolongation of AH time observed in five and marked prolongation of the Wenckebach cycle length in three of the five. We conclude that controlled cryothermal injury to the AV node-His bundle region may be useful to create a desirable negative dromotropic response without creating complete AV block.

Serodiagnosis of ocular toxocariasis: a comparison of two antigens.

This study was designed to compare the sensitivity and specificity of enzyme linked immunosorbent assay (ELISA) for the serodiagnosis of ocular toxocariasis using Toxocara canis embryonated egg antigen (TEE) and toxocara excretory-secretory or exoantigen (TEX) produced in vitro. TEE and TEX ELISA were comparably sensitive, but TEX ELISA was better able to discriminate between serum samples from patients with ocular toxocariasis and those from patients with retinoblastoma. In addition, preabsorption of sera with Ascaris suum embryonated egg antigen seemed to be essential to prevent false positive results with TEE ELISA but was not so critical for TEX-ELISA. Further studies are still required to standardise TEX for serodiagnosis.

Effect of speaking rate on judgments of disordered speech in children with cleft palate.

The effect of increased speaking rate on the perception of disordered speech in children with cleft palate was studied. Six children with cleft palate (ages 7 years to 10 years of age) produced three different sentences over a continuum of speaking rates. Fifteen judges rated each sentence according to the severity of disordered speech perceived, including nasality, nasal distortions, nasal emission, glottal stops, and pharyngeal fricatives. Mean ratings for the 15 judges were determined for each sentence. The results indicated that the perception of disordered speech did not increase as a function of increased speaking rate. Possible explanations for the results include: (1) The speakers with cleft palate made adjustments in movement strategy to accommodate the reduced time in a manner similar to speakers without a cleft. (2) The speakers with cleft palate used movement strategies which, although unlike those used by speakers without a cleft, allowed them to avoid speech deterioration. (3) Increasing speaking rate did not reduce the time available to an extent that it was necessary to change movement strategy.
