Concurrent use of buspirone in anxious patients during withdrawal from alprazolam therapy.

A multicenter, double-blind, placebo-controlled study was conducted to determine the safety of concurrently using buspirone with alprazolam before and during a gradual tapering of the alprazolam dose. Thirty-six patients received placebo t.i.d. and 36 received buspirone 5 mg. t.i.d. Findings included significantly greater anxiety (as measured by the Hamilton Rating Scale for Anxiety) in the placebo group and significantly reduced manifestations of abstinence (as measured by the Abstinence Rating Scale) in the buspirone group. Buspirone and alprazolam may be used together safely, and buspirone may be started early in the alprazolam tapering process.

A preliminary report on anti-depressant therapy and its effects on hope and immunity.

Psychological states such as bereavement and depression have been associated with suppression of the immune response. The authors present a pilot study utilizing the anti-depressant, maprotiline hydrochloride (Ludiomil) to determine its effects on the mental depression and immune reactions of T&B lymphocytes in patients with neurotic depression (dysthymic disorder by DSM III). Hope scores, derived from the content analysis of verbal samples, and Beck depression scores were examined as they covaried with various indices of immune response before and 3 months after the patients were administered either maprotiline or no anti-depressant medication. Although the study did not provide definitive findings regarding improved response following treatment of depressed patients with maprotiline hydrochloride, it did suggest further avenues of research for investigation.

Current explorations in psychoimmunology.

Psychoimmunology is a new discipline linking psychological and immunological events. Studies to date have shown parallels between animal studies which measure response to stress and human reaction to loss, such as bereavement. Stein et al., have studied T and B lymphocytes in a bereaved population, noting suppression of function for six months to one year after a loss. Bartrop has found similar correlations. Gottschalk reviewed leukocyte function in depressed patients, finding a correlation between degree of depression and suppressed chemiluminescence. Udelman attempted a correlation between immunity and psychological states. Correlations were found between hope and T- and B-cell counts. The importance of psychoimmunology is underscored by the measurable impact of emotions on the body's defenses. It would seem reasonable to assume that adequate psychological defenses would be of aid in reducing vulnerability to illness. Thus, the psychotherapist is confronted by a newly measurable challenge to aid in control of not only emotional, but also physical illness. The enhancement of immunocompetence is a truly essential goal in prevention of illness.

Group therapy with rheumatoid arthritic patients.

A husband-wife co-therapist team, in clinical collaboration with rheumatologists of the Phoenix Arthritis Center, has instituted a brief inpatient group therapy program in an arthritis unit of a general medical hospital. Problems of initiating the group are presented. Both preliminary findings and research possibilities are discussed.
