[Studies on organ-related postoperative utilization of administered amino acids].

The exclusive postoperative supply of carbohydrates resulted in a significant improvement (24%) of cumulative nitrogen balance, because increased insulin production apparently caused augmented recycling of amino acids liberated during catabolism. Through addition of amino acids the decreased branched chain amino acids returned to normal levels, whereas the significant fall in plasma proteins with short half-life remained unchanged. Therefore it is assumed that the available amino acids (64% of supply) have predominantly anabolic functions in peripheral tissues and fewer are used to synthesize plasma proteins in the liver.

[Behaviour of serum phosphorus level during the postoperative period (author's transl)].

Behaviour of serum phosphorus and phosphate-excretion in urine has been observed during the postoperative period in 40 patients who obtained different amounts of phosphate-substitution. The findings were compared with 8 patients who had a phosphate-depletion syndrome. It was shown, that the appearance of hypophosphataemia during the postoperative period can be prevented by a daily supply of at least 25 mmol phosphate. If a phosphoate depletion syndrome has occurred, the substitution of 40 to 50 mmol phosphate and simultaneous reduction of caloric supply is recommended. Furthermore, routine control of serum phosphorus level and, if necessary, measurement of phosphate excretion in urine should be performed in any type of postoperative parenteral nutrition.

[Risk of combined surgical procedures in operations of colon and rectum (author's transl)].

In a period of 4 years in the Clinic of Abdominal- and Transplantationsurgery of the Medical School Hannover 63 combined surgical procedures were performed in 56 patients with operations of colon and rectum. In comparison with a group of patients, who were treated with similar operations as a single procedure, the increase of risk was studied. Small combined procedures like appendectomia, herniotomia, exstirpation of cysts or removal of diverticula indicate no elevation of the operative risk. Cholecystectomy or prostatectomy, though increasing the entire trauma of operation, did not elevate the rate of complications significantly. In accordance with our results outlines regarding the performance and indication of operations in combination with colon- and rectum surgery are discribed.

Multiple transplantation of islets of Langerhans.

In 6 diabetic rats about 1,000 isolated islets of Langerhans were transplanted into the liver with little effect on the uninsulinism and hyperglycemia. Subsequently, a second transplantation resulted in a decrease of blood sugar to normal and in a reversal of the loss in body weight. Finally, in animals who had reversed to diabetes again, a third islet transplantation was performed. These rats with a total of 3,100 islets showed a decrease in the glucose levels from 250-305 to 110 +/- 27 (X +/- SEM) mg/100 ml. The insulin level after glucose stimulations was measured at 27 less than X less than 35 micromicron/ml.

[Comparison of peripheral parenteral feeding with an exclusively carbohydrate diet during the postoperative period].

40 patients, who underwent abdominal surgical procedures, received 3 different types of peripheral-venous postoperative infusions. Two groups had peripheral parenteral nutrition with different amounts of amino acid substitution, whereas a comparable group only received carbohydrates and electrolytes with same energy content. Regarding plasma proteins there was no evident difference among the various groups. In all patients, however, we found a significant decrease of protein-levels in the early postoperative period indicating a malnutrition at this time. Supply of amino acids had a significant positive effect on nitrogen balance. Utilisation of added protein theoretically came up to 65-70%. Therefore postoperative peripheral parenteral nutrition should be preferred.

[Hepato-biliary sequence-scintigraphy in pre- and postoperative upper abdominal diagnosis. Studies on 200 patients using the radiopharmacon 99mTechnetium-Solcoscint-Hepatobida].

Hepato-biliary dynamic studies with 99mTc-Solcoscint-Hepatobida are suggested in the pre- and postoperative investigation of patients with possible surgical disease of the biliary tract. The equipment used for the studies was a gamma camera. The diagnostic criteria used were those of blood retention, liver, gall bladder, and biliary tree visualisation. Results from findings in 200 patients are presented to demonstrate the ability of the technique; especially to disclose evidence indicating intra- or extrahepatic causes of cholestasis.

[Use of a new method for the determination of the duodeno- and jejunogastral reflux after gastric surgery].

The use of hepato-biliary sequential scintigraphy with 99mTechnetium as a new method to demonstrate duodenogastric or jejunogastric reflux is described. In contrast to other techniques, the benefit of minimal stress for patients, routine practice, and combined measurement of fasting and postprandial reflux is pointed out. First clinical studies in postoperative patients are presented and it is accentuated, that this technique has a special field of application in gastric surgery.

[Combined surgical procedures in operations of stomach, duodenum and biliary system (author's transl)].

In a period of 4 years in the Clinic of Abdominal- and Transplantation Surgery of the Medical School Hannover in 52 patients with operations of the biliary system 56 combined surgical procedures and in 48 patients with operations of stomach and duodenum 51 combined surgical procedures were performed. In comparison with groups of patients, treated with similar operations as single procedures, the increase of risk was studied. It was found, that combined procedures of medium-sized trauma, which mostly depended on simultaneous injuries of both organs, caused an elevation of operation stress as well as an increase of complications. Small combined procedures indicated no elevation of risk. In accordance with our results, we postulate a very careful reflection on indication of combined surgical procedures together with operations of stomach, duodenum or biliary system.

[Polyposis coli. Comparison between the familial and non-familial form].

Since 1970 16 patients with polyposis coli have been treated at the Department of Surgery, Medical School, Hannover. Of these, 9 patients showed the classic type of familial polyposis, while 7 patients denied a positive family history. Comparing both groups, the nonfamilial type of polyposis coli can be separated as well from the familial polyposis as from multiple adenomas of the large bowel. Operative treatment in both groups of polyposis coli and management of nonfamilial polyposis coli are discussed.

Percutaneous transhepatic angioplasty and stent implantation for prehepatic portal vein obstruction.

Twenty-nine months after a Whipple procedure for pancreatic carcinoma, a 47-year-old woman developed esophageal variceal bleeding. Percutaneous transhepatic portography revealed a severe stenosis of the portal vein with prehepatic portal hypertension and collateral circulation mainly to the gastric and esophageal veins. Percutaneous transhepatic balloon angioplasty was used to dilate the stenoses, but it did not remove the stenosis sufficiently. Therefore, an 8-mm, self-expandable stent was implanted, creating a nearly normal lumen without a pressure gradient. Portal hypertension was relieved, and the patient had no recurrent variceal bleeding for the 5 months up to her death.
