[Cystic chondromalacia of the auricle. A case report. Review of the literature].

Cystic chondromalacia is a clinical and histopathological entity which can be clearly distinguish from all other cystic lesions of the auricle. We report a case which was clinically asymptomatic and involved the scaphoid fossa of the anterior surface of the pinna. The pathological process consisted of degenerative changes of the auricular cartilage which produced a cavity containing a serous fluid. No etiologic factor was found, in particular no trauma. The posterior wall of the cyst was excised under local anesthesia. This case allowed us to review the clinical, histopathological features and surgical difficulties of this rare lesion.

[Basocellular epitheliomas of the eyelids].

On the basis of a retrospective study of 354 basal-cell epitheliomas of the eyelids, we present a study of the most frequently used treatments: surgery, conventional radiation therapy and curietherapy. The results of treatment, both functional and oncological, are generally satisfactory, whatever the method chosen, but with a set of arguments in favor of surgery. The major therapeutic indications are discussed, without formally ruling out any treatment.

Cloning and characterization of two tandemly arranged DNA methyltransferase genes of Neisseria lactamica: an adenine-specific M.NlaIII and a cytosine-type methylase.

The gene encoding the Neisseria lactamica III DNA methyltransferase (M.NlaIII) which recognizes the sequence CATG has been cloned and expressed in Escherichia coli. DNA sequencing of a 3.125 kb EcoRI-PstI fragment localizes the M. NlaIII gene to a 334 codon open reading frame (ORF) and identifies, 468 bp downstream, a second ORF of 313 amino acids, which is referred to as M.NlaX. Both proteins are detectable in the E. coli coupled in vitro transcription-translation system; they are apparently expressed from separate N. lactamica promoters. The N-terminal half of the previously characterized M.FokI, which methylates adenine in one of the DNA strands with its asymmetric recognition sequence (GGATG), is found to have 41% sequence identity and a further 11.7% sequence similarity with M.NlaIII. Among the conserved amino acids is the wellknown DPPY sequence motif. With one exception, analysis of the nucleotides coding for the DP dipeptide in all known DPPY sequences shows the presence of an inherent DNA adenine methylation (dam) recognition site of GATC. A low level of expression of M.NlaX in E. coli prevents the elucidation of its sequence recognition specificity. Sequence analysis of M.NlaX shows that it is closely related to the group of monospecific 5-methylcytosine DNA methyltransferases (M.EcoRII, Dcm, M.HpaII and M.HhaI) which all have a modified cytosine at the second position of the recognition sequences. Both M.EcoRII and Dcm amino acid sequences are about 50% identical with M.NlaX; a considerable degree of sequence identity is found in the so-called variable region which is believed to be responsible for sequence recognition specificity. M.NlaX is probably the counterpart to the E. coli Dcm in N. lactamica.

[Nasal reconstruction: a comparative study between the Converse scalping flap and expanded median forehead flap].

The authors, reviewing a series of 16 at least subtotal nasal reconstructions over the past 4 years, discuss the respective advantages of the two methods, which both give good results for this purpose. Nowadays, the balance favors expansion despite its duration and complexity for the very significant aesthetic benefits in terms of forehead scarring. The guidelines concerning this technique employed in 6 cases are described and reviewed in detail. Nevertheless, the Converse scalping flap remains a simple and quick solution especially after cancer resection in the elderly; some details seem important to diminish its sequelae.

[Use of a customized nasal airflow mask in patients with Duchenne's myopathy].

Manufacture of personalized nasal masks by Biostar-press hot stamping of a bioplast plate has allowed for better prognosis of children with muscular dystrophy owing to the usually well supported, air-tight ventilation provided. Results have been encouraging and motivate us to utilise this type of mask in other patients as well, namely those with sleep apnea.

[The subperiosteal subtemporal approach in the surgical treatment of craniostenosis. Apropos of 9 cases].

The authors describe the approach consisting of direct subperiosteal, subtemporal cleavage based on existing anatomical continuity between the subtemporal plane and the cranial periosteum in young children. This allows for downward cleavage up to the level of the malar processes, for dissecting both orbits as well as the external columns. Frontoorbital remodeling which is needed for management of craniostenosis may be readily carried out with the improved field of vision. It will suffice, then, to depress the scalp and to suture the skin to obtain perfect repositioning of the temporal muscle, remaining one piece with the cutaneous plane and maintained steady thanks to the salvaged continuity of the periosteum with the subtemporal plane. Utilization of this approach avoids having to cleave the temporal muscle superficially, to section and then reinsert it to the bony frontoorbital fascia after remodeling, thereby constituting a simplified technique of craniostenosis management. Experience gained in craniostenotic children treated via this direct subperiosteal subtemporal approach opens the way for possible applications in adult traumatology patients, as well as for the constitution of unilateral pterion flaps without jeopardizing the frontal ramus of the facial nerve.

[Reconstruction of the vermilion border after vermilionectomy. Apropos of 16 cases].

The authors are concerned with 16 recently operated cases, and express their renewed interest in vermilionectomy in the management of precancerous states affecting the vermilion, immediately followed by reconstruction with a mucous advancement flap. After describing the method, the authors stress the benefits afforded by the method from both oncologic and prophylactic viewpoints. Functional and esthetic results are excellent.

High-performance liquid chromatographic determination of tauro- and glyco-conjugated bile acids in human serum.

A method for the identification and individual determination of the ten tauro- and glyco-conjugated bile acids is described. It consists in a specific three-step extraction from small serum samples (500 microliters), high-performance liquid chromatographic separation and direct spectrophotometric detection at 119 nm. Extraction can be checked by the use of an internal standard. The reproducibility, recovery and separation of fractions were satisfactory.

[Malignant eccrine poroma. Apropos of 2 facial sites].

The authors report 2 cases of malignant eccrine poroma (M.E.P.) located on the face: one on the auricle which was treated by triangular shape resection, the other on the nasal tip which underwent a reconstruction with a Converse scalping flap after large surgical resection removed underlying skin and cartilages. A study reviewing the main features of M.E.P. is presented on the basis of these clinical cases. M.E.P. are skin appendage tumors originating from the intra epithelial portion of the eccrine sweat duct. Such denominations as malignant hidroancanthoma, eccrine porocarcinoma, poroepithelioma, correspond to the same tumor. They are very rare: 73 cases have been reported in the world literature with only 12 located on the face; the main localisations involve the lower extremities, but the palmo plantar areas are involved more frequently in benign tumors. Elderly people over 60 are concerned in almost all cases. Many of the patients have noticed a lesion for years before a diagnosis was made suggesting one of the etiopathogenic theories that M.E.P. may result from malignant transformation of a benign eccrine poroma. Their clinical appearance is non specific nevertheless their malignant nature is suspected and misdiagnosis with basal cell carcinoma is frequent. Microscopic findings demonstrate the diagnosis by showing the association of malignant features with eccrine poroma. These tumours invade both by superficial spead and by deep extension towards dermal and hypodermal structures. Their prognosis is marked by the rapid development, in approximately on half of cases, of cutaneous, lymp node and visceral metastases. Clinical and histological examination have no prognostic value exception the case of deep invasion.(ABSTRACT TRUNCATED AT 250 WORDS)

[Therapeutic measure in the repair of traumatic loss of substance of the scalp].

Repair of scalp tissue losses must reach two ends: Cover cranium and its contain while restoring hair. Only scalp flaps can attain these aims, but their disadvantage is most of the time to shift alopecia. For the major tissue losses, since skin expansion, treatment proceedings have changed: an immediate and temporary cover by a skin graft can be proposed, instead of multiple scalp flaps or free flaps. Few cases illustrate this alternative allowing discussion about their respective indications.

[Esthetic refinements in cheiloplasty or the use of the V, W, X, Y, Z technics].

Classically, V-wedge excision of the lip allows direct closure of defects less than a third of its width. But undue tension may cause dehiscence of the suture lines responsible of narrowing of the mouth and excessive scarring especially on the external mucosa. To avoid these aesthetic complications modified incision should be made: W followed by a V-Y advancement closure, completed by a Z plasty on the vermilion.

A constitutive member of the rat cytochrome P450IIB subfamily: full-length coding sequence of the P450IIB3 cDNA.

The prototypic members of the rat liver cytochrome P450IIB subfamily, P450b and P450e, have long been the subjects of intense interest, in part because they are highly inducible by phenobarbital (PB). We have previously cloned and sequenced an 858-bp cDNA fragment (the PB24 insert) that encodes the carboxy-terminal portion of a P450b/P450e-like protein, henceforth referred to as P450IIB3. A Bam HI-Eco RI fragment of the PB24 insert hybridizes with a 1.9-kb mRNA present constitutively in rat liver and not inducible by PB (Affolter et al., 1986). We have now obtained, from a lambda gt11 rat liver cDNA library, cDNA inserts corresponding to the complete coding sequence of the IIB3 mRNA. The 491-amino acid IIB3 protein sequence, deduced from the cDNA sequence, is 77% identical to that of P450b. Northern blot analysis, with a IIB3-specific oligonucleotide probe, confirmed the constitutive presence of the polyadenylated 1.9-kb IIB3 mRNA in male rat liver. The IIB3 mRNA was undetectable in the lung, kidney, and prostate. It was constitutively present, and not inducible by PB, in female rat liver. Our results demonstrate unequivocally the existence of a constitutive member of the rat P450IIB subfamily. The remarkable inducibility of the P450b/P450e genes is in striking contrast to the absence of a PB effect on IIB3 gene expression. The molecular basis for this difference remains to be revealed.

[Possibilities of the use of the Converse scalp flap in maxillofacial surgery. Review of 13 cases].

13 Converse scalping flaps have been done in the past 3 years. 11 cases were for nasal reconstruction, at least half nose which is its main indication including 4 after gun shot wounds and 7 for invasive skin cancers. The preference of this flap is discussed versus the other processes for reconstruction: median forehead flap whatever its design, Washio or orther fronto-auricular flaps, free flaps. Lining and framework repair are done in the same operation than cover when usefull. 2 other clinical cases illustrate other areas than nose where the Converse flap can be employed.
