Tumor-associated proteolytic factors uPA and PAI-1 in endometrial carcinoma.

The levels of plasminogen activator urokinase (uPA) and of its inhibitor (PAI-1) were measured by use of ELISA in the cytosol of tissue homogenates obtained from endometrial carcinomas and the marginal, tumor-free endometrium of postmenopausal patients (n = 64). Significantly higher median levels of uPA and PAI-1 were found in malignant endometrium (uPA 1.89 ng/mg, PAI-1 3.04 ng/mg) compared to tumor-free endometrium (uPA 0.84 ng/mg, PAI-1 1.01 ng/mg). Concerning uPA, no significant differences were found in dependence on histomorphological prognostic factors (staging, grading), but the median level of PAI-1 was significantly higher in G2/G3 carcinomas compared to G1 tumors (5.08 ng/mg vs 2.19 ng/mg). Because of the good prognosis of operated patients with endometrial carcinomas, the prognostic value of uPA and PAI-1 can only be decided by a larger number of patients and a long observation time.

[The prognostic value of cathepsin D in endometrial carcinoma].

The concentration of the lysosomal protease Cathepsin D was tested by means of RIA. We used the cytosols of 44 endometrial carcinoma tissue specimens and of the corresponding normal endometrial tissues (n = 42) obtained from the respective uteri after hysterectomy. All patients were in postmenopausal stage. A significant higher level of Cathepsin D expression was found in endometrial carcinoma (median value = 24.2 pmol/mg) compared to normal endometrium (median value = 11.4 pmol/mg). No difference in Cathepsin D concentration was found in relation to grade of histological differentiation (G1 vs. G2/G3), depth of myometrial invasion and tumor stage (stage IA vs. IB/IC vs. > I) as well as state of estrogen receptor (ER) and progesterone receptor (PR). After an average follow-up period of 18 months there was no significant difference in survival rate of operated patients depending on Cathepsin D concentration. However, the prognostic significance of this parameter need to be evaluated after a long-term follow-up. The overexpression of Cathepsin D seems to be important for exact biologic characterization of each endometrial carcinoma with respect to local proteolytic activity. The enhanced activity of cathepsin D may therefore serve as an additional objective malignancy criterion independent of the well-known prognostic factors.

[Anesthesia for laparoscopy in sterile patients].

During 5 years 382 laparoscopies were carried through in female patients with sterility in different kinds of anaesthesia (Intubation anaesthesia, spinal anaesthesia, and i.v. anaesthesia). We found the most advantageous results in the cases of i.v. anaesthesia with Ketamin and Diazepam.

[The cooperation between anesthesiologists and surgeons].

With the development of anaesthesiology as an independent medical field, the anaesthesiologists were structurally brought together as an autonomous unit. They are not included in the leadership structure of the surgical institutions in which they work. Between surgeons and anaesthesiologists there is no system of super- or subordination. They participate in the operation on an equal footing. This requires regulation or responsibility in all phases of the operation and the postoperative therapy as well as an atmosphere of good team-work. In the interest of the patient, vital decisions are to be made jointly.

[Therapeutic efforts and prenatal modification of selected disordered fetomaternal relations].

Based on the current view about disturbances of the feto-materno-placental unit we examined 37 women with suspected intrauterine fetal growth retardation and 12 women with threatened premature labor and looked for possibilities of antenatal therapeutic influence of nutritive, respiratory, endocrine and hemodynamic insufficiency. In a randomized study we can prove a normal increase of the distance between symphysis pubis and fundus as well, as the biparietal diameter not only following bedrest but also following bedrest with additional intravenous infusion of glucose respectively oral application of galactose. The high rate of hypotrophic babies is no argument against a positive influence on nutritive insufficiency, but the acquired retardation can not be compensated totally. Neither a positive influence on the endocrine insufficiency nor the moderate respiratory one could be found. As well by maternal transcutaneous nerval stimulation as by maternal oxygen inhalation an oral long-term tocolysis we can demonstrate a considerable improvement of the uteroplacental perfusion measured with isotopes. These positive aspects are basis for further investigations. An important supposition to a successful therapy remains an early diagnosis.

[Analysis of eclampsia cases of the years 1957-1982 at the University Gynecologic Clinic in Leipzig].

Of the total of 104 873 patients who have been delivered between 1957 and 1982, 99 women (0.094 per cent) were diagnosed as having eclampsia. Maternal lethality was 4.0 per cent, and perinatal lethality was 20.8 per cent. Delivery was spontaneous in 34.3 per cent, and caesarean section, vacuum extraction, and forceps delivery were performed in 45.5 per cent, 8.1 per cent and 12.1 per cent respectively. The incidence of eclampsia decreased generally. The prognosis for both mother and fetus could be improved by modern methods of intensive care in close connection with termination of pregnancy.

Birth Weight↗

[Anaesthesia and therapy for gestosis - impact on the newborn (author's transl)].

Adaptation of 33 newborns and their clinical symptoms during the first weeks of age were studied retrospectively, with reference to cases in which the pregnant mothers had undergone treatment for hypertension, anaesthesia, and caesarean section. Primary adaptability was unambiguously limited in all newborns, with symptoms of impaired sensual motoricity being additionally recorded from 16 of them. Apnoeic seizures occurred to six children on their first day of age and called for continuous cardiorespiratory monitoring. Premature and hypotrophic newborns were primarily affected. Those delayed symptoms were attributed, first of all, to medicamentous effects. More caution should be exercised in prescribing neurodepressors of the Diazepam type, particularly in early pregnancy, say, prior to the 37th week. - An analysis was made of blood gas data in the blood of umbilical arteries of infants whose birth had been preceded by gestosis or eclampsia. Yet, their intranatal respiratory situation was not found to be basically worse than that of a control group.
