[Percutaneous transluminal endarterectomy of femoro-popliteal atheromatous lesions. Technique. Indications. Results after 6 months].

Percutaneous atherectomy was performed in 25 patients with 30 symptomatic femoro-popliteal stenosis. INITIAL RESULTS: 23 patients were successfully treated with disappearance of claudication and a mean 0.35 increase in the ankle-arm-index. Two early thrombosis occurred. Angiographic analysis: residual stenosis lower than 30% in 25 lesions; no dissection or embolization; effective treatment of eccentered and calcified lesions; no significant dissection after additional angioplasty. RESULTS AT 6 MONTHS FOLLOW-UP: (17 patients, 19 lesions): stable improvement in 14 patients; 2 restenosis; 1 new disease. Atherectomy restores a large lumen with minimal wall trauma, thus perhaps decreasing the restenosis rate.

The role of gastric lipolysis on fat absorption and bile acid metabolism in the rat.

In vivo studies were carried out in young Sprague-Dawley rats to examine the role of gastric lipolysis on fat absorption and bile acid metabolism. When fed by gastric perfusion 5 times (corn oil, 4 g/day) their usual dietary intake of fat, rats deprived of lingual lipase by the creation of an esophageal fistula had a significant degree of fat and bile acid malabsorption as well as a shortened bile acid half-life when compared to animals with a gastrostomy. The % fat absorption, bile acid loss and bile acid pool were normal in 2 groups of esophageal fistula rats fed the same quantity of corn oil or twice (8 g/day) that amount as a fine emulsion. In view of a negligible gastric lipase activity in animals with an esophageal fistula and of decreased hydrolysis of a triglyceride test meal, these data suggest that gastric lipolysis is of physiological importance in situations where lipolytic mechanisms are stressed by a large fat intkae. Its principal role is to potentiate intestinal lipolysis by facilitating the emulsification of dietary lipids through its formed products and, therefore, the contact of pancreatic lipase with its substrates.

Inhibition of T-cell differentiation by an antibody to a soluble thymic factor.

An antiserum to chicken soluble thymic factor (STF) was prepared in rabbits and absorbed with chicken thymocytes until it reacted only with thymic reticuloepithelial cells. When injected into 6-day-old embryonated eggs, this antiserum caused a severe depletion of small lymphocytes in the cortical region of the thymus which was filled with large undifferentiated cells. In addition, reticuloepithelial cells displayed a marked disruption of cytoplasmic elements. The percentage of thymic lymphocytes bearing the T antigen was greatly reduced. It is believed that a factor responsible for differentiation of stem cells into T cells has been inhibited.

The authors carried out a study on 53 patients operated on for varices with the object of determining the risk of postoperative thromboembolism. They tried to answer three questions : 1) What is the magnitude of the risk of thromboembolism after this kind of surgery? 2) What is the value of the different methods of detection available? 3) What attitude should be adopted towards therapy, not of thromboembolism as a complication, but of the threat of thromboembolism?

The authors consider the Extent of the several anatomical structures which constitute the deep venous network, and the part they can take in the case of deep venous insufficiency. Congenital Vascular diseases are called back in mind when considering Embryology, and point out the extent of the deep venous state; such veins can gather in periarterial plexus, at the muscular Level, and in going past all along main arterial axes. Among all the critical points of these axes, we must keep the popliteal veins, the femoral crossing, and the iliocaval one and elsewhere, the important question of venous valves and communicating and perforating veins. The part of the lymphatic network must be pointed out, because of its extent in the stasis phenomenons.

According to an embryological basis, the Authors analyze the development of the vascular system and try to establish a differentiation between angiodysplasias and abnormalities. They define the characteristics of each group and make an attempt at classification. The major characteristics of differentiation are the evolutional aspects and the presence of arteriovenous fistulas.
