Hairy cell leukemia: a tumor of pre-plasma cells.

Monoclonal antibodies defining B-, T-, and myeloid-restricted cell surface antigens were used to characterize the lineage and state of differentiation of tumor cells isolated from 22 patients with hairy cell leukemia (HCL). These tumors were shown to be of B lineage because they strongly expressed the B cell-restricted antigens B1 and B4 and lacked T cell- and monocyte-restricted antigens. Moreover, the strong expression of the plasma cell-associated PCA-1 antigen on the majority of hairy cells suggested that these tumors correspond to later stages of B cell ontogeny. Dual fluorescence experiments further confirmed that HCL splenocytes that coexpressed B1 and PCA-1 demonstrated both the morphology and tartrate-resistant acid phosphatase positivity of hairy cells. The observation that some hairy cells either spontaneously produce immunoglobulin (Ig) or could be induced to proliferate and secrete Ig provides complementary support for the view that HCL is a pre-plasma cell tumor. However, staining of hairy cells with anti-IL2R1 monoclonal antibody, which is directed to the T cell growth factor receptor and/or with the anti-Mo1 reagent, directed to C3bi complement receptor, distinguish these cells from currently identified B cells.

The parastomal hernia.

The development of a hernia in relation to an intestinal stoma is a common event. Complications of such a hernia are rare, but sometimes serious. For the occasional parastomal hernia that requires operation, a number of different techniques are available.

Can insulin-treated diabetics be given beta-adrenergic blocking drugs?

Lack of awareness of hypoglycaemia leading to loss of consciousness is a serious problem in some insulin-treated diabetics, and beta-blocking drugs may increase this hazard. A prospective study was therefore carried out over eight months to determine the incidence of hypoglycaemic episodes in 50 insulin-treated diabetics taking beta-blockers, as compared with 100 diabetic controls matched for age, sex, and duration of diabetes. The incidence of loss of consciousness from hypoglycaemia was the same in both groups and was unrelated to the dose of beta-blocking drug used. Five patients taking beta-blockers and 10 controls had episodes of unconsciousness, but four of these patients taking beta-blockers had had similar episodes in the two years preceding treatment. It is concluded that beta-blocking drugs are generally safe in insulin-treated diabetics and that hypoglycaemic unconsciousness resulting from their use is rare.

The "yo-yo" operation for inguinal hernia.

This article presents a new principle in the repair of difficult inguinal herniae. The testis is mobilized from the scrotum, returned to the abdominal cavity, brought down again through a new canal constructed through the rectus abdominis muscle, and then restored to the scrotum. The original inguinal canal can then be completely obliterated.

Normal alkaline phosphatase in primary biliary cirrhosis.

A patient with primary biliary cirrhosis is described. Throughout her illness the serum alkaline phosphatase remained normal and this unique feature emphasizes the importance of histology in diagnosing the disease.

Effect of declining mental health service use on employees of a large corporation.

This study examines concurrent changes in use of mental and general health services and in annual sick days among 20,814 employees of a large corporation. From 1993 to 1995 mental health service use and costs declined by more than one-third, more than three times as much as the decline in non-mental health service use. However, employees who used mental health services showed a 37 percent increase in use of non-mental health services and significantly increased sick days, whereas other employees showed no such increases. Savings in mental health services were fully offset by increased use of other services and lost workdays.
