Spinal cord lesion management in Italy: a 2-year survey.

DESIGN: Prospective 2-year survey from 1 February 1997 to 31 January 1999. OBJECTIVES: To compare the route from injury to rehabilitation, and the outcome of care in a large sample of traumatic (T) and nontraumatic (NT) spinal cord lesion (SCL) patients at their 'first admission'. SETTING: T and NT SCI patients consecutively admitted to 37 SCL centres in Italy. METHOD: Data were recorded on simple, computerised, closed-question forms, which were Centrally collected and analysed. Descriptive and inferential analysis was conducted to define the characteristics and compare the T and NT populations, and to identify correlations among the variables examined: time from the event to admission (TEA); pressure sores (PS) on admission; length of stay (LoS) and destination on discharge. RESULTS: A total of 1014 SCL patients, 67.5% with a lesion of T and 32.5% of NT aetiology were analysed. The subjects in the T group were younger (median 34 versus 58 years), with higher probability of cervical involvement (OR 2.47, CI 1.8-3.4) and completeness of the lesion (OR 3.0, CI 2.3-4.0), shorter median TEA (37 versus 64 days, P<0.0001) and less frequent admission from home (3.6 versus 17.4%) compared to the NT group. TEA and PS on admission were analysed as indicators of the efficacy of the courses from injury to rehabilitation. Longer TEA was reported for people with NT aetiology, admitted to rehabilitation centre (RC), not locally resident, transferred from certain wards and to a lesser degree female subjects and those with complications on admission. PS were associated to completeness of lesion, longer TEA, admission to RC, nonlocal residence and coming from general intensive care units, or general surgery wards. Median LoS was 99 days (mean 116 and range 0-672), and was statistically shorter in the NT group (122 versus 57 median, P<0.00001). Upon discharge, bladder and bowel autonomy were, respectively, obtained in 68.1 and 64.5% of the whole population without significant difference between the T and NT groups. A total of 80.2% of patients were discharged home and the following factors: not living alone, being discharged after longer LoS, having sphincterial autonomy and no PS, were all independent predictors of outcome. CONCLUSION: There are important obstacles in the admission route to rehabilitation facilities, greater for NT, as longer TEA and more complications on admission testify. Moreover, the LoS is shorter for NT population. Our findings suggest that rehabilitation outcome could be improved through an early multidisciplinary approach and better continuity between acute and rehabilitation care, especially for the 'neglected' NT SCL patients.

Risk factors for cutaneous melanoma: a case-control study in Argentina.

BACKGROUND: Although cutaneous malignant melanoma (CMM) is still a relatively rare neoplasm, incidence rates are increasing in many white populations worldwide. A case-control study was performed in Argentina to estimate the risk factors for CMM. METHODS: The study group comprised 101 hospital cases with histologically verified melanoma and 246 controls, frequency matched by age, sex, and hospital. Individuals were interviewed about demographic and constitutional factors and sun exposure history. RESULTS: The following risk factors were significant in the logistic analysis: nevi on arms (odds ratio (OR) = 6.3 for 20 or more), skin phototype (OR = 4.1 for type l or ll), holidays spent at the beach (OR = 4 for more than 6 weeks lifelong), fair skin tone (OR = 3.4), light eye color (OR = 2.8), sunburn before the age of 15 years (OR = 2.4), and participation in outdoor sports (OR = 3.2 for more than 5790 h accumulated in a lifetime). Occupational sun exposure was not related to CMM risk. Educational level, hair color, and the presence of freckles were not independent risk factors for CMM. CONCLUSIONS: The risk factors found for the studied population, with nearly 90% of the individuals born in Argentina and with nearly 50% with four southern European grandparents, were consistent with those described for other countries. This investigation has resulted in findings that may be of use in preparing actions to prevent CMM in Argentina; these should be directed at those individuals who present the above mentioned risk factors. These actions should include instructions to avoid excessive sun exposure, with the aim to reduce melanoma incidence.

Reciprocating gait orthoses: a multicenter study of their use by spinal cord injured patients.

OBJECTIVE: Use of reciprocating orthosis (RGO, ARGO, HGO) by 74 patients with complete traumatic spinal cord injury was studied. Lesion levels ranged from T1 to T12. STUDY DESIGN: Patients were enrolled in seven Italian rehabilitation centers an average of 37 weeks after acute trauma. The training period averaged 39 days. Discontinued use of the device was recorded at follow-up. Before discharge, functional walking, gait velocity, donning and doffing time, and the ability to climb stairs were recorded. The same items were evaluated at 6-month follow-up. RESULTS: At follow-up, 24 patients had abandoned the orthosis, 19 used the device for therapeutic exercise, 31 used it for functional gait, and 9 also used it outside the home. Functional walking was correlated with age, level of lesion, ability to climb stairs, duration of training, and lapse of use of the orthosis. The results showed a correlation between use of the orthosis and the ability to climb stairs, as well as a high Garrett score. CONCLUSION: This orthosis is not considered as an alternative to the wheelchair, despite its greater speed, simplicity of use, and greater autonomy provided to the user.

Assessing risk factors for chronic diseases.

A pilot survey of men aged 35 and over in Buenos Aires indicated that many had one or more risk factors for chronic diseases. A low response rate hampered the investigation: on average it proved necessary to visit several homes in order to obtain one interview. Furthermore, at the cost of US$ 10 incurred per interview, large prospective investigations would be precluded in most developing countries, but case-control studies assessing tobacco use and other risk factors retrospectively would be a good alternative.

Prognostic factors in laryngeal carcinoma. Experience in 296 male patients.

BACKGROUND: Laryngeal cancer is the primary cause of death among patients with head and neck cancer in Argentina. Different prognostic factors have been incidentally dealt with in a number of articles, but there are few studies of prognostic variables, such as age, tobacco, and alcohol, using multifactorial analysis. METHODS: Between 1969-1987, 296 male patients with epidermoid carcinoma of the larynx treated at the Institute of Oncology "Angel H. Roffo" were studied regarding prognostic factors of survival. The effect of age, tobacco, alcohol, histologic grade, extent of primary tumor, and cord mobility on survival were assessed by means of the Cox proportional-hazards model. RESULTS: The univariate analysis showed that primary tumor extent, nodal involvement, stage, alcohol intake, previous tracheostomy, and cord mobility were statistically significant predictors of survival. Multivariate analysis disclosed only nodal involvement, alcohol intake, and cord mobility as statistically significant variables in regard to survivorship. With these variables, a hazard index was calculated for each patient, and four risk groups were formed. Five-year survival rates were as follows: Group A, 79%; B, 69%; C, 47%; and D, 14% (A versus C, P < 0.005; A versus D, P < 0.005). CONCLUSIONS: This study confirmed the prognostic impact of nodal involvement and also showed the importance of cord fixation and alcohol drinking as predictors of survival.

Leather tanning workers: chromosomal aberrations in peripheral lymphocytes and micronuclei in exfoliated cells in urine.

A cytogenetic study was performed on workers of a leather tanning industry. Two different approaches for the biological monitoring of the individuals were used: chromosomal aberration analysis in peripheral lymphocytes and the frequency of micronucleated cells exfoliated in urine samples. 26 men working in the sections considered to present a greater risk were included in the study. Controls were 20 men that were not exposed to chemicals. The percentage of abnormal cells was higher in workers than in controls. Smokers showed higher values of chromosome breaks than non-smokers in both groups. These differences were not statistically significant. The percentage of cells with chromatid and chromosome gaps in workers and controls was different (p less than 0.01). A slight but not significant increase in the mean percentage of micronuclei was observed in the exposed group. We conclude that exposure to chemicals during leather tanning did not produce genotoxic effects measured by chromosomal aberrations in peripheral lymphocytes and micronuclei in urine in this group of workers.

Nitroso-aldicarb induces sister-chromatid exchanges in human lymphocytes in vitro.

Nitroso-aldicarb was tested for its ability to induce sister-chromatid exchanges (SCE) and cell-cycle delay in human peripheral blood lymphocytes in vitro. This derivative of aldicarb induced a dose-dependent increase in SCE values per cell. In addition, a slight decrease in the successive mitotic progression of cells in culture was observed.

Sister-chromatid exchanges and chromosomal aberrations in a population exposed to pesticides.

Sister-chromatid exchanges (SCE) and chromosomal aberrations were studied in a population of floriculturists occupationally exposed to organophosphorus, carbamate and organochlorine pesticides. Blood samples from 36 individuals from a community of 154 persons of asiatic origin were obtained. Among the group sampled, 21 individuals exhibited at least one symptom of chronic intoxication. SCE analysis was performed in 14 symptomatic and 13 asymptomatic persons. The asymptomatic group showed a SCE frequency of 5.47 +/- 1.03 and the symptomatic group a frequency of 6.45 +/- 1.19. Comparison between both groups with the Mann-Whitney 'U' test revealed a significant difference (p 0.0409). Case-control analysis of 9 pairs matched by sex and age also showed significant differences between both groups (p 0.0104). In contrast, the frequencies of chromosomal aberrations were not correlated with intoxication symptomatology, though a significant increment of exchange-type aberrations in relation to a group of non floriculturists was observed in the population studied.

Interferon induction by a tumoral DNA in an homologous culture system.

The interferon induction by Rous sarcoma DNA in an homologous culture system and a further insight of its kinetics and antimetabolites action were the principal aim of the present study. There was a direct relation between the dose of the inducer and the protection against the cytoplathogenic effect of the challenging virus (VSV) reaching the highest activity (68-75% CPE inhibition) with 100 mug RS-DNA. The kinetics of the induction revealed a peak inhibition by 18 hours after the inducer. Treatment with Actinomycin D evidenced that both interferon production and activity are modified. Its early addition resulted in a poor protection; but an accentuated interferon release was observed when antimetabolite was added 18 hours after the inducer. Similar results were obtained when its effect was studied on the activity of exogenous interferon. Interferon induction by Rous sarcoma DNA in an homologous system, allows the detection of a difference between tumoral and normal DNA at a biological level.
