[Fine-needle aspiration biopsy and cytologic diagnosis in thyroid disease-a 3-year experience].

Fine needle aspiration biopsy (FNA) is a morphological method, which may bridge the diagnostic gap between initial clinical and sonographic examination and histological diagnosis in thyroid disease. Our study was aimed at a review of 3-year aspiration cytology of the thyroid gland in clinical practice. 175 FNA were assessed (165 in females and 10 in males). Aspiration biopsy was performed using 20 to 23 G needles and aspirates were stained after Pappenheim. The highest number of performed FNA was between 40 and 60 yrs of age. The most commonly observed clinical diagnosis requiring FNA included multinodular goiter, Hashimoto thyroiditis and thyroid adenoma. These were also the most common cytological findings. Unsatisfactory specimens were found in 21% of aspirates. Most pitfalls were found in cases of multinodular goiter and Hashimoto thyroiditis. Our results are comparable to those of other clinical centers. They reflect the age-adjusted incidence of thyroid disease. We conclude that combination of ultrasound tomography, aspiration puncture, and cytological evaluation improves the diagnosis of thyroid disease and reduces the duration of the examination.

[Thyroid hormone dynamics during the thyrostatic treatment of Basedow's disease].

The efficacy of the generally accepted therapeutic scheme was studied in 52 newly discovered and untreated patients with Basedow's disease with the purpose to clarify the frequent failures of the thyrostatic treatment. The thyroid hormones levels were determined in dynamics initially and in the course of the disease. The results show that the efficacy of the thyrostatic drugs is individually different and depends on the supersensitivity of the thyroid gland. The remission occurred on the average on the 50th day--considerably earlier than the usually expected time limit. In order to study the expedience of the therapeutic scheme applied, the active dose required by the scheme was continued. As a result in the 4 month after the beginning of the treatment the thyrotropic hormone increased to a mean level of 35.7 mU/l--hypothyroid value. A monthly control of the thyroid hormones levels during the course of the active treatment is necessary and a reduction of the doses of the thyrostatic drugs when a remission is achieved must be performed.

[Creatine kinase in patients with diabetes mellitus].

In 72 diabetic patients the serum levels of creatine kinase (CK) and its isoenzyme MB were examined together with the levels of other enzymes most frequently examined in practice. A transient elevation of the CK level, mainly of the muscular isoenzyme, was found in 12.5% of the patients examined. In none of these patients lesions of the myocardium, thyroid gland or the striated muscles, which could explain the rise of CK, were found. There was a moderate correlation between the degree of glycemia and the CK elevation but there was no such correlation with other enzymes. With improvement of the carbohydrate metabolism the CK level became normal. The possible pathogenic mechanisms for the rise of the CK level in diabetes mellitus are discussed. The limited number of the patients examined does not allow final conclusions about these mechanisms.

[The TRH test using the Bulgarian preparation Protilerin in the diagnosis of hyperthyroidism].

The stimulation test--TRH test was carried out with the Bulgarian drug "Protilerin" in 63 patients (Basedow's disease--53 patients, basedoid goiter--5 patients, infiltrative ophthalmopathy--5 patients) and in 20 healthy controls. The healthy controls showed normal reaction to the stimulation. All patients with hyperthyroidism showed a suppressed response to the stimulation. In two of the patients with euthyroid infiltrative ophthalmopathy the response was similar to that of the healthy controls. The Bulgarian drug Protilerin is suitable for the performance of the TRH test in the diagnosis of hyperthyroidism.

[A case of Pendred's syndrome].

A very rate case of a combination of congenital deaf-mutism and sporadic goiter due to peroxidase insufficiency is presented. This condition is known as Pendred's syndrome. The special features of the case requiring surgical treatment in contrast to the generally accepted conservative attitude are pointed out.

[Dynamic thyroid hormone levels during the thyrostatic treatment of Basedow's disease].

The thyroid hormones and thyrotropic hormone levels were followed up dynamically in the course of thyrostatic treatment in 24 newly discovered patients with Basedow's disease. The following findings deserve special attention: 1. The early appearance of therapeutic remission--mean after 2.4 months of treatment, 2. The increase of the thyrotropic hormone following soon after remission is achieved--mean at the end of the forth month of treatment it reaches 19.5 mIU. The analysis revealed that this high mean level was due to 9 patients who had reacted with initial hypothyroidism though they were under the same therapeutic conditions. The data allow the conclusion that there are individual differences in thyroid suppressibility. This implies a dynamic follow up of the thyrostatic hormonal action and timely correction of the dose which would remove the consequences of improper dosage.

[Our experience in treating diabetes mellitus with the preparation Minidiab].

55 patients with non-insulin-dependent diabetes mellitus were treated with the new oral drug Minidiab. In 58.2% of the patients the carbohydrate metabolism was influenced. No relation between the immunoreactive insulin increase and the effect of the treatment was found. An extrapancreatic action of the drug is suggested as its hypoglycemic activity.

[Digestive system and bladder manifestations in diabetic autonomic neuropathy].

The survey covers the diabetic autonome neuropathy. After a general introduction, discussing the history, incidence, histological characteristics, pathogenesis and some of the diagnostic methods of diabetic neurovegetopathy , some problems, not sufficintly known in the clinical practice of the neurovegetopathy of the ga tro -intestinal and excretory systems were discussed. The survey contributes to the more thorough knowledge of that relatively rare complication of diabetes and to its more effective treatment.

[Somatotropic hormone in the serum of diabetics].

The results from the studies on basic values of somatotropic hormone (STH) in 142 patients with diabetes mellitus are reported. No linear dependence was found between blood sugar and STH level in serum in the morning before meals. As compared with the subjects from a control group, STH in serum of diabetics without retinopathy, with retinopathy, with obesity and renal insufficiency resulting from nephropathy was increased with a statistical significance. STH values in serum of patients without microangiopathies were lower than those in patients with manifested retinopathy and higher than those in patients with obesity, the difference being not statistically significant. The serum level of somatotropic hormone in patients with manifested renal insufficiency was higher, with statistical significance, than that of patients without renal insufficiency.

[Clinical x-ray studies of arterial calcifications in the lower extremities of diabetics].

The authors studied the X-ray grams of soft tissues of the lower limbs of 120 patients with diabetes mellitus--54 males and 66 females and 42 control subjects (18 males and 24 females). The average age of the latter was 56.3 and of the patient--52. According to their characteristic the calcifications were localized in intima (spotted) and in media (linear). Besides localization, the extend (whole artery or partial) and degree (light, pronounced) of calcifications were determined. Mediocalcinosis in the arteries of lower limbs of diabetics was established to be almost four times more frequent than that among the controls, the arteries of the thighs, legs and foot being equally affected. In 1/3 of the cases the whole vascular system of the lower limbs was involved, being more frequent in males, as compared with females, with a statistical significance, and correlated significantly with age and duration of the disease. Mediocalcinosis was more frequent with statistical significance, in patients, treated with sulfonylurea preparations. Attempt was made to explain that fact, besides by the older age of those patients with some humoral and hormonal characteristics of diabetes of the elderly, sensitive to SU preparations namely: the presence of hyperlipoproteinemia, hypersomatotropism and relative hyperinsulinism, favouring the proliferation of the cells of media and deposition of calcium salts in it. No statistically significant correlation was found between the incidence of mediocalcinosis on the one hand and the type of diabetes, severity of the disease, occupation, past infections, the presence of hypertension, obesity, retinopathy and disorders of lipid metabolism--on the other.

[Carbohydrate tolerance and serum insulin in hypertension].

The glycemic and insulinemic curves were followed up in 20 patients with hypertonic disease during the standard oral glucose-tolerance test. Normal glucose-tolerance (NGT) was found in 55%, suspected pathological (SPGT)--in 25% and pathological (PGT)--in 20%. Hypertonics have higher, later reaching its maximum and more prolonged insulin secretion. Normal or borderline normal glucose tolerance is maintained owing to the higher capacity of insulin secretion as compared with the control subjects. Glucose tolerance becomes pathological with the decrease or exhaustion of insulogenic index under that of the control subjects. The elevated blood sugar levels in patients with SPGT and PGT, accompanied with relative (statistically insignificant) hyperinsulinemia in the patients with NGT and PGT and statistically significant in patients with SPGT provide conditions for the enhancement of endogenous synthesis of triglycerides, related to atherogenesis. An opinion is expressed that: I. The hypertonic patients are threatened with atherosclerosis advancement not only by the effect of the increased vascular tone upon that process but also by the enhancement of triglyceride synthesis; 2. Hypertonic disease could be considered a risk factor in the origination of diabetes mellitus.
