BCL-2--general considerations.

BCL-2 protein is a oncoprotein considered to have an important role in the regulation of programmed cell death--apoptosis. The intracellular localization, the relation with other oncoproteins, the antioxidant function and the pathological implications are discussed.

[The study of the frequency and features of bacteria isolated in patients with non-tuberculosis respiratory disorders admitted in "M. Nasta" Institute in the first trimester 2001].

The aim of this study was to determine the spectrum of bacteria isolated in patients with non-TB respiratory disorders, in order to find the frequency of germs isolated globally and in each of the 6 clinical syndromes: suppurations, chronic obstructive diseases exacerbations (COPD, bronchial asthma), pneumopathies, post-TB syndromes, tumors and interstitial lung diseases. We found the greatest global frequency in anaerobes (35%). This is in concordance with the high frequency of broncho-pulmonary suppurations (51% of the cases studied). In second place comes Ps. aeruginosa (18%), followed by Str. Pneumoniae (16%), H. influenzae (11%) and Klebsiella pn. (10%). Considering the germs identified in the various clinical syndromes, we found a high frequency of anaerobes associated to suppurations (51%), interstitial lung diseases (43%) and tumors (37%), while Ps. aeruginosa is first in post-TB syndromes (50%) and COPD exacerbations (21%), equal to H. influenzae. In pneumonias, Str. Pneumoniae was most frequently isolated (38%) followed by H. influenzae (25%). The susceptibility testing of strains of Klebsiella and Ps. aeruginosa revealed the increasing tendency to resistance to broad spectrum antibiotics, especially for Ps. aeruginosa, with consecutive difficulties in finding the appropriate treatment.

[The pulmonary manifestations in AIDS].

Clinical and paraclinical experience in HIV infection, though the time elapsed since the first observations is relatively short, begins to get typical outlines. In the case of AIDS, the lung is the main place of opportunistic infections, other inflammatory processes and neoplasia. The present work deals with six clinical cases with positive serum tests for HIV and secondary respiratory phenomena such as: Kaposi sarcoma, pneumonia with Pneumocystis carinii, tuberculosis, candidosis, pneumonia with common germs. Particular aspects of treatment and disease evolution are commented.

[Computed tomography in establishing the therapeutic management of lung cancer].

Computer tomography (CT), a noninvasive method of high complexity, compelled recognition in pneumological practice, becoming indispensable. The present study deals with 59 patients suspected of bronchopulmonary cancer, who were submitted to computer tomography, besides other examinations. CT contribution in establishing the diagnosis, the therapeutical line (particularly in determining the surgery moment) as well as the correlations (concordance and discordance) between the information offered by CT and by other conventional (radiological, bronchological) methods are analysed. The association of thin needle trans-thoracic biopsy (TNB) to CT ensures an important histologic confirmation which amounts to 75% of investigated cases, according to our statistical data.

[Mediastinal-pulmonary lymphangioleiomyomatosis. The anatomicoclinical aspects].

The lymphangioleiomyomatosis is a rare disease which has a severe course and prognosis and occurs exclusively in females. The symptomatology is not characteristic and the radiological picture is that of a reticular and micronodular diffuse interstitial process. The morphological examination is the only which elucidates the diagnosis. Two cases are reported, the diagnosis of which was established postmortem. In the first case, the site was predominantly pulmonary; in the second case, in addition to the lung, the mediastinal lymph system was also strongly damaged. The morphological changes are characterized by an abnormal proliferation of muscular tissue at the level of the pulmonary interstitium and of the pulmonary and mediastinal lymphatics. The process develops towards destructive lesions of the bronchiolo-alveolar and vascular structures. The pathogenesis is unknown, but genetic and endocrine factors seem to be involved.

[The advantages and limits of computed tomography (compared with conventional methods) in the diagnosis of pulmonary adenocarcinoma (a case report)].

The authors analyse the advantages offered by the computerized tomography in comparison with the conventional methods of diagnosis, the study and classification (TNM) of a case of pulmonary adenocarcinoma with multiple metastases: renal, suprarenal, hepatic, peritoneal, bony, lymph node. At the same time there are also pointed out the limits of this new method.

Behaviour of uricemia in hyperlipoproteinemic subjects.

When compared to values obtained in 40 normalweight normolipemic healthy control subjects, serum uric acid levels were found to be significantly increased in 68 subjects with type II-b and especially in 68 subjects with type IV hyperlipoproteinemia. A much lesser increase of uricemia occurred in 22 patients with type II-a. A significant positive correlation between uricemia and serum triglyceride level could be demonstrated but no such correlation was found with serum cholesterol. Overweight and hypertensive hyperlipemic subjects were presenting higher levels of serum uric acid than normalweight normotensive ones. On the other hand, clinical atherosclerosis and diabetes mellitus had no additive effects upon uricemia in hyperlipoproteinemic patients. It is suggested that hyperinsulinism and accelerated turnover of lipoproteins, often encountered in obese and hypertriglyceridemic subjects, might lead to an enhanced synthesis of purines and subsequently increased production of uric acid.

[The etiological profile of the pulmonary manifestations in AIDS patients (studies of 65 cases recorded in Romania up to 1 March 1993)].

HIV infection and its final form as AIDS were ignored by the public opinion in Romania till 1990. Nowadays it is real fact in our country. Out of total number of 2235 AIDS cases declared till March 1st 1993 by the Ministry of Health only 134 were adult persons. The study concerns 65 cases of AIDS in adults in order to find out the lung symptoms proportion as etiologic features, to settle the ways of transmission of HIV infection and to warn the romanian pneumologists on this problem. The study shows that in 56.9% of total cases, lung symptoms were noted and in 49.2% the lung disease revealed the AIDS. Out of the total number of 65 cases, 24.6% showed tuberculosis as a first disease, 12.4% lung pneumocystosis, 7.76 Kaposi syndrome with lung expression and 4.5% other pneumopathies.
