Malignant mesodermal (mesenchymal) tumors of bladder.

Malignant mesodermal tumors originating in the bladder are rare. Only a few cases could be found in large series of autopsies or in gathered series of primary retroperitoneal space tumors. Sarcomas originating in the bladder wall grow rapidly and usually infiltrate into neighboring tissues. Two cass of such rare tumors are presented. The polar discrepancy between the clinical situation and the histopathologic findings are emphasized. Therapeutic outlines for these rare cases are suggested.

Ectopic origin of main renal artery.

Two cases with an unusually high origin of a single main left renal artery, angiographically investigated, are presented. In the first case the artery originates as the most cephalad branch of the abdominal aorta above the celiac artery. In the second case its origin is from the celiac axis. Embryologic as well as some practical aspects are mentioned.

Renal sinus lipomatosis: differentiation from space-occupying lesion with aid of computed tomography.

Forty-two cases of renal sinus lipomatosis were diagnosed by intravenous urography and nephrotomography. The differential diagnosis from a malignant process necessitated selective renal angiography in 9 cases. With the aid of computed tomography (CT), an invasive technique such as renal angiography appears to be unnecessary in the differential diagnosis of renal sinus lipomatosis. CT seems to be more accurate since it revealed a concomitant cyst in 2 of 4 cases.

Total uterine prolapse causing hydroureteronephrosis.

Six women with advanced uterine prolapse underwent examination by drip infusion urography. A new hypothesis is proposed for the pathogenesis of bilateral hydroureteronephrosis: the ureters are entrapped by the hiatus genitalis, not against the bladder but against the fundus of the fully prolapsed uterus. Drip infusion urography should include an exposure area of about 15 centimeters below the pubis, and at least one exposure should be taken with the patient in the standing position. Thus, the hourglass appearance of the prolapsed bladder will not be overlooked and would be helpful in explaining the cause of the hydroureteronephrosis.

Treatment of benign prostatic hyperplasia with hydroxyprogesterone-caproate: placebo-controlled study.

A placebo-controlled study with progesterone compound, 17-alpha-hydroxyprogesterone 17-n-caproate (Primostat), in 39 patients with benign enlargement of the prostate is reported. Statistical analysis of the results showed no evidence of significant improvement in patients receiving hydroxyprogesterone-caproate. No evidence of an effect as compared with the placebo was found when the residual urine, prostatic size, and histologic and ultrastructural changes of the removed prostatic gland in 6 of the patients, and in the luteinizing hormone, follicle-stimulating hormone, and estrogen urine levels in 21 patients were examined. Subjective effects, when carefully analyzed, provided some beneficial evidence, however not substantiated, when the patients' mode of voiding was carefully watched. The reported beneficial subjective improvement might be attributed to the enhancement of the beta-adrenergic response by the progesterone compound of the adrenergic receptors in the posterior urethra and bladder, presumably causing relaxation of its smooth muscle. The problems associated with the choice and measurement of parameters to be used in this type of investigation are discussed, and the absolute necessity of proper controls, statistical analysis, and close follow-up of the patients is pointed out.

The use of the CO2 laser for partial nephrectomy: an experimental study.

The CO2- laser knife was used for partial nephrectomy in an experimental study of 20 mongrel cats. Of these, 16 survived, two died of peritonitis, and two died of unrelated causes. The inherent characteristics of precision and hemostasis of this modality, already found useful in controlling complications in other fields of surgery, proved to be a safe and effective method for partial nephrectomy as shown by angiography and histopathologic examinations of the operated kidneys at 0, 7, 14, 21, and 90 days after surgery. Complications such as hemorrhage or urinary fistula were nonexistent.

Factors influencing sexual activity after prostatectomy: a prospective study.

Between 16 and 30 per cent of all prostatectomy patients become impotent after an operation for benign prostatic hyperplasia. Since the surgical technique does not seem to be the factor responsible for such a serious problem, more accentuated by the fact that this operation is becoming increasingly frequent with the increase in life expectancy, an assessment of 15 patients before and after prostatectomy is presented. With a statistical analysis of a structured interview (including a mini-Minnesota Multiphasic Personality Inventory test before and after the operation) 3 main differentiating factors emerged between the potent and the impotent group: 1) the level of anxiety exhibited by the patient, 2) whether the patients received an explanation about the surgery and its outcome prior to the operation and 3) the patient's general satisfaction with life.
