[Goodpasture syndrome: role of an epidemiological factor? Apropos of two cases].

Goodpasture's syndrome is a rare pneumorenal syndrome. Although the antigenic target of this auto-immune disorder is now known, its etiology remains debated. We report two cases of Goodpasture's syndrome occurring in similar epidemiologic conditions concerning the moment the disease began, the age and sex of the patients, their place of residence and work and manipulation of chemicals. Thus, a common environmental factor could have been the trigger event of the Goodpasture's syndrome. The epidemiologic features of this disease are reviewed.

[Supernumerary testis and homolateral varicocele. Apropos of a case, study of spermatogenesis and review of the literature].

Polyorchidism is a rare abnormality. A case is reported of a small scrotal supernumerary testis associated with a large varicocele. A torsion of this testis probably occurred and the vascular pedicle seemed atrophic. Despite abnormal fertility with oligozoospermy the accessary testis was removed. A review of relevant literature is made giving details of embryology of this anomaly.

[Multiple wounds caused by a 5.56 mm bullet: the Famas, a competitive weapon].

A case is reported of multiple wounds to abdomen and spine from a 5.56 mm bullet. [Congenital esophagobronchial fistula in the adult].

The authors report the case of a 60-year-old man presenting with an esophagobronchial fistula. The congenital origin of the lesion was demonstrated by the history of the clinical symptomatology and by radiological, endoscopic and anatomical arguments. While it is a rare complaint in adults, it is worth considering in the diagnosis of a chronic cough, because it can be cured surgically without any problems.

Bronchial Fistula↗

[Urinary tract infection and surgical disorders of the lower urinary tract].

Infection is a constant concern during perioperative period. Bacteremia occur during intervention and immediate postoperative period. They are the risk of acute complications, mainly septic shock with a high mortality rate. Escherichia coli is the germ most often found, then Enterococcus and other gram-negative germs. Diagnosis of an urinary infection is made difficult because the presence of a vesical catheter; so, 10(3) bacteria per ml. have to be considered as pathological. Two therapeutic behaviours can be considered in practice: suitable antibiotherapy in obvious infections, but sterilization of urines must not delay surgical intervention, short-time peri-operative prophylaxis cephalosporins of the 2nd or 3rd generation are most often utilized. Overseas, trimethoprim associated with sulfamides can be administrated per os every 2 hours before surgical intervention, at the time of anesthesia for example.
