Compensation of strong thermal lensing in high-optical-power cavities.

In an experiment to simulate the conditions in high optical power advanced gravitational wave detectors, we show for the first time that the time evolution of strong thermal lenses follows the predicted infinite sum of exponentials (approximated by a double exponential), and that such lenses can be compensated using an intracavity compensation plate heated on its cylindrical surface. We show that high finesse approximately 1400 can be achieved in cavities with internal compensation plates, and that mode matching can be maintained. The experiment achieves a wave front distortion similar to that expected for the input test mass substrate in the Advanced Laser Interferometer Gravitational Wave Observatory, and shows that thermal compensation schemes are viable. It is also shown that the measurements allow a direct measurement of substrate optical absorption in the test mass and the compensation plate.

The treatment of hereditary multiple exostosis of the upper extremity.

Thirty of 50 patients with hereditary multiple exostosis developed significant deformities of the arm in one extremity. The degree of deformity is dependent on the location of the osteochondroma. If the osteochondroma is on the radius, deformity will usually be only minimal. If the osteochondroma is at the distal end of the ulna, the epiphysis usually stops growing. We believe that the ulnar collateral ligament then acts as a tether, very similar to that seen in the ulnar clubhand. The radius then has to either bow or dislocate at the elbow, and the wrist displaces ulnarly. We performed operations on 10 patients that consisted of cutting the ulnar collateral ligament, lengthening the ulna, osteotomizing the radius, and removing any osteochondromas. In the young, growing child, staples are placed across the lateral side of the distal radial epiphysis. The cosmetic results of the surgery were very gratifying. Nine patients also had osteochondromas removed from the hands and the forearms.

The effects of topical lidocaine on infant response to circumcision.

The effects of a topical anesthetic used prior to newborn circumcision on the parameters of heart rate, respiratory rate, oxygen saturation levels, seconds of crying time, and overt generalized reaction to the procedure were investigated. In the double-blind design, an experimental group (n = 20) received 4% lidocaine cream mixed with 500 mg acid mantle cream two hours before circumcision, and a control group (n = 24) received 500 mg acid mantle cream two hours before circumcision. The circumcision was divided into five events with parameters recorded 30 seconds into each event. The results showed a significant difference in the heart rate between the treated and untreated group, but the respiratory rate and oxygen saturation levels did not differ significantly. Seconds of crying time also differed significantly between groups, as did overt generalized response. There were no untoward reactions. Results showed that topical lidocaine, simple and safe to use, reduced parameters associated with pain and stress.
