[Smoking as a risk factor for dental implants and implant-related surgery].

Cigarette smoking is known to adversely affect wound healing, and thus may jeopardize the success of dental implantation and implant-related oral surgery. The present study is aimed to present the influence of cigarette smoking on the success and survival of dental implantation as well as on the complication and success of implant-related surgery. We conclude that smokers undergoing both implant-related surgical procedures and dental implantation should be encouraged by their dentists, oral and maxillofacial surgeons, or treating physicians to cease smoking, emphasizing that smoking can increase complications and reduce the success rate of these procedures.

Bridging individual, family and community care: a comprehensive treatment program for the chronic mentally ill.

For about 25 years an integrated individual-family-community program for chronic mentally ill patients has been implemented in Israel, mostly in the kibbutz setting. The treatment model is based on a combined professional and paraprofessional therapeutic team that designs a structured community program in the following areas: individual and family therapies, life-skills training, medication management, and enhancing performance in work and social spheres and in physical and leisure-time activities. This study reports on the treatment of 124 psychiatric patients who have been followed-up for at least four years. Fifty percent of these chronically incapacitated patients achieved the goals of the community program and were found to be totally or greatly improved in terms of marked improvement of functioning and quality of life, resocialization, work stability and avoidance of rehospitalization. Fourteen percent of the patients were substantially improved, and 36% did not achieve the desired goal of satisfactory integration into community life. A case study that demonstrates the clinical application of the program is presented and the implications of the findings are discussed with respect to the community mental health service delivery system in Israel.

Does an impacted tooth cause root resorption of the adjacent one?

External root resorption caused by an adjacent impacted tooth was studied radiographically in 199 cases. Resorption of the root had occurred in fifteen instances (7.5 per cent), the cervical region being least affected. The incidence was highest in the 21 to 30-year-old group and mainly involved male patients. Resorption of the periodontium alone was found in sixteen additional patients; of these, five were followed for 1 year after extraction of the impacted tooth. During this period, the damaged periodontium was completely re-established.

Aging of tissues of the roots of nonfunctional human teeth (impacted canines).

In order to examine the aging of dentin and cementum, seventy-two impacted human canines, nonfunctional and not affected by external factors, such as attrition, abrasion, caries, etc., were surgically extracted. Ground sections were prepared and the areas of sclerotic dentin and cementum were measured with the aid of a planimeter and expressed as a percentage of the total root area. It was found that there is a significant increase in sclerotic dentin and apposition of cementum with an increase in age.

Experimental infections with anaerobic bacteria in mice.

Experimental anaerobic infections in mice are reviewed with a description of a model of pure Bacteroides fragilis infection. In this experimental situation, B. fragilis produces large subcutaneous abscesses in the groins of mice that can be diagnosed without autopsy. This infection was treated effectively with clindamycin in doses that produced levels of drug in blood similar to those attainable in humans.

Chemotherapy of an experimental Bacteroides fragilis infection in mice.

The efficacies of five common antimicrobial agents were determined for a pure Bacteroides fragilis infection in mice. Therapy was begun 4 h after bacterial injection and given every 8 h thereafter for 5 days. Blood levels were determined over an 8-h period for each concentration of antibiotic tested. Clindamycin and tetracycline were the most effective in preventing the formation of abscesses. Chloramphenicol, penicillin G, and cephalothin were not effective in protecting the mice from infection.

Arch expansion: an integral part of surgical orthodontics.

Two cases of mandibular prognathism have been presented. In both cases expansion of the dental arches was performed prior to surgery for the achievement of good and stable interarch relationships postoperatively. In the first case maxillary arch was expanded with a rapid-expansion technique; in the second, expansion of the mandibular arch was performed with a modified removable Coffine appliance. Results at 15 months and 2 years after retention showed the expansion to be stable.
