General practice based diabetes clinics. An integration model.

BACKGROUND: Diabetes is a common chronic health problem in general practice. The quality of care for patients with diabetes has been shown to depend on many factors involving the patient and the general practitioner (GP) as well as the care environment. OBJECTIVE: To measure the benefits of general practice based diabetes clinics that integrate all patient care needs in the one place at the one time. METHODS: General practice based diabetes clinics were conducted in the rooms of 23 practices involving 54 GPs. The clinic team consisted of a diabetes educator, a dietitian and the usual GP. The clinic was conducted in accordance with the NSW Clinical Management of Diabetes Guidelines. RESULTS: Follow up after 12 months showed a significant drop in the number of patients with very poorly controlled diabetes (HbA1c > 3% of normal range). Management of co-morbidities (hypertension, hyperlipidaemia, microalbuminuria) improved and complication screening for active/high risk feet and diabetic retinopathy were performed. CONCLUSIONS: This coordinated and integrated approach to the management of diabetes improved glycaemic control of the poorly controlled patient with diabetes and improved GPs' management of complication risk factors.

An epidemiological study of trichostrongylidiasis in dairy cattle grazing irrigated pastures.

Trichostrongylid burdens in 1 to 2-year-old dairy heifers were estimated after they grazed with a dairy herd for up to 12 months. Ostertagia ostertagi, Trichostrongylus axei and Cooperia oncophora represented 79.5, 12.7 and 7.8% respectively of all trichostrongylids recovered. The largest burden was 97,000 trichostrongyles with less than 20,000 being the typical burden. Clinical disease was not observed. The strain of O. ostertagi present was capable of inhibition and was not necessarily controlled by anthelmintics. Availability of infective larvae increased with the onset of spring when "modified tracer" calves accumulated up to 30,000 trichostrongyles over intervals of one month. From July to September each year, up to 80% of the Ostertagia burden in these animals were inhibited larvae. Faecal strongyloid egg counts fell from less than an average of 60 eggs per gram to 10 eggs per gram when the heifers were one and 2 years old, respectively. Mature cows continually passed few eggs in their faeces.

The effect of temperature on times to hatching of eggs of the nematode Ostertagia circumcincta.

A delayed gamma distribution satisfactorily described the distribution of times to hatching of Ostertagia circumcincta eggs incubated in 0.1% saline at temperatures between 6 and 20 degrees C. Below 6 degree C hatching of eggs was extremely variable. The relationship between rates of development and temperature within the range 10 to 20 degrees C was more closely described by a non-linear function than a linear one. The non-linear function was incorporated into a temperature summation equation which satisfactorily predicted the hatching of eggs incubated under conditions of alternating temperatures.
