Genomic structure of the EPHA1 receptor tyrosine kinase gene.

Some receptor tyrosine kinase genes are mutated in inherited and somatically acquired human cancers. To permit mutational analysis, the complete genomic structure of the human EPHA1 gene on chromosome 7q34 was determined and oligonucleotide pairs were designed to amplify coding regions. The gene contains 18 exons, two more than the related tyrosine kinase, EPHB2. Presumed sequencing errors in the published cDNA sequence of EPHA1 were identified in exons 10 and 11. Availability of this information will facilitate mutational analysis of EPHA1.

Radionuclide scanning for the rapid diagnosis of acute cholecystitis.

Radionuclide scanning with Technetium 99-m HIDA was performed in eighty-nine patients with suspected acute cholecystitis. In twenty-three patients (25.8%), the study demonstrated evidence of cystic duct obstruction and non-visualisation of the gallbladder. Almost all of these patients subsequently were shown to have acute cholecystitis at surgery. The sensitivity and specificity of the procedure for the diagnosis of acute cholecystitis was 95.8% and 100% respectively. The use of radionuclide cholescintigraphy in the assessment of hepatobiliary disorders is reviewed, and it is suggested that it is the test of choice at present for diagnosis of acute cholecystitis.

Scintigraphic detection of segmental bile-duct obstruction.

In a patient with acute obstructive jaundice, cholescintigraphy with technetium-99m-labeled iminodiacetic acid (HIDA) showed uniformly reduced uptake in the left lobe of the liver. Endoscopic retrograde cholangiopancreatography (ERCP) demonstrated cholelithiasis and obstruction of the distal hepatic duct. Surgery, and later a T-tube cholangiogram, confirmed the presence of numerous stones in the left intrahepatic and common hepatic ducts. The liver was free of tumor. Intrahepatic segmental ductal obstruction may produce a spectrum of patterns on hepatobiliary imaging ranging from reduced uptake to intrahepatic pooling.
