[Chronic septic granulomatosis. Disclosed by hepatic abscesses. Contribution of echography].

Skin infections and lymph node abscesses are the most common manifestations of chronic granulomatous disease (CGD) during the first year of life. Intra-hepatic abscesses occur later on. The case reported concerns a 4 month-old infant, in whom the disease presented initially with liver abscesses. Percutaneous needle aspiration of the liver under ultrasonographic control permitted the isolation of Staphylococcus aureus and allowed partial regression of the abscesses. Surgery was not required.

[Bronchiolitis obliterans: technics of exploration and diagnosis].

Bronchiolitis obliterans is an anatomo-clinical entity which may, in young pediatric patients, follow severe viral infections, especially due to measles virus and adenoviruses. After a transient remission, the disease progresses to chronic respiratory failure with dyspneic paroxysms. Diagnosis is based on different investigations which confirm the absence of proximal bronchiolar obstruction and the presence of bronchiolar obliteration. Deficient pulmonary perfusion due to ventilatory defects are demonstrated by pulmonary scintigraphy and even better by digital subtraction pulmonary angiography. Obliteration of non-cartilaginous bronchioli by granulation tissue produces bronchiolar destruction and explains the permenence of alterations. Dissimilarity of the responses of children to acute respiratory viral infection remains unexplained. The degree of the viral load and of the initial immune deficiency as well as the persistence of the virus within the pulmonary tissue probably account for the chronic pulmonary damage.

[Ultrasound and portal hypertension in children. Demonstration of gastro-oesophageal collaterals. Preliminary report (author's transl)].

The authors studied the value of ultrasonography in demonstrating oesophageal collaterals in portal hypertension in childhood. 22 children with portal hypertension underwent successively an ultrasound examination and an endoscopy looking for submucosal oesophageal varices. Oesophageal collaterals were said to be present when ultrasound showed vascular structures in the gastro-hepatic ligament above the coeliac trunk origin. Among those 22 patient 18 had a satisfactory ultrasound examination. 12 true positive, 3 true negative, 3 false negative ultrasound examinations were obtained. 4 ultrasound studies were inconclusive. The value of ultrasound in the pre and post surgical check-up of portal hypertension in children is emphasized.

Transcatheter arterial chemotherapy using doxorubicin, iodized oil and Gelfoam embolization in hepatocellular carcinoma.

Computed tomography (CT) was performed in 54 patients with hepatocellular carcinoma three weeks after transcatheter arterial chemotherapy using iodized oil and doxorubicin with or without gelfoam embolization. Patients with iodized oil retention in the tumor greater than 50 per cent of tumor size survived longer than patients with retention of less than 50 per cent. Differences were also found within Okuda stages I and II, but they were significant only in Okuda stage I (p less than 0.0001). These results suggest a possible relationship between iodized oil retention and survival. In addition to Okuda stage, several factors affected iodized oil retention: tumor vascularity, tumor size, portal thrombosis and Gelfoam embolization. These factors may thus influence the prognosis after transcatheter arterial chemotherapy.

[Hemodynamic effects of dobutamine in asphyxiated newborn infants].

The hemodynamic effects of dobutamine were studied by non invasive techniques in 6 full term neonates with severe perinatal asphyxia. At 10 micrograms/kg/min, dobutamine significantly increased the cardiac output, the heart rate and the aortic blood flow velocity. The mean arterial pressure increased but not significantly and the stroke volume remained unchanged. These changes seem to be dose dependent. The increase of cardiac output may be due to a chronotropic and inotropic effect of dobutamine. Dobutamine seems to be an effective agent for the treatment of low cardiac output in asphyxiated neonates.

[Angiodysplasia or vascular malformations of the cervical spine (author's transl)].

Following exploration by angiography , 45 patients with cervical spine malformations ( out of 220 cervicocephalic examinations for vascular malformations), were treated by embolization. The 45 cases included 13 facial lesions, 4 pharyngolaryngeal lesions, 2 cervical venous ectasias, 4 arteriovenous fistulae of the vertebral artery, 19 medullary lesions, and 3 pseudomalformations (hemangiopericytomas). Though embolization is difficult it is of value in definitely vascular occlusions (follow-up for 3 years), and miniaturization of catheters and balloon sounds enables very selective occlusion of vascular tumors or malformations to be carried out, micro-emboli of 30 to 500 mu have been obtained.

[Involvement of the common bile duct in Hodgkin's disease : report of three cases (author's transl)].

The authors report 3 cases of extra-hepatic obstruction of the common bile duct in Hodgkin's disease, in one case due to lymph nodes compressing the hepatic pedicle, in the second case due to infiltration of the common bile duct alone, in the third case by a large Hodgkin tumour of the head of the pancreas. Although emphasizing the rareness of these cases of jaundice in Hodgkin's disease, they emphasize the necessity of exploring these patients in order to make a precise assessment of the lesions and thus choose the best treatment.
