Capsule endoscopy: improving transit time and image view.

AIM: To evaluate the effect of various methods of small bowel preparation on the transit time and the quality of visualization of the entire small bowel mucosa. METHODS: Ninety-five patients underwent capsule endoscopy (CE) by easily swallowing the capsule. They were divided into three study groups according to the preparation used: group A (n = 26) by polyethylene glycol (PEG) liter or with sodium phosphate (SP) 12 h prior to the CE study; group B (n = 29) by erythromycin 1 h prior to the CE study; and group C (n = 40) without any preparation. Visualization ranged from good to satisfactory to poor. RESULTS: The gastric emptying time in the group prepared with erythromycin was shorter but without statistical significance and the small bowel transit time was unaffected. In elderly subjects prepared by PEG or SP, the gastric emptying time was significantly longer (163.7 min, P = 0.05). The transit times of the three sub-groups were not affected by gender or pathology. The grade of cleaning of the entire study group was 3.27+/-1.1. The erythromycin group presented significantly the worst quality of images (P = 0.05) compared to the other sub-groups. Age, gender, and pathology had no effect on the quality of the cleaning of the small bowel in the sub-groups. One (1.05%) case had no natural excretion. CONCLUSION: Erythromycin markedly reduces gastric emptying time, but has a negative effect on the quality of the image in the small bowel. The preparation of elderly subjects with PEG or SP has a negative effect on the small bowel transit time.

The fundamental role of increased production of nitric oxide in lipopolysaccharide-induced embryonic resorption in mice.

Nitric oxide (NO) fulfils important functions during pregnancy and has a role in implantation, decidualization, vasodilatation and myometrial relaxation. However, at high concentrations, such as those that are produced in sepsis, NO has toxic effects as it is a free radical. The aim of this study was to characterize uterine and decidual NO production in lipopolysaccharide (LPS)-induced embryonic resorption in mice and to determine which isoforms of nitric oxide synthase (NOS) take part. LPS produced 100% embryonic resorption at 24 h, with complete fetus expulsions at 48 h. Decidual and uterine NO production were increased by LPS, with maximum production at 6 h. This increase was due to the induction of expression of inducible nitric oxide synthase (iNOS) isoform in the decidua and uterus, and neuronal nitric oxide synthase (nNOS) isoform in the decidua, as detected by western blot analysis and immunohistochemistry. LPS increased iNOS expression in decidual and myometrial cells and increased nNOS expression in decidual cells. In addition, LPS caused fibrinolysis and infiltration of mesometrial decidua by macrophages positive for iNOS and CD14 (LPS receptor). Endothelial nitric oxide synthase (eNOS) was found in decidual and uterine arteries but LPS did not modify its expression. LPS induced CD14 expression in endometrial glands, and this could have amplified the inflammatory response. Aminoguanidine, an inhibitor of iNOS activity, totally reversed the LPS-induced embryonic resorption. This result could be explained by an inhibition of the increase in NO production but also by an inhibition of the cellular infiltration and fibrinolysis. These results show that NO fulfils a fundamental role in LPS-induced embryonic resorption.

Regulation of metalloproteinases by nitric oxide in human trophoblast cells in culture.

The process of embryo implantation requires extensive remodelling of the endometrial extracellular matrix, a function largely performed by matrix-degrading metalloproteinases (MMPs). In the present study, we used trophoblast cells isolated from human term placentas to study the regulation of MMPs by nitric oxide (NO). Using a combination of zymography, Western blot and indirect immunofluorescence, we showed that MMP-2 and MMP-9 are increased during the conversion from low-motile cytotrophoblast cells to the highly motile and differentiated syncytiotrophoblast multinucleated cells. We also observed an increase in NO production and NO synthase (NOS) expression during this cellular differentiation process. In addition, we demonstrated a positive regulatory role of NO on the activity and protein expression of MMP-2 and MMP-9, because NO donors (NOC-18 and spermine-NONOate) or the NOS substrate (L-arginine) stimulate, whereas NOS inhibitors (N(G)-nitro-L-arginine methyl ester and N(G)-monomethyl-L-arginine) reduce the expression and gelatinolytic activity of MMP-2 and MMP-9 in isolated trophoblast cells. Taken together, these results suggest that, in differentiating trophoblasts, NO regulates the induction of matrix-degrading proteases required for invasion during embryo implantation.

Achyrocline satureioides essential oils from southern Brazil and Uruguay.

The essential oils of the aerial parts of Achyrocline satureioides (D.C.) Lam., a regional medicinal plant, from different collection locations in the South of Brazil and Uruguay were examined by GC and GC-MS. Monoterpene and sesquiterpene hydrocarbons were the main fractions of both the oils from Brazil and Uruguay. The oxygenated monoterpenes and sequiterpenes were at much lower percentage in both samples. The Uruguayan samples have 1,8-cineole which is not present in the Brazilian samples and has not been reported in other samples from Brazil. The results indicated a high biodiversity of the native populations of A. satureioides.

Atrial natriuretic factor (ANF)-like immunoreactivity and secretory granules in internal gills tadpoles of South American toad Bufo arenarum (Hensel).

In this study, an ultrastructural and immunohistochemical analysis was carried out on the presence of Atrial Natriuretic Factor-like substance in the internal gills of premetamorphic larval stages of the South American toad. The results of immunohistochemical study indicate that, at larval 27 and following stages, ANF-like immunoreactive cells are seen in internal gills. At electron-microscopy level "secretory granules" similar to those ANF-immunoreactives granules of myoendocrine cells of the same species are seen only in clear cells of gill rakers. Our results reveal the occurrence of ANF-like substance in Bufo arenarum gills and may suggest that "cardiac hormones" might play a paracrine and/or autocrine role in the water and electrolyte balance at internal gills level.

Demonstration of atrial natriuretic peptide immunoreactivity and ultrastructurally related secretory granules in myoblasts of the chick myotome.

In this study, an ultrastructural and immunohistochemical examination was carried out on the presence of an 'atrial-natriuretic-peptide (ANP)-like factor' in the myoblasts of the brachial myotome of chicken embryos at Hamburger and Hamilton stages 17 +/- to 19. The results of the immunohistochemical study indicate that, at these stages of their development, the myoblasts of the brachial myotome show ANP immunoreactivity. The ultrastructural analysis of the myoblasts shows the presence of 'granules' or 'secretory vesicles', similar to the secretory granules which contain natriuretic peptides. The demonstration of immunoreactivity to ANP in the myotome and the specific localization of secretory vesicles in myoblasts, as the first evidence relating ANP with skeletal muscle development, reveal a new site at which an ANP-like factor may be synthesized. The hypothesis of a paracrine and/or autocrine function(s) during myogenesis is suggested.

The relationship between age and cognitive impairment in HIV-1 infection: findings from the Multicenter AIDS Cohort Study and a clinical cohort.

Previous studies have identified age as a risk factor for many neurologic disorders, and a "cerebral reserve" factor has been postulated to explain these findings. This study examined whether age represents a risk factor for HIV-1-related neuropsychological dysfunction. Subjects for study 1 were primarily asymptomatic seropositive (n = 1,066) and seronegative (n = 1,004) nonelderly male community volunteers who completed neuropsychological and reaction time measures. Data analyses revealed a significant effect for age on reaction time and timed neuropsychological measures, but no interaction between age and serostatus. Study 2, employing a similar neuropsychological battery, consisted of 76 seropositive men (29 over age 55) recruited from community outpatient clinics and 47 seronegative controls. We found serostatus and age to have main effects on a number of measures, but a trend for an effect of age-serostatus interaction on only one measure.

Calcium-antagonist effect of quercetin and its relation with the spasmolytic properties of Psidium guajava L.

The antidiarrheal properties of water and methanolic extracts of Psidium guajava leaves have been demonstrated with anteriority and their spasmolytic effect was attributed to quercetin, a flavonoid contained in this plant. The present paper reports the intestinal smooth muscle relaxation produced by quercetin on isolated guinea pig ileum previously contracted by a depolarizing KCl solution. Quercetin also inhibited intestinal contraction induced by different concentrations of calcium, shifting the contraction curve to the right showing a clear calcium-antagonistic effect. Quercetin effect on ileal and aortic smooth muscles are compared, the ileum being more sensitive to this flavonoid. The calcium-antagonist property of quercetin is discussed and also its contribution to explain the spasmolytic effect of this popular herbal remedy.

Differentiation of endocrine myocardiocytes in the developing heart of the toad (Bufo arenarum Hensel).

The differentiation of endocrine myocardiocytes was investigated in the heart of developing toad Bufo arenarum Hensel, combining ultrastructural and immunocytochemical procedures. The distribution of immuno-reactive atrial natriuretic peptide (ANP) in the whole heart was appraised by light microscopy, applying biotin-streptavidin and immunofluorescence techniques. With the latter procedures ANP was first recognized at embryonic stage 22, in both atrium and ventricle. In the ensuing stages the ANP-reactivity became stronger in the atrium, while it became dimmer in the ventricle. At the end of the larval prometamorphic stage, atrial myocardiocytes acquired almost all the features of adult myoendocrine cells. At electron microscope level, small inclusions, about 110-120 nm in diameter, resembling secretory granules were found in myoendocrine cells beginning at embryonic stage 22. However, no immunogold labeling of ANP occurred until stage 25. The number of secretory granules diminished in the ventricles and increased in the atrium of the larval heart and at the end of the prometamorphic stage the atrial myoendocrine cells presented the ultrastructural characteristics of active secretory cells. The synthesis of ANP in larvae is enhanced at a critical period of development when the developing toad switches from an aquatic environment to terrestrial life. The cardiac hormones seem to play a key role in the regulation of the osmolarity of body fluids at this developmental stage.

Metacognition in HIV-1 seropositive asymptomatic individuals: self-ratings versus objective neuropsychological performance. Multicenter AIDS Cohort Study (MACS).

This study examined the relationship between actual and self-reported neuropsychological deficits, depression, and HIV-1 serostatus. The subjects, who consisted of 479 individuals, 256 who were HIV seronegative (SN) and 233 who were HIV-1 seropositive though still asymptomatic (ASP), were administered a standardized neuropsychological screening battery consisting of measures of attention, motor speed, psychomotor speed, verbal memory, verbal fluency, and depression. To assess subjects' subjective sense of their cognitive status, the Cognitive Failures Questionnaire (CFQ), a 25-item self-report questionnaire, was also administered. The results of MANOVA failed to reveal group differences between the SN and ASP groups on the measures of neuropsychological function. Similarly, the ASP and SN groups did not differ on the number or severity of reported cognitive failures. However, a positive correlation was found between CFQ scores and level of depression. These results do not support the hypothesis that ASP individuals are aware of cognitive decline prior to detection using standard neuropsychologic screening instruments. The data do suggest that the presence of depressed mood, independent of serostatus or actual neuropsychological impairment, is associated with increased cognitive complaints.

Numerical model and experimental investigation of blood flow through a bifurcation. Interaction between an artery and a small prosthesis.

The use of prosthetic vascular grafts as bypass to arteries of small diameter (< 5 mm) often is thwarted by occlusion after months of apparently normal function. This work presents a numeric simulation of the flow through a distal anastomosis. A finite element method has been applied, providing information on the velocity field, streamlines, wall shear stresses, and vorticity in the different geometries. The study illustrates the dependence of the flow pattern and wall shear stress distribution upon Reynolds number, and the results show high shear stresses, approximately 3-5 times larger than normal, acting on the vessel wall at the branching point for the small diameter branch.
