Cytogenetic biomonitoring of an Italian population exposed to pesticides: chromosome aberration and sister-chromatid exchange analysis in peripheral blood lymphocytes.

Chromosome aberrations (CA) and sister-chromatid exchanges (SCE) were measured in lymphocytes of (A) 32 healthy individuals working in the flower industry and exposed to pesticides, (B) 32 individuals exposed as above and hospitalized for bladder cancer, and (C) 31 controls. Compounds to which floriculturists were exposed included 18 nitro-organic herbicides and fungicides, 9 nitro-organic fungicides, 12 organophosphate and organothiophosphate insecticides, 4 hydrocarbon derivative herbicides and 5 inorganic fungicides and insecticides. 150 and 70 metaphases per individual were scored for CA and SCE, respectively. A significant increase in the incidence of CA and SCE was observed in both exposed groups. Cancer patients showed the presence of rare rearrangements (dicentrics, rings and quadriradials) that were not observed in controls and were present at a lower frequency in healthy exposed people. Hyperdiploid and polyploid metaphases were also significantly increased in the 2 exposed groups compared to controls. Stratifying for age or smoking habits, although affecting the significance of individual data, did not change the substance of the results.

Results of radical nephrectomy with extensive lymphadenectomy for renal cell carcinoma.

Radical nephrectomy with extensive lymphadenectomy was performed by the same surgeon on 104 consecutive patients with renal cell carcinoma. No significant differences in over-all survival owing to age, sex and preoperative embolization were noted. Only extension of the disease proved to be important as a prognostic factor. In the light of the statistical analysis of this variable the prognosis for patients with renal cell carcinoma has been classified as 1) good--patients with tumors confined to the kidney, 2) intermediate--patients with perirenal fat involvement, lymph node metastases and venous extension, and 3) poor--patients with distant metastases.

Cytogenetic analysis of transitional cell carcinoma of the bladder. Clinical implications.

Cytogenetic analysis of resected bladder tumors was performed in 30 patients. None of these patients had previous irradiation or chemotherapy. Direct chromosome preparations were made. Of the 30 preparations, 20 had chromosome abnormalities. We have observed a good correlation between the chromosome abnormalities and the stage/grade of the tumors. Patients were followed from 3 to 23 months. During this period, 61% of the patients with noninvasive or submucosal invasive bladder tumors and chromosome abnormalities have had recurrences. The existence of a good correlation between chromosomal abnormalities and the capacity of the neoplasm to recur was confirmed.

[Surgical treatment of renal adenocarcinoma with neoplastic thrombosis of the inferior vena cava].

Personal experience in the surgical management of 15 cases of renal adenocarcinoma with neoplastic thrombosis of the vena cava inferior is described, and an account is given of the procedures adopted in accordance with the degree of endocavitary extension of the tumour. Emphasis is also placed on the need to ensure, as in all cases of nephrectomy for tumours, "neoplastic asepsis" by means of a preliminary vascular stage with the compartment intact. For this purpose, it is felt that sufficient preoperative angiographic evaluation of the intracavitary tumour is essential, coupled with absolute intraoperative control of the vena cava and/or right atrium proximally to the thrombus, to prevent the intraoperative mobilisation of neoplastic embolisms.

Prolactin secretion in benign prostatic hypertrophy.

Prolactin secretion was evaluated in 22 patients with benign prostatic hypertrophy, before and after thyrotropin-releasing hormone stimulation. Increased prolactin reserve was found. In 11 patients, in whom the test was performed twice, the prolactin response after surgery was higher than in the pre-surgical test. The possible role of prolactin in the genesis of benign prostatic hypertrophy is discussed.

[Renal adenocarcinoma with neoplastic thrombus in the inferior vena cava. Prognosis and operative risk].

10 cases of renal adenocarcinoma invading the lower vena cava and subjected at the Genoa urology clinic to radical nephrectomy and cavotomy, with thrombectomy or resection of the vena cava, are reviewed. Of 5 patients who did not present long-term metastasis, 2 are alive and in good condition at 30 and 64 months, one died from myocardial infarction after 34 months, and two died in the postoperative course from respectively renal insufficiency and myocardial infarction. In patients presenting long-term metastasis at the moment of surgery, results were poor: four patients died from 4 to 7 months later and only one patient, with bone metastasis, is alive after 33 months.

Treatment of advanced prostatic carcinoma with cyproterone acetate and orchiectomy--5-year follow-up.

We report the results from 106 patients with extracapsular (stage C) or widespread (stage D) prostatic cancer, who were treated with cyproterone acetate and orchiectomy and followed for 5 years. As controls we used 40 patients, chosen at random, with stage C and stage D cancer of the prostate gland who were treated with stilbestrol and orchiectomy. The survival rate is improved in patients treated with cyproterone acetate and orchiectomy compared to the patients treated with stilbestrol and orchiectomy. Treatment with cyproterone seems more effective in those patients with low-stage and low-grade prostatic cancer. The side effects of this therapy are less and milder than those described in estrogenic treatment alone or with orchiectomy.

The effect of cyproterone acetate and orchiectomy on metastases from prostatic cancer.

We have studied 38 patients with previously untreated, widespread prostatic cancer, who were submitted to therapy with cyproterone acetate and orchiectomy. 70% of patients with symptoms have shown subjective improvement. Moreover, it was possible to observe the regression, stabilization or progression of metastases in 32, 50 and 18% of the cases, respectively. We conclude that such therapy is effective in the initial treatment of metastatic prostate cancer and can be continued under radiographic control until the appearance of new metastases. At this point radiotherapy and alkylating agents may be effective.

Evaluation of LH, FSH, TSH, Prl, and GH secretion in patients suffering from prostatic neoplasms.

The concentration of luteinizing hormone (LH), of follicle stimulating hormone (FSH), of thyroid stimulating hormone (TSH), of prolactin (Prl) and of growth hormone (GH) was evaluated, in basal conditions and during stimulation tests, in serum of 32 patients with benign prostatic hypertrophy (BPH), of 29 patients suffering from prostatic carcinoma and of a control group of similar age. Significant difference in pituitary Prl secretion was found between the prostatic patients and the control group.

Bivalve nephrolithotomy in kidneys with ileal ureter.

We describe the surgical management of 2 patients with staghorn stones in kidneys previously operated upon and an ileal ureter. Bivalve nephrolithotomy was accomplished in situ using local hypothermia and anatrophic nephrotomy. This approach seemed to be the most suitable in these cases because other procedures could have been difficult and risky for the pelvioileo-anastomosis.

Increased serum prolactin pituitary reserve in patients with prostatic neoplasms.

Prolactin (PRL) pituitary reserve was measured in 8 patients with prostatic adenoma and 11 patients with prostatic carcinoma. The PRL reserve was significantly higher in these patients compared to an age-matched control group of 6 subjects. A possible role for PRL-inhibiting drugs in the management of oestrogen-resistant carcinomas of the prostate is suggested.
