[The video-assisted thoracoscopic treatment of spontaneous pneumothorax--our experience with 42 patients].

We present our experience with video-assisted thoracoscopic surgical (VATS) treatment of spontaneous pneumothorax (SP) in 42 consecutive patients. We performed 45 video-thoracoscopic procedures during 44 hospitalisations. The most common procedures were: Endo-stapling of bullae in 12 pts, 4 ligations of bullae, 8 suturing of bullae, 27 bleb ablations and catheterizations. In all patients a parietal pleural abrasion was performed. We describe a new variation of mechanical pleurodesis for prevention of recurrences--"linear pleurotomy"--performed in 15 patients. No major complication occurred during surgery. The mean operative time was 67 minutes (range 30 to 160 min). We have 2 conversions to thoracotomy in patients with secondary SP. Two early recurrences were treated one by thoracotomy and the other by re-thoracoscopy. One case of postoperative bleeding required re-thoracoscopy. Four patients had a prolonged air-leak (more than 7 days), but recovered without additional treatment. The mean duration of intercostal drainage was 4.64 days, the mean postoperative hospital stay--6.8 days. We report 95.6% success rate after VATS treatment of SP with mean follow-up period of 21.1 months (2-49 months).

[Omentoplasty in surgical management of postpulmonectomy pleural empyema].

A method of treating postpulmonectomy pleural empyema, practically implemented in this country for the first time, is described. Three patients are subjected to operation. Right pulmonectomy for lung cancer and chronic inflammatory process is done twice, and left pleuropulmonectomy for tuberculosis--once. Postpulmonectomy empyema persists in all three cases regardless of the adequate continuous suction drainage and intrapleural antiseptic management. Anaerobic and gram-negative flora is isolated. The size and location of the intrapleural cavity being cured are assayed by CT, thoracic ultrasonography and fistulography. In none of the patients is clinical and FBS evidence of bronchopleural fistula established. The operative procedure consists in resection of a 4 cm segment from the underlying rib in the drainage zone, and further cavity treatment under thorascopic control. Minor median laparotomy and skeletization of the greater omentum are performed preserving a major nutrient vessel depending on the location of the cavity. The omentum is drived into the pleural cavity through a parietal opening of the diaphragm, measuring 3-4 fingerbreadths. Pleural cavity drainage is carried out according to Redon. Two patients run an uneventful postoperative course. One female patient develops pylorospasm successfully cured by spasmolytic therapy and H2 blockers. CT and thoracic echography do not show presence of residual cavities. The patients are free of any complaints over periods ranging from 6 to 16 months postoperatively. The inference is reached that omentoplasty is a new method promoting successful elimination of both postpulmonectomy empyema, and other residual pleural cavities as well, with or without bronchopleural fistula.

[Plastic and reconstructive surgery of the bronchial tree].

Bronchoplastic and reconstructive operations (BPRO) are a major issue in the broad methodological spectrum of thoracic surgery. It is the aim of the study to analyze the indications, operative technique and results of such operations on the basis of experience gained in the Clinic of Thoracic Surgery over a 5-year period. A total of 19 patients (14 men and 5 women) at mean age 50.7 y (range 16 to 70 y) are operated. By histological variant of the tumor operated on, the patients are distributed as follows: carcinoid--4 cases, fibromas--1, squamous cell carcinoma--10, adenocarcinoma--1, bronchoalveolar carcinoma--1, small-cell carcinoma--1 and leiomyosarcoma--one. The reconstructive operations performed include: isolated bronchus resection--2, right upper lobectomy with cuff resection--7, right upper bilobectomy with cuff resection--2, left upper lobectomy with cuff resection--7 (in two instances in conjunction with angioplasty), and left lower lobectomy with cuff resection and angioplasty--one. No intraoperative and perioperative lethality (within 30 days) is recorded. An overweight female patient with diabetes hardly lending itself to compensation develops severe suppuration. In two instances serious concurrent complications necessitate reoperation. Overall postoperative hospital stay--20 days; without the 3 severe complications--12.8 days. One patient dies of brain metastases within 6 months of the intervention. The survivorship term in the remainder varies from 1 year to 4 years 9 months, averaging 31 months. There are no stenoses or granulations of the anastomoses requiring endoscopic treatment. Presumably, BPRO are an adequate therapeutic approach to patients presenting centrally located malignant and benign tumors. The results of their application in the series being examined are estimated as very good.

A new surgical approach to internal genitalia in water buffalo (Bubalus bubalis): a preliminary study.

New surgical access to internal genitalia through either the left or right lateral wall of the pelvic cavity was sought in 3 multiparous buffalo cows. (MurrahxMediterranean buffalo crossbreed). An incision 12 to 13 cm long was made in the dorso-caudal part of the regio glutea, equidistant from the tuber ischiadicum and the most prominent point of the crista sacralis mediana (in the space confined dorsally by the caudal part of os sacrum and first 2 to 3 vertebrae coccygeae and ventrally by the spina ischiadica of os ischii), just at the site where the lateral wall of the pelvic cavity is thinnest. Ovaries, oviducts and uterine horns were successfully exteriorized through this glutea approach in all 3 animals.

Monoclonal antibodies directed to the O antigen of Salmonella serogroup E cross-react with lipopolysaccharides of Salmonella serogroups C, F and S.

Four murine hybridoma clones secreting monoclonal antibodies (mAb) directed against lipopolysaccharide (LPS) antigen of S. newington, serogroup E1, were obtained after a fusion of spleen cells of mice immunized with formaldehyde-killed bacteria and mouse myeloma cells of the X63-Ag8.653 line. Antigen binding properties and specificity of the mAb were studied in bacterial agglutination tests, passive hemolysis and its inhibition, passive hemagglutination tests, passive hemolysis and its inhibition, passive hemagglutination and immunoenzyme tests (ELISA and immunoblotting). Three of the mAb (24E6, 29E1 and 45F6) were agglutinating and were active in all tests used, while mAb 31H12 did not agglutinate bacteria but revealed a high reactivity in the immunoenzyme reactions. It was found that the mAb reacted with LPS and Salmonella strains from serogroup E (E1, E2, E3 and E4) as well as from serogroups C (C1 and C4), F and S thus showing that the O3 antigen possesses more than one epitope, one of which is represented on the LPS antigens of the serovars from the cross-reacting groups mentioned. According to the known chemical the most probable recognized epitope consists of mannose with beta-linkage to the next monosaccharide residue in the LPS chain.

Characterization of bacteriophage P1 library containing inserts of Drosophila DNA of 75-100 kilobase pairs.

A multiple-hit bacteriophage P1 library containing DNA fragments from Drosophila melanogaster in the size range 75-100 kb was created and subjected to a preliminary evaluation for completeness, randomness, fidelity, and clone stability. This P1 library presently contains 3840 individual clones, or approximately two genome equivalents. The library was screened with a small set of unique-sequence test probes, and clones containing the sequences have been recovered. In situ hybridization with salivary gland chromosomes indicates that the clones originate from the site of the probe sequences in the genome, and filter hybridization of restriction digests suggests that the clones are not rearranged in comparison with the genomic sequences. Approximately 1.7% of the clones contain sequences that hybridize with ribosomal DNA. A small subset of these clones was tested for stability by examination of restriction fragments produced after repeated subculturing, and no evidence for instability was found. The P1 cloning system has general utility in molecular genetics and may provide an important intermediate level of resolution in physical mapping of the Drosophila genome.

[Malignant schwannoma of the lung].

A case of a histologically confirmed malignant schwannoma of the lung is reported. The pathological and clinical features, the immuno-histological diagnosis and the prognosis of the tumor are reviewed. Furthermore, the successful surgical treatment of the patient is described; he survived 7 years after the operation, and now there aren't any clinical and roentgenological signs of recurrences.

[The development of remission in patients with bronchial asthma].

The efficacy of the therapeutic programs and rehabilitation measures for achievement of a permanent remission of bronchial asthma was studied in 436 patients, 18 to 71 years of age (mean age 45.3 years), 283 women and 153 men. In 42.89% of the patients the asthma was severe, in 47.07% it was moderate and in 10.09%--mild. 30.05% of the patients were with atopic asthma, 52.06%--with infectious dependence, 8.03%--with glucocorticoid deficiency and 2.98%--with hormonal imbalance. In all patients remission was achieved with a different reliability. Full remission was achieved in 24.08%, partial remission 1 degree--in 58.03% and partial remission II degree--in 17.89% of the patients. The duration of the remission varied from 2 up to 46 months. A remission longer than 6 months was achieved in 72.10% of the patients treated with intal or zaditen, in 61.36% of the patients treated with corticosteroids and only in 27.70% of the patients treated with symptomatic and general hygienic means. The duration and the quality of the remission depend on the correct selection of the therapy in the acute stage, the severity and the form of bronchial asthma. Better results are achieved in the mild form of bronchial asthma. The most frequent causes for destabilization of the remission were bronchopulmonary infections, incorrect reduction or discontinuing of the medication, formal supportive therapy, psychologic demobilization and irrational supportive therapy.

[The prognostic significance of high leukocytosis in acute myocardial infarct].

39 patients with an acute myocardial infarction and high leucocytosis of greater than or equal to 15.0,10(9)/l, were studied in order to find out the relation of the high leucocytosis and the final prognosis of the disease. The high leucocytosis of greater than or equal to 15.0.10(9)/l was found for several up to 10-12 h after the onset of the painful attack in patients with a large, combined and obligatory transmural acute myocardial infarction. Leucocytosis of greater than or equal to 19.5.10(9)/l was found in all patients who died. The high leucocytosis is due to the mobilization of the bone marrow neutrophil reserve. The high leucocytosis of greater than or equal to 15.0.10(9)/l is an important prognostic sign for the final outcome of acute myocardial infarction.

[The clinico-diagnostic and therapeutic problems of patients with bronchial asthma combined with ischemic heart disease].

The combination of bronchial asthma and ischemic heart disease presents new qualitative problems related to the diagnosis and treatment, 476 patients with bronchial asthma above 48 years of age (mean age 56.9 years) were studied. In 294 of these patients (61.7%) ischemic heart disease was found as an accompanying disease. In 31 patients (6.15%) there were signs of past myocardial infarction. The ischemic heart diseases presented with typical clinical features in 33.33% of the patients only. In the remaining 66.67% the disease ran a silent course with-out pain. In the combination bronchial asthma and ischemic heart diseases the diagnostic criteria of the ECG in rest fall to 55%. Many pathogenic factors, facilitating and aggravating, change the clinical characteristic of the combined diseases. The combination leads to new therapeutic problems, since the therapeutic approaches to them are different and sometimes opposite. The active treatment of one of the disease may lead to iatrogenic deterioration of the other.

[Hypokalemic periodic paralysis in thyrotoxicosis].

For the first time in Bulgaria a case of hypokalemic periodic paralysis in thyrotoxicosis is described. Three periods of paralysis were observed and documented in a hospital. The thyrotoxicosis was well manifested with disturbed carbohydrate tolerance and responded favourably to methizole treatment. It is suggested that the mechanism for the development of the hypokalemic periodic paralysis is the intracellular blockade of potassium by the surplus of thyroid hormones.

[Glucocorticosteroids in the therapeutic program of status asthmaticus in patients with bronchial asthma].

The therapeutic efficacy of glucocorticosteroids in asthmatic state was studied in 216 asthmatic patients, 17 to 72 years of age (mean age 46.3 years). 141 patients (65.78%) were in the first stage, 57 patients were in the second stage and 18 patients were in the third stage of asthmatic state. The comparative assessment of the results shows that the glucocorticosteroids in high doses are obligatory in the treatment of asthmatic state independently of its stage. The initial dose is 1000-1200 mg of hydrocortisone and up to the 6-th hour a dose of 3000-3500 mg is reached. If the asthmatic state persists the total dose may reach 4000-7000 mg for 34-48 hours. When the asthmatic state is improved the dose is reduced with 25-50%. The use of high doses of glucocorticosteroids improves the outcome of asthmatic state.

[Bronchial asthma and pulmonogenic arterial hypertension].

The arterial hypertensive syndrome was studied in 216 patients with bronchial asthma, 17 to 67 years of age (mean age 43.6 years), duration of the disease form 1 up to 19 years. To all patients bicycle ergometry with a permanent load of 60 W for 5 min. was applied. The results showed that in 145 patients (67.12%) arterial hypertensive syndrome was present during the bronchopulmonary obstruction. In 105 of these patients (48.6%) the hypertension was pulmonogenic and in 40 patients (18.51%) the arterial hypertension was an accompanying disease. In 62 patients (28.70%) the pulmonogenic hypertension was of a labile type and in 43 patients (19.90%) it was stable. A marked correlation was found between the values of the increased arterial pressure and the indices of bronchopulmonary obstruction--forced expiratory volume/min, Tiffneau's index, maximal expiratory flow, mean maximal expiratory flow. Hypotensive treatment is recommended in patients with stable pulmonogenic hypertension and with accompanying arterial hypertension only.

[Variant (forme fruste) forms of bronchial asthma].

The variant forms of bronchial asthma were studied. It was established that in 66.4% of the patients examined the bronchial obstructive syndrome was caused by bronchial asthma. In 50.30% of the patients bronchial asthma had an atypical course--variant (forme fruste) forms. Three types of variant forms of bronchial asthma are differentiated: allergic tracheobronchitis (47.23%), the so called "wet" asthma (3.27%) and the so called "dry" bronchial asthma (0.8%). The therapeutic conduct should take into consideration the presence of variant forms of bronchial asthma.
