Clinical characteristics of haemorrhagic fever with renal syndrome in children.

From January 1988 to September 1989, seven patients (4 girls and 3 boys, aged 3-12 years) with haemorrhagic fever with renal syndrome (HFRS) were hospitalised at the University Children's Hospital in Belgrade. In four patients the disease appeared as a family outbreak, the others were sporadic cases. In six patients the clinical presentation was suggestive of HFRS, as they had fever with headache, myalgia, sore throat and gastrointestinal illness followed by renal abnormalities. However, severe haemorrhagic syndrome with petechia, haematoma, haematemesis and melaena was present in one patient only. Renal disease presented as nephritic syndrome and/or acute renal failure. Five patients recovered after 2-3 weeks without sequellae, one patient had decreased renal function 17 months after the start of the disease and the remaining patient died. In six patients the diagnosis of HFRS was confirmed serologically by a significant rise in antibody titres against hantaviruses, while in the patient with the fetal and fulminant course of the disease, the diagnosis was established on the basis of epidemiological and autopsy findings. We suggest that children living in endemic areas who develop an ill-defined, febrile and gastrointestinal disease with renal dysfunction should be evaluated for HFRS.

Microcomputer-based portable long-term spasticity recording system.

A device for long-term monitoring of muscle activity (EMG) with surface electrodes and method of its application are described in this paper. This device is called a microcomputer two-channel EMG monitor. The device can be used for up to 24 h monitoring of EMG activity, followed by data transfer to a host computer for signal analysis. This device records amplified, rectified, and integrated EMG activity. Shorter recording time allows shorter sampling periods suitable for different other EMG analysis. Recording of spontaneous EMG in complete spinal cord injured subjects was the original reason for the design of the long-term monitor. These recordings were used for estimation of spasticity in complete spinal cord patients.

Electromyography

Optimal control for the active above-knee prosthesis.

Control of an active above-knee prosthesis has been simulated for a selected gait activity using a hierarchical closed-loop method. An extension of finite-state control, referred to as artificial reflex control, was adopted at the strategic level of control. At the actuator level of control an optimal tracking method, based on dynamic programming, is applied. This deals mainly with the actuator level of control, but considers the interaction of the leg dynamics and the switching effects of artificial reflex control. Optimal tracking at the actuator level of the above-knee prosthesis reduces the on-off effects of finite-state methods, such as artificial reflex control. The proposed method can also be used for the design of prosthetic elements. Specific attention is paid to the limited torque and power in the prosthetic joint actuator, which are imposed by the principle of self-containment in the artificial leg. The hierarchical structure, integrating artificial reflex control and optimal tracking, can be used in real time, as estimated from the number of computer operations required for the suggested method.

Medium- and long-term prognosis of patients with acute poststreptococcal glomerulonephritis.

The prognosis of acute poststreptococcal glomerulonephritis (APSGN) is still a matter of considerable debate. In an attempt to elucidate this controversy, the medium-term prognosis was evaluated in 40 patients 5-9 years after the onset of the disease, and the long-term prognosis in 88 patients 10-17 years after the onset of the disease. All were sporadic cases. In the medium-term follow-up study, abnormalities were revealed in 5.0% (2/40) of the patients. Hypertension and proteinuria were the only abnormalities detected. In the long-term follow-up study, abnormalities were revealed in 6.8% (6/88) of the patients. Hypertension was found in 3.4, proteinuria in 2.3, and microhaematuria in 2.3% of the patients. In both studies, all patients had normal creatinine clearance. We conclude that the medium- and long-term outcome of patients with APSGN is excellent.

[Attitude of an urban female population to detection of breast carcinoma].

An anonymous poll was conducted among Novi Sad women (957 employed and 1000 unemployed) in order to investigate the attitude of women towards breast cancer (BC). Preliminary research established the fact that the majority of women did not have their breasts examined in the last 3 years (64% employed and 60.7% unemployed). The consideration of the possibility of becoming affected by BC is rare or doesn't exist at all in 86.3% of women, therefore the very important emotional stimulus is absent. The general knowledge about BC is insufficient and 1395 women (69.4%) have no knowledge about the methods of breast examination or are very poorly informed, and that also regards the method of breast self-examination which is regularly used by only 9.9% of employed, and 13.3% of unemployed women. A part of the polled women does not want to see a physician out of various reasons: ignorance, indifference, cancerophobia or distrust of used methods, but a portion of the women wants to be examined but cannot make this happen because of the small number of institutions in which such examinations are conducted. Seeing a physician for breast examination is usually provoked by personal motivation, and the influence of the press, radio and TV is very small (1.7% with the employed and 3.1% with the unemployed), as well as the advise of girlfriends or the presence of BC in the family. Under all of these circumstances about 50% of the women are ready to pay all costs of breast examination regardless of the existing health insurance, and the rest of them do not accept this because of various reasons.(ABSTRACT TRUNCATED AT 250 WORDS)

[The importance of women's knowledge about cervical carcinoma and its effect on early detection].

The importance of women information on CC and its impact on their motivation for detection examinations was assessed through a poll of 3474 women (1957 from Novi Sad and 1157 from several villages in Vojvodina). The knowledge about CC reflected a rather poor and insufficient impact of standard education (5.0% in urban, 7.93% in rural areas), newspapers and magazines (4.4% in urban, 5.3% in rural areas), radio and TV (9.3% in urban, 14.7% in rural areas), specialized health magazines and books (about 7-8%), while the impact of conversation with women--14.0% in urban and 18.8% in rural areas, and of different sources--33.3% in urban and 19.8% in rural areas, was notably higher. We found the knowledge acquired in this way insufficient and incomplete which was proved by numerous answers obtained later on. There is an insufficient knowledge on the methods of early detection, unrecognized or poorly recognized by 2790 women (80.3%), with no significant differences between urban and rural areas. Data on the idea concerning possibilities of developing CC was similar--2856 women (86.3% in urban, 83.4% in rural areas) hardly if ever think of the disease. Consequences are seen in the fact that in the last three years the women never underwent DE--66.8% in urban (1302 women) and 82.3%$ in rural area (1103 women), while the minimum of two or more examinations was carried out in only 17.3% of women from the city and 6.9% of women from the villages. Insufficient knowledge was also shown on the impact of risk factors for CC.(ABSTRACT TRUNCATED AT 250 WORDS)

[Epidemiologic and clinical approaches in the prevention of carcinoma of the uterine cervix].

The current state of the epidemiology and the clinical results of UCC in our surroundings is not satisfactory and is irregular according to different parts of Yugoslavia. In the purpose of attaining a better view of the state in our surroundings, 1957 sexually mature females were polled in the territory of Novi Sad during 1987-1988 (1000 were unemployed and 957 employed) thus we will use parts of this poll in the areas which are of interest for the problems studied. According to our survey, most of the women were unadequately and unsufficiently informed about the causes of uterine cervix diseases, therefore little attention is given to the early start of sexual intercourse (1.7-2.7%), the changing of several sexual partners in a lifetime (5.5-9.8%), hygiene of male sexual organs (1.6-18.7%), and the role of abortions was overestimated (11.5-14.7%) with a considerable number of uncertain responses (22.4-2.6%). Almost 50% of the polled women do not know any possibilities for the individual prevention of UCC risk factors. Most of the women in both of the categories polled (66.0-46.6%) express the opinion that the necessary knowledge should be acquired in these areas through regular education which should be conducted at a suitable age, and there are less of those which think that this knowledge should be acquired through other sources of information (women's magazines or means for public informing and so on).(ABSTRACT TRUNCATED AT 250 WORDS)

[Morphofunctional changes in potential liver allografts in experimental conditions].

As a part of morphofunctional investigations of validations cadaveric grafts for transplantation evolution of the histomorphological changes was followed by the experiments on guinea pigs. Liver samples were immediately after sacrificing the animals and 1, 2, and 4h later and kept at body temperature, which corresponded to the conditions of "warm ischemia". Second group of samples was immerged in Saline are +4 degrees C (hibernation-preservation) and was taken for analysis in same intervals as previous ones. Evolution of morphological and enzymatic changes was evident in short interval after sacrificing the animals. First changes were observed in cytoplasm and then in nuclei and interestitum. Changes of hepatic cells from hibernation preservation group were significant damaged comparing with the corresponding samples obtained from cadaver. ("warm ischemia"). These preliminary results indicated that length of liver graft preservation is very limited. Furthermore, this results demand for the future investigations in different conditions of preservation.

[Menarche in schoolgirls in Novi Sad].

Out of 947 examined schoolgirls aged 8.5-15 from the two schools in Novi Sad, 327 of them had menarche. Arithmetic mean value of the appearance of menarche was 12.25 years, the standard deviation being 0.91 years. Median was 12.59 years. The normal appearance of menarche (X +/- 2 SD) ranged from 10.4-14.1 years in our environment. According to X2 (Chi-square) test and asymmetry coefficient (beta 1) it was pointed out that the distribution of menarche in examined schoolgirls was normal and symmetric.

Evaluation of the spectrophotometric assay of guanase with 2,2'-azino-di(3-ethylbenzthiazoline-6-sulphonate) (ABTS) as chromogen.

A simple spectrophotometric assay for serum guanase based on the oxidation of 2,2'-azino-di(3-ethylbenzthiazoline-6-sulphate) (ABTS) using xanthine oxidase, uricase and peroxidase is described and statistically examined through its application to normal and pathological sera. The method is very sensitive, precise (CV below 8.13%) and linear up to 152.5 U/l. Comparison with the methods of Hue & Free ((1965) Clin. Chem. 11, 708-715), and Giusti ((1974) In: Methods of Enzymatic Analysis, Bergmeyer, H. U., ed., p. 1086), and Ito et al. ((1981) Clin. Chim. Acta 115, 135-144) gave a good correlation (r greater than or equal to 0.969). The reference values for the ABTS-method are 2.93 to 23.92 U/l (mean = 13.57 U/l, CV = 22.43%). The mean values of guanase activities determined in sera of patients with different liver diseases (mean = 30.29 U/l), or chronic alcoholics (mean = 35.41 U/l) were significantly higher than normal. The patients with chronic diseases had significantly lower activity (mean = 7.22 U/l, t = 9.25, p less than 0.001).

Experimental soft tissue wounds caused by standard military rifles.

Soft tissue wounds were caused by penetration of a bullet through both thighs in 42 pigs, fired from N48, M70, M16A1 and FAL rifles at 50 meter range. Four comparable groups of wounds, consisting mainly of injured muscle, were examined. Primary debridement was carried out 5-6 hours following wounding, multiple muscle biopsies up to 40 mm sidewise of permanent wound channel were taken for HE and histochemistry. Primary contamination was traced using marker aerobic bacteria. Bullet velocities at impact and exit were measured and transfer of energy to liver tissue calculated when feasible. The condition of the bullet after exit was precised. The surgical estimation of tissue injury, the mass of excised tissue and microsections have served to evaluate the extent of tissue injury. For 22.5 cm mean channel length the most injury was caused by the FAL, followed by M16, M70 and M48 rifles in declining order. Local effects are ascribed to bullet tumbling, break-up and energy transfer of corresponding missiles. Correlation of surgical and pathology methods is discussed.

[Significance of angiography of the deep femoral artery in surgical therapy of obliterative diseases of the lower extremities].

Difficulties occuring in angiography of profunda femoris artery during performance of peripheral angiographies are presented. Better presentation can be achieved when applying two of the known methods: by contrast injecting during Valsalva manouever, or synchronously with heart action. Collateral circulation in stenosis or obliteration of profunda femora artery are here presented.

[The role of colposcopy in the follow-up examinations of the treated genital carcinoma in women].

The authors present their own experience about the colposcopic findings in the vagina after radical operative interventions, radiological or combined radiological and surgical treatment of carcinome of the genitals in women. On the basis of 902 checkups of such patients in whom there were 65 (7,20%) suspect colposcopic findings and 43 (4,76%) relapses the authors present detailed descriptions of the colposcopic patterns of relapses, that is, metastases and degenerative changes in the vagina after the above mentioned therapeutical procedures.
