The negative impact of oxidative stress in chronic cardiac failure and coronary heart disease calls for a new pathophysiologic and therapeutic approach.

Oxidative stress, when associated with ECG aspects of "coronary disease", indicates an accelerated course of atherosclerosis in the onset phase (by cellular involvement, migration, proliferation, etc.) and an additional thrombogenic risk in the "ischemic disease" phase, when associated with chronic cardiac failure, it indicates a progressive form with "replacement fibrosis". The therapy that reduces the oxidative stress can slow down the physiopathologic process, and clinically improves the patients' condition and prognosis.

Cardiothyreosis.

In the present study: (a) physiopathology, (b) clinics, and (c) therapy of cardiothyreosis are discussed. (a) The hyperkinetic syndrome, the earliest clinical sign in thyrotoxicosis (vasodilatation, increase in inotropism, automatism, etc.), is mediated by a two-fold increase in the number of beta-receptors, and supported by an adequate synthesis of ATP and creatinphosphate (CP) in the young and, to a lesser extent, in the elderly. Genetical heart reserves are mobilized, thus significantly increasing the number and the size of mitochondria and also the enzymatic equipment (such as: the alpha-glycerophosphate-dehydrogenase, malic, pentosic cycles, etc.), a.s.o. Due to an excessive adrenergic action (glycogenolysis, an excessive oxygen consumption, up to necrosis, the ATP and CP syntheses dramatically drop; the phosphorus/oxygen ratio decreases to 2 (normal = 4). In this condition, the high functional cardiovascular performances are also impaired (the submaximal effort capacity is attained at a smaller and smaller oxygen consumption; Propranolol 2 mg i.v. decreased the cardiac output by above 30% (vs 10%--normal); electrocardiogram presents aspects of "coronary disease", tachycardia, etc.). An ultrastructural damage occurs: from "mitochondrial disease", partial lysis of myofibrils, to myofibrosis (revealed postmortem), in spite of a reduced degree of coronary atherosclerosis. Ultrastructural and biochemical experimental data support this point of view. (b) The incidence, precocity and severity of the thyrotoxic heart increase with age and the existence of a previous cardiovascular pathology. Cardiothyreosis is not present under 27 years; in 4,353 patients its incidence is of 25% (arrhythmia--21%, heart failure--12%, coronary insufficiency--1-3%). Of a major interest are tachyarrhythmias which may lead to a high mortality by hypodiastolic congestive heart failure, heart failure with secondary hyperaldosteronism, thromboembolic episodes and ventricular fibrillation. Thyrotoxicosis favours the disease of papillary muscles--mitral prolapse and insufficiency, reversible especially in children. (c) The treatment of thyrotoxic heart is an etiologic one (medical, surgical, radioactive--the last two being preferable after the adequate medical therapy). In particular, cardiothyreosis requires a reinforced irradiation (10,000 rads instead of 7,000 rads) in smaller 131I doses. The protection against the increased nocivity of catechols in thyrotoxicosis is very important (which explains the high mortality in the thyrotoxic "storm") and requires propranolol; doses above 2 mg/kilo body/day are recommended. In the elderly, the sensitivity to propranolol decreases: verapamil i.v. is more efficient in paroxysmal tachyarrhythmias (flutter, atrial fibrillation) and in those occurring intra-operatively during halothane narcosis. The anticoagulant therapy is administered in tachyarrhythmias with high ventricular rate, especially in the elderly, to avoid the embolic risk, higher in defibrillation condition.(ABSTRACT TRUNCATED AT 400 WORDS)

[Test for evaluation of the effectiveness of hormone treatment in carcinoma of the prostate].

Cytological examination of superficial epithelial cells systematically sampled at the level of the navicular fossa of the urethra in 61 patients with prosthatic carcinoma that had underwent surgery or that had not been operated, in whom longterm treatment with oestrogens was applied led to the conclusion that the caryopyknotic index shows variable increases, in some cases attaining more than 30-40%, in direct connection with the amount and the duration of administration of the oestrogen substances. The results obtained are encouraging and suggest the possibility to apply an objective test in the application and the evaluation of the efficiency of a hormonal treatment.

Deviation of plasmatic aldosterone and significance of the stimulating (ACTH-cortrosyn) and inhibitory (propranolol) effects in hypertension and cardiovascular pathology.

Starting from the interference between the renin-angiotensin-aldosterone (RAA) system and cardiovascular pathology in arterial hypertension (AH) we have made a correlative study of the cardiovascular system and plasmatic aldosterone in normal and hypertensive subjects under conditions of stimulation (ACTH) and inhibition (propranolol). After administration of ACTH (one of the physiologic mediators of stress) increased values for plasmatic aldosterone were found. Negative cardiovascular effect: arrhthmia, angor, AH, cardiac asthma, under the condition of preexisting cardiovascular pathology or altered steroidogensis, were also noticed. Inhibition with propranolol does not have conclusive effects in AH with normal aldosterone. The best effect were noticed in that hypertension which implies the RAA pressor system in its pathogeny, irrespective of etiology, the reduction AT occurring in parallel with decline in plasmatic aldosterone values and total peripheral resistance (RPT). Administration of propranolol in AH with activated RAA system irrespective of etiology represents a pathogenic treatment able to prevent efficiently the major complications of AH.

Adrenocorticotropic Hormone

Cardiovascular pathology in acromegaly and some effects of the 90 yttrium implant in the hypophysis.

Examination of the cardiovascular apparatus (cv) of 25 acromegalic patients revealed an increased incidence of cardiovascular pathology as against normal individuals. Acromegalic patients with arterial hypertension (AH) show a twice higher incidence of ischemic cardiopathy, cardiomegaly with obvious or latent cardiac failure, arrhythmias, which frequently lead to death. The uncertain pathogeny of cardiomyopathy implied: hypoxia due to heart hypertrophy (over 500 g and increased cross-section of the fibre to over 26/mu), adrenergic deficiency resulting from increased protein synthesis and decrease in thyrosine, a precursor of myocardic cathecolamines. The increased incidence of coronary pathology is favoured in acromegalic patients by diabetes, hyperlipoproteinemia, low endogenous heparin, increased ureic acid and platelet adhesivity. Hypophysectomy by hypophysolysis (20 cases) brings relief and cures cardiovascular pathology, reduces AH, corrects the coronary risk factors, cures hypophyseal diabetes. The existence of AH and cardiovascular pathology is an indication for surgical treatment in evolutive acromegaly.

Acromegaly

Disorders induced by pheochromocytoma in the cardiovascular system. A therapeutic approach.

The diagnosis of pheochromocytoms is still made only post mortem in a high percentage of the cases. It is important to know both the diversity of clinical forms (of the 16 cases investigated 13 (80%) presented permanent AH, of which 12 cases with paroxysmal attacks, followed by collapse only in one case, and 3 (20%) with paroxysmal AH on a normotensive background) and the severity of cardiovascular accidents, which increases twofold during crises (arrhythmias, left ventricular failure, coronary insufficiency, cerebrovascular strokes, secondary adrenalinic shocks). These complications are preceded by the "alarm syndrome". The preoperatory treatment is associated: blocking of adrenergic alpha-receptors and beta-receptors; correction of hypovolemia, also applied during the surgical phase I (until the venous ligature is made and the tumor excised) under continuous monitoring (ECG, ABP and central venous pressure). In phase II vasoplegin should be promptly corrected by nor. E+E, and corticoids.
