The incidence of total hip replacement in orthognathic surgery patients receiving short-term steroid therapy.

It is well known that long-term corticosteroid therapy can lead to avascular necrosis of the femoral head. There is suspicion among orthopedic surgeons that short-term, high-dose steroid use poses similar risks. The records of 2,773 patients (1,497 who had total hip replacement and 1,276 who had orthognathic surgery) were retrospectively reviewed. No hip replacements had been done in the orthognathic surgery group and none of the patients in the total hip replacement group had undergone orthognathic surgery. The use of short-course, systemic corticosteroids to reduce postoperative swelling in orthognathic surgery did not seem to cause avascular necrosis of the femoral head.

Risks and benefits of removal of impacted third molars. A critical review of the literature.

A critical review of the literature about risks and benefits of the removal of impacted 3rd molar teeth is presented in 4 categories: risk of non-intervention, risk of intervention, benefit of non-intervention and benefit of intervention. There are well-defined criteria for removal of impacted 3rd molar teeth. Absolute indications and contra-indications for the removal of asymptomatic 3rd molar teeth cannot be established because no long-term studies exist which validate the benefit to the patient either of early removal or of deliberate retention of these teeth. The prudent course of action for the clinician to follow is based on rational clinical decision-making using traditional methods of evaluation to effect the optimal outcome, keeping the interests of the individual patient above all else.

Balanced facial growth: a schematic interpretation.

Balanced facial growth is a complex process that involves maxillary, mandibular, dental, and cranial growth. Growth of the maxilla is due to special movements of translation, rotation, and elongation as well as to growth of its skeletal units and skeletal structures. Growth of the mandible is the sum of growth of all of its skeletal units. Cranial growth influences both maxillary and mandibular positions, which themselves can vary interdependently, in part because of the mutual influences of the maxillary and mandibular dentitions.

Influence of the nasal septum on maxillonasal growth in patients with congenital labiomaxillary cleft.

An interpretation of the role of the nasal septum on maxillonasal growth in patients with congenital labiomaxillary clefts is presented. Salient features of the anatomic pathology and resultant dysfunction of the premaxillary-maxillary suture and the position and orientation of the nasolabial muscles relative to the nasal septum are discussed for unilateral and bilateral deformities. The nasal septum plays an important direct role in growth of the premaxilla and an indirect role in growth of the maxilla.

Avoidance of the use of vomerine mucosa in primary surgical management of velopalatine clefts.

A brief review of normal maxillary growth and the dysfunctional pathoanatomy of velopalatine clefts is presented. A plan of management for patients with these deformities is proposed; this is based on the degree to which the deformity departs from normal. The authors stress the importance of early, complete reconstitution of the soft palate and avoidance of the use of vomer flaps in the closure of the hard palate. The influence of the vomer, nasal septum, and labionasal musculature on postnatal facial development is discussed.
