Epidemilogy of stroke in the Poznań district of Poland.

The incidence of cerebrovascular disease was established in the Poznań district in Poland for 1985. Mortality from cerebrovascular disease during this year was compared with mortality during 1977. The autopsy material from the years 1925 to 1985 was analyzed to differentiate ischemic from hemorrhagic stroke. The incidence of stroke was 198.3 per 100,000 inhabitants greater than 20 years of age. Mortality from cerebrovascular disease decreased significantly in both males and females from 1977 to 1985. Analysis of the pattern of cerebrovascular disease in the autopsy material from 1925 to 1965 demonstrated an overwhelming percentage of hemorrhagic stroke. In the years that followed, an inversion was noticed in the ratio of ischemic to hemorrhagic stroke. It should be assumed that the growing efficacy of antihypertensive therapy largely explains the decreasing incidence of stroke-induced mortality.

[Epidemiology of Parkinson disease in the Pozna'n province].

The authors analyzed the prevalence and incidence of parkinsonism in the Poznań region. The research led to the following conclusions: 1. Incidence of parkinsonism in the Poznań region amounts to 12.6 per 100,000 of population, which is the middle value of scatter in the investigation on a world-scale. The prevalence of the disease--65.9 per 100,000--is the lower range of values found in many countries of the world. This seems to point to a shorter survival time of patients with parkinsonism in our country. 2. Among various age groups, the highest incidence and prevalence of parkinsonism is found in the age groups of 70-79 and 80-89, with slightly higher prevalence rates among males. 3. The authors found in the Poznań region irregular prevalence of parkinsonism, that is the existence of small focuses of the disease, which seems to depend on unfavourable genetic factors existing in those places.

[Changes in stroke mortality rates during the years 1977 and 1985].

The mortality from stroke in the years 1977 and 1985 was compared in the Province of Poznań. The mortality from strokes decreased in that time period considerably in the population of males and females. The differences were significant from the age group 40-49 years. Reduced mortality from stroke in the Province of Poznań contrasted with a reverse trend in the mortality from ischaemic heart disease, which has been increasing continuously. It may be supposed that increasing effectiveness in the treatment of arterial hypertension may explain, in part at least, reduction of the mortality index from strokes. One cannot exclude, however, taht partial elimination of other risk factor due to changed way of life, changed dietary habits, may play also a considerable role in the positive trends in the epidemiology of strokes.

[A case of demyelinating disease difficult for classification].

A case of difficult for classification demyelinating disease of diffuse (transitional) type is presented. The described case seems to testify about the identity of hyperergic (para-infections) encephalomyelitis and acute multiple sclerosis of diffuse type.
