Adult Kawasaki syndrome.

Kawasaki syndrome (KS) is an idiopathic, acute, febrile, exanthemous illness that primarily affects infants and children. We describe a 20-year-old black woman who fulfilled the clinical criteria for the diagnosis of KS and excluded other possible causes. In addition, we reviewed data on 21 patients with adult KS reported in the English literature and accepted ten cases as representing this syndrome. The epidemiologic, clinical, laboratory, and pathologic features of the 11 cases representing adult KS are discussed. Although the initial reports of adult KS in the United States may have actually represented toxic shock syndrome, the occurrence of KS in adults should be acknowledged.

Von Recklinghausen's disease associated with gastrointestinal carcinoid tumors.

The occurrence of both gastrointestinal carcinoid and von Recklinghausen's disease in the same patient is uncommon. There seems to be a predilection for ampullary carcinoids in these patients. This article reports the sixth case, including a review of the literature and a brief discussion of the implications of this finding. A carcinoid tumor should be suspected in any patient with von Recklinghausen's disease presenting with gastrointestinal bleeding, obstruction, abdominal pain, and particularly obstructive jaundice.

Peripheral arterial thrombosis due to the nephrotic syndrome: the clinical spectrum.

We reviewed the literature concerning arterial thromboses associated with the nephrotic syndrome and the hypercoagulable state. An illustrative case of peripheral arterial thrombosis and pulmonary emboli attributed to the hypercoagulable state of the nephrotic syndrome is included. In our patient, serum levels of antithrombin III, plasminogen, factor VII activity, factor VIII activity, and factor VIII antigen were abnormal. Arterial thromboses in patients with the nephrotic syndrome are often associated with steroid and diuretic use. In pediatric patients, a predisposing factor may be femoral venipuncture.

Mega cisterna magna: diagnosis using metrizamide computed tomographic cisternography.

The mega cisterna magna is one cause of a midline, extra-axial, posterior fossa cyst. The computed tomographic features and differential diagnosis of midline posterior fossa cystic structures are reviewed, and the clinical features of the mega cisterna magna are discussed. We describe a method of metrizamide cisternography and report a case of mega cisterna magna that was diagnosed by this technique.

Abdominal aortic aneurysm with initial symptom of duodenal obstruction.

Duodenal obstruction as the initial symptom of an abdominal aortic aneurysm is unusual. We report the 13th case and our findings from a review of the literature. This entity should be considered in an older individual when duodenal obstruction is associated with findings of a pulsatile abdominal mass, or when abdominal films show calcification of the aneurysm or bone erosion.
