Clear cell sarcoma of soft tissues. Mayo Clinic experience with 35 cases.

Thirty-five cases of clear cell sarcoma of soft tissues were studied to determine the clinical or morphologic features that are important in predicting prognosis. Tumors occurred most commonly in the extremities, and the majority of the patients were young women. Surgery was the elected treatment in every case. Five patients experienced local recurrences, and metastases developed in 22. Fifty-four percent of the patients died of tumor, 11% are alive with disease, and the remaining 34% are alive and well; the average survival for each group was 67 months, 113 months, and 103.5 months, respectively. This sarcoma is characterized by small clusters of polygonal to spindle cells featuring clear to slightly basophilic cytoplasm and vesicular nuclei with prominent nucleoli. The clusters are separated by delicate fibrous septa. In a deletion, clear cell sarcoma has low mitotic activity, little or no necrosis, and mild nuclear pleomorphism. Tumor size and the presence of necrosis are statistically significant predictors of prognosis. All 12 patients with tumors measuring > 5 cm died of disease or are alive with disease. Eleven of the 20 patients with tumors measuring < 5 cm are alive with no evidence of disease. Tumor necrosis was present in 10 cases; eight of these patients died of disease and one is alive with disseminated metastases.

Adolescent

Arborescent bacterial keratopathy (infectious crystalline keratopathy).

Bacterial colonisation of the cornea is described in two cases, one bilateral. Discreet, white, branching opacities are produced without associated inflammation. It is suggested that due to its aetiology "arborescent bacterial keratopathy" is a better name for this condition than "infectious crystalline keratopathy".

Malignant melanoma of the conjunctiva.

The clinicopathological features of 37 patients with invasive melanoma of the conjunctiva have been studied. Prognosis was closely related to the subsite and size of the primary tumour. Twenty of 21 patients with small localized bulbar neoplasms (95%) and four of six patients with diffuse bulbar melanomas (67%) have survived with no evidence of secondary spread. By contrast, only one of six patients with neoplasms involving the fornix (17%) and two of four (50%) with caruncular melanomas have survived. Metastatic spread was very uncommon in patients with melanomas less than 1.5 mm in maximum thickness, but the outcome of the disease in patients with tumours greater than 1.5 mm was not always bad. Treatment by local excision biopsy was followed by a high rate of conjunctival recurrence (59%). Exenteration of the affected eye guarded against the development of further orbital disease, but was not infrequently followed by the appearance of metastases. Many (62%) of the tumours appeared to have arisen in a pre-existing melanotic lesion or pigmented naevus of long-standing. Histologically, the tumours could be divided into those with an adjacent intra-epithelial component, manifest as atypical melanocytic hyperplasia in the conjunctival epithelium adjacent to the invasive melanoma, and those without (nodular melanoma). However, clear separation of the former group into the subtypes described for cutaneous melanomas proved impossible. Prognosis was not related to the type of melanoma, mitotic rate, cell type or degree of pigmentation.

Adult

Successful intraocular lens implantation: a histological and scanning electron microscopic study of four cases.

The histological findings in four globes obtained at autopsy from clinically uncomplicated cases with intraocular lens implants, one glass posterior chamber and three anterior chamber lenses consisting of two pupillary fixated and one "in-the-bag" fixated Binkhorst lens, are reported. Minimal changes could be detected in the tissues with which the implants were in contact. Scanning electron microscopy of the cornea in the three anterior chamber implants showed a loss of endothelial cells in the axial region of roughly 30-50%. There was marked vertical disparity in the loss. Scanning microscopy of the implants showed evidence of surface degradation of the nylon loops of all the anterior chamber implants.

Getting the message across.

Messages do not simple 'get across'. The author points out the importance (1) of seeing good communication as an effort of understanding between human beings and (2) of dismissing any idea of a single communicator's skill. There are many and different skills to be applied.

Communication

Ocular injuries from liquid golf ball cores.

Tissue removed from nine new cases from 18 hours to 20 weeks after injury by a golf ball contained crystalline and other foreign material to which there was a mild inflammatory reaction followed by macrophagic activity and fibrosis. Optical and electron probe analysis showed that the crystalline material was crushed barytes containing small quantities of muscovite as is typical in natural deposits. The centres of several golf balls were shown to contain essentially identical material. By contrast with previous reports, no zinc sulphide was found. The form and frequent location of the deposits in the conjunctiva as compared with cornea and eyelid is related to the structure of these tissues.

Adolescent

Experimental immune retinitis: induction by isolated photoreceptors.

An antigen prepared from the outer limbs of the photoreceptors failed to produce retinal lesions within 3 months by systemic immunisation alone in the rabbit. Intra-vitreal injection of the antigen in a pre-immunised rabbit resulted in an Arthus type response. Repeated systemic immunisation for over 1 year after an intravitreal injection produced retinal degeneration in the uninjected eyes of two rabbits. Anti-visual cell antibodies were not shown to be cytotoxic to organ cultures of retina. Possible reasons are discussed in relation to in vitro studies of thyroid, kidney and spinal cord.

Animals
