[Magnetic resonance imaging--diagnosis and differential diagnosis of tumor formations (cyst) in the abdomen cavity during pregnancy].

The existence of tumor formations with unidentified origin in the abdomen cavity of the pregnant can cause serious problems during the operative intervention. It is critical to find the organ origin of the existing formation and it's relations with the neighboring organs. In the following article we present some of the capabilities of the magnetic resonance tomography as a complementally to the existing in the clinical practice classical and special methods for diagnosis and differential diagnosis of tumor formations in the abdomen cavity during pregnancy. MRI locates precisely the organ origin, the type and the size of the cyst formation, furthermore it helps to determine in detail the access point, the type and size of the operative intervention. The technique is harmless concerning the pregnancy and the development of a normal, healthy fetus.

[Choledochoduodenal anastomosis--pro and contra].

Choledochoduodenostomy (CDS), suggested by T. Roth, and performed for the first time by O. Sprengel (1891), became a definitely affirmed operation after the publications of F. Sasse (1913). From purely anatomical standpoint, CDS contributes to bypass all problems relating to obstruction in the retroduodenal, transpancreatic and intramural segments of the hepatocholedochus with a wide orifice for spontaneous passage of eventual newly formed or residual concrements being secured. The basic indications for performing CDS include: multiple concrements in the extrahepatic biliary ducts, ampullar stenosis, cholangitis, chronic pancreatitis, cyst induced enlargement of choledochus and the like. It is worth noting that in most patients more than a single indication are present. The major shortcomings of the procedure with a reference to the long-term results are attributed to potentialities of cholangitis and cholangiohepatitis development, and "blind sac" formation between the anastomosis and p. Vateri. It is established that all patients with poor post-CDS results have stenosis of the anastomosis, or else, the restoration of papillary patency fails (endoscopically confirmed), i.e. they are most frequently a sequel of technical errors. CDS is a safe, readily performed and effective surgical procedure with few (sporadic) postoperative complications, including patients undergoing long-term follow-up study.

[Morphological changes in experimental duodenogastric reflux].

The model of a total duodenal-gastric reflux was developed in 20 dogs and morphological changes in the stomach were studied. 5 dogs served as control. Histologic changes occurred 3.5 months and 1 year after the operation were characterized by metachromasia, increased mucus formation, proliferation of epithelial and parietal cells, moderate inflammation. Process of the disease had fluctuations. 6 months after the operation 5 dogs were reoperated for the elimination of reflux and were observed for another 6 months. A complete reversibility of all changes was established. Bacteriological studies showed a considerable increase of bacteria population in the stomach and a pH increase. The authors suggest the term 'reflux gastropathy" to replace the term "alkaline reflux-gastritis".

[The place of the screening method in the early discovery and treatment of cryptorchism].

According to modern concepts, only early operative treatment of cryptorchidism may guarantee good results. This has been proven by the results obtained by the authors, who for five years have on mass prophylactic examination (1984-1988) searched for cryptorchidism in children attending kindergartens in the town of Tolbukhin. In this way the authors succeeded in realising highest operative activity in the age from 3 to 4 years.

[The incidence of duodenogastric reflux and its relation to stomach and duodenal diseases].

The data available in the literature on the incidence of duodenogastric reflux and its relation with diseases of the stomach and duodenum are controversial. A study was performed based on the results of 4256 primary endoscopies of the stomach and duodenum: 3673 in nonoperated patients and 583 in patients having undergone vagotomy and resection. For duodenogastric reflux one judged from the yellow coloration of the gastric juice and regurgitation of bile at the moment of examination. Reflux had 428 patients (10.05 per cent)--330 in the nonoperated group (8.98 per cent) and 98 in the operated group (16.81 per cent). Inference is drawn that duodenogastric reflux does not coincide in incidence with reflux gastritis and that causal relation might be searched between gastritis, reflux esophagitis and duodenitis, on the one hand, and duodenogastric reflux, on the other.

Duodenal Diseases↗

[The radiological study of duodenogastric reflux following surgery for perforated duodenal ulcers].

Radiologic aspiration method (99mTc-bound with EHIDA-148 MBq) was used for quantitation of duodenogastric reflux, 3 months after operations for perforated duodenal ulcers. The patients were divided in three groups of 10 each: gr. II--after simple suturing; gr. III--after truncus vagotomy and Heinecke-Mikulicz pyloroplasty and gr. IV after proximal selective vagotomy and rrhaphy. The results were compared with those in a group of practically healthy patients (gr. I). The radioactivity of the gastric aspirate was measured between the 30. and the 60. min at 15 min interval. It was found that proximal selective vagotomy with rrhaphy had minimal duodenogastric reflux and there was no difference with practically normal subjects. Truncus vagotomy was associated with significant duodenogastric reflux. Similar were the results with simple suturing.

Duodenal Ulcer↗

[The quantitative assessment of duodenogastric reflux following operations for perforated duodenal ulcers].

Sixty patients operated for perforated duodenal ulcer were examined for the presence of duodenogastric reflux by quantitative estimation of bilirubin in gastric aspirate for a period of 3 to 6 months after the operation. Twenty healthy subjects served as controls. The operated patients were divided in three groups depending on the type of operation: simple suturing, truncus vagotomy with pyloroplasty according to Heinicke-Mikulicz technique and proximal selective vagotomy with rrhaphy (20 patients in each group). It was shown that duodenogastric reflux following proximal selective vagotomy with rrhaphy was almost as common as in the control group. Peak levels of gastric bilirubin were found after truncus vagotomy with pyloroplasty (2.01 +/- 0.51 mmol/l). A practicable method of examination of bilirubin in gastric juice, which can gain acceptance in every hospital laboratory, is suggested.

[Our experience in treating sub crustam skin burns].

Alcoholic solution of chlorhexidine and methylene blue was used in 64 patients for local treatment of burned skin surfaces. The wounds healed under the crust, formed from denatured and desiccated fibrin coats and necrotic tissues. The two antiseptic agents (chlorhexidine and methylene blue) provided long-term antibacterial effect. The results were encouraging; epithelialization of rather deep burns was shortened by 4 or 5 days.
