Magnetic resonance imaging evaluation of the ascending aorta after graft-inclusion surgery: comparison between an ultrafast contrast-enhanced MR sequence and conventional cine-MRI.

The aim of this study was to compare the value of two magnitude-based MR sequences, a contrast-enhanced ultrafast MR sequence and a cine MR sequence, for the detection of flow around the graft in the postoperative ascending thoracic aorta. Thirteen patients who underwent ascending aortic surgery for dissection (n = 11) and aneurysm (n = 2) were enrolled. They were referred to MRI for mediastinum enlargement on the chest x-ray or mediastinal symptoms. All patients had replacement of the diseased ascending aorta using a graft-inclusion technique (with wrapping of the native aorta over the graft). Delays between surgery and MRI ranged from 15 days to 8 years (mean: 10 months). All patients were examined using a T1-weighted spin-echo sequence, a magnitude-based cine gradient-echo sequence, and an ultrafast contrast-enhanced MR sequence. MR images of the cine and the contrast-enhanced sequences were retrospectively and independently analyzed by two qualified radiologists for evidence of flow or thrombus around the graft, including perigraft structure and aneurysmal formations. Evidence of flow was detected by both methods in all aneurysmal formations of the ascending aorta located at suture level (n = 6). Evidence of flow in the perigraft structure was found in seven cases by contrast-enhanced MR sequence and in only four cases by cine MR sequence. Contrast-enhanced ultrafast MR sequence could be proposed as a minimally invasive and fast modality for assessing the perigraft structure, in graft-inclusion surgical procedures of the ascending aorta, when noncontrast enhanced MR sequences do not exclude the possibility of flowing blood in this structure.

Ultrasmall superparamagnetic iron oxide particles (AMI 227) as a blood pool contrast agent for MR angiography: experimental study in rabbits.

The purpose of this study was to evaluate the contribution of an ultrasmall superparamagnetic iron oxide particles (USPIOs) based contrast agent (AMI 227), in a transverse three-dimensional time-of-flight TONE MR angiography sequence of abdominal aorta in rabbits. The main goal was to assess improvement in the visualization of small arteries such as renal arteries, when using such a sequence. Imaging experiments were performed on a 1.5 T magnet, using a transverse 3D time-of-flight (TOF) tilted optimized nonsaturating excitation (TONE) sequence with magnetization transfer suppression. The contrast media used were composed of a USPIO core surrounded by a dextran-surfactant (AMI 227). Different concentrations of AMI 227 were evaluated in 12 rabbits. Concentrations varied within the range 8.5-34 micromol Fe/kg - bw: 8.5 micromol Fe/kg (three rabbits); 17 micromol Fe/kg (three rabbits); 25.5 micromol Fe/kg (three rabbits); 34 micromol Fe/kg (three rabbits). A visual analysis based on the improvement of visualization of small arteries (renal arteries) on MIP images and a quantitative analysis based on the percentage of contrast enhancement of the aorta plotted against distance in the slab from the top edge of the acquisition volume were obtained. A signal-to-noise ratio enhancement of the distal part of the aorta and only improvement in the delineation of the renal arteries were noted when using low concentrations of the contrast media. A loss of signal-to-noise ratio of the aorta and a decrease in arterial visualization were respectively noted with higher concentration of contrast media. In this experimental study, using a transverse three-dimensional TOF TONE MR angiography sequence of renal arteries, in which sequence the saturation effect is minimized, the use of AMI 227 allows only improvement in the delineation of small arteries when using low concentrations of contrast media.

Clinical assessment of a new stereoscopic digital angiography system.

PURPOSE: To assess the clinical feasibility of an experimental modified angiographic system capable of real-time digital stereofluoroscopy and stereography in X-ray angiography, using a twin-focus tube and a stereoscopic monitor. METHODS: We report the experience obtained in 37 patients with a well-documented examination. The patients were examined for coronary angiography (11 cases), aortography (7 cases), pulmonary angiography (6 cases), inferior vena cava filter placement (2 cases), and cerebral angiography (11 cases). Six radiologists were asked to use stereoscopic features for fluoroscopy and angiography. A questionnaire was designed to record their subjective evaluation of stereoscopic image quality, ergonomics of the system, and its medical interest. RESULTS: Stereofluoroscopy was successfully used in 25 of 37 cases; diplopia and/or ghost images were reported in 6 cases. It was helpful for aortic catheterization in 10 cases and for selective catheterization in 5 cases. In stereoangiography, depth was easily and accurately perceived in 27 of 37 cases; diplopia and/or ghost images were reported in 4 cases. A certain gain in the three-dimensional evaluation of the anatomy and relation between vessels and lesions was noted. As regards ergonomic considerations, polarized spectacles were not considered cumbersome. Visual fatigue and additional work were variously reported. Stereoshift tuning before X-ray acquisition was not judged to be a limiting factor. CONCLUSION: A twin-focus X-ray tube and a polarized shutter for stereoscopic display allowed effective real-time three-dimensional perception of angiographic images. Our clinical study suggests no clear medical interest for diagnostic examinations, but the field of interventional radiology needs to be investigated.

MR imaging of body fluid collections.

To evaluate the potential of magnetic resonance (MR) to characterize body fluids in vivo, we determined the relaxation times and the relative MR signal intensities of 42 body fluid collections in 42 patients. Twelve normal volunteers served as controls. We also studied albumin solutions at different concentrations and blood at various periods in vitro. Because of their long T1 relaxation times, most nonhemorrhagic fluid collections had low to intermediate intensity on images obtained with short repetition time (TR) and short echo time (TE) settings. Although the relaxation times and relative MR signal intensities of noninfected collections differed from those of infected collections, the values overlapped. On images obtained with short TR and short TE, blood in acute hemorrhages had intermediate signal intensity and serum in subacute hemorrhagic collections was the only pathologic fluid producing high signal intensity. Because of their relatively long T2 values, all the fluid collections were intense on images obtained with long TR and long TE settings; consequently, differences in intensity were less evident than on images obtained with short TR and short TE settings. Magnetic resonance allows reliable discrimination of subacute hemorrhagic collections from collections of other types, but the differentiation between acute hemorrhagic collections; nonhemorrhagic, noninfected collections; and nonhemorrhagic, infected collections is less accurate.

MR diagnosis of lipoid pneumonia.

High signal intensity on both T1 and T2 spin-echo sequence was found with magnetic resonance imaging in one case of lipoid pneumonia related to mineral oil aspiration. This finding, rather specific of lung parenchyma infiltrated by lipids, may help for the diagnosis of lipoid pneumonia.

Magnetic resonance imaging of human orthotopic heart transplantation: correlation with endomyocardial biopsy.

The ability of magnetic resonance imaging (MRI) to detect allograft rejection was studied concomitantly in two centers. In 29 patients MR images performed on a 0.5 T imager were compared with pathologic findings obtained by transvenous right ventricular endomyocardial biopsies. Eight patients had grade 1 or 2 acute rejection, and their myocardium thickness was increased significantly compared with values obtained in normal volunteers, in normal heart allograft patients, and in patients during the resolving phase of an acute rejection episode, whereas no abnormal signal intensity was found in the myocardium of heart allograft patients with acute rejection. This finding is quite in opposition to what was found in experimental models. This could be the result of the immunosuppressive regimen of those patients including cyclosporine, whereas few experimental studies included cyclosporine treatment. For chronic rejection or fibrosis, MRI did not allow the diagnosis because the MRI appearance was close to normal. Finally, MRI appears as a complementary technique to B-mode ultrasound in detecting nonimmunologic complications such as pericardial effusions and endocavitary processes. These data suggest that MRI could be used in the future to monitor rejection, to guide the timing of endomyocardial biopsy, and to assess the response after immunosuppressive treatment.

Comparison of ionic and non-ionic nitroxide spin labels for urographic enhancement in magnetic resonance imaging.

Two nitroxide spin labels (NSL) were compared for in vitro relaxivity and in normal rats for efficiency of urographic enhancement. One of the NSL, PCA, a pyrrolidinyl agent, was ionic and one, NAT, was a non-ionic pyrrolidinyl NSL with multiple hydroxyl substituents for water solubility. Using both NSLs the renal medulla and papilla were noted to show greater contrast enhancement than the cortex, with a maximum enhancing effect between 5 and 15 minutes. Using doses of 1.0 and 2.5 mmol/kg, more than 100 per cent increases in spin echo intensities above the baseline were observed. The lowest tested dose of 0.1 mmol/kg showed an easily detectable enhancing effect for NAT. The good contrast enhancing properties of NAT, considered together with its better acute tolerance, justifies further investigation of this non-ionic compound.
