Pellet embolization to the right atrium following double shotgun injury.

A 28-year-old man sustained two shotgun injuries of the left inguinal region from a distance of about 1.5 m by simultaneous discharge of both shells from a sawn-off double-barrelled 16-bore shotgun (diameter of the lead pellets, 4 mm). The first X-ray examination carried out soon after hospital admission showed a single embolized pellet near the right margin of the cardiac silhouette. Eight months later, the man committed suicide by drug intoxication. At autopsy, the embolized pellet was found embedded between the pectinate muscles of the right atrium. On the basis of the reported case and with reference to the pertinent literature, the paper points out the medico-legal aspects of venous bullet/pellet embolism and the risk of lead poisoning after shotgun injury.

Clinical findings in the medico-legal investigation of arsonists.

Criminal and pathological arson is often committed with the help of fire accelerants. The formation of an explosive gas/air mixture may lead to an explosion whose extent and fierceness is often underestimated by the offenders. This explosion causes typical heat changes on exposed persons: * Singeing of eyelashes, eyebrows and hair growing near the frontal hairline * Singeing of the beard * Singeing of body hair in regions not covered by clothing (e.g. back of the hand) * In severe cases, skin burns of varying degree (back of hand, face). Such heat-related changes can sometimes be detected weeks after the offence.

Fractures of the base of the skull in charred bodies--post-mortem heat injuries or signs of mechanical traumatisation?

Based on a recent case, in which an expert opinion had to be prepared, the question was investigated if fractures of the base of the skull can result from the influence of heat on the human skull. Neither the retrospective analysis of autopsy records nor the prospective examination of charred bodies revealed any cases with heat-induced fractures of the base of the skull. Observation of cremations showed that the changes caused by the fire followed certain rules: fractures of the calvaria were seen after approximately 20 min; the base of the skull became exposed after about 45 to 60 min. In none of the 20 cremations watched could any fractures of the base of the skull be detected.

Complete transection of the trunk of passengers in car accidents.

Traumatic amputation of extremities and complete severance of the trunk have been reported in extra-urban collisions between passenger cars and pedestrians at collision speeds of > 80-100 km/h (50-62 mi/h). In car passengers, such extreme types of injuries are very rare. Two cases are presented in which the cars had a lateral collision with road trees near the right B column (column between front and back door), as a result of which the car was torn into two parts just in front of the rear axle near the back seats. Under these circumstances, the trunk of the back passenger on the side of the collision was completely severed. Both accidents happened in left-hand bends and the speed of collisions amounted to 120 and 180 km/h (74 and 111 mi/h), respectively. All passengers were flung out of the cars.

Postmortem injuries inflicted by domestic golden hamster: morphological aspects and evidence by DNA typing.

A case of postmortem animal scavenging by a domestic golden hamster (Mesocricetus auratus) is presented. A 43-year-old woman, who was not under medical treatment, was found dead in her flat with the lower part of her body naked and her legs straddled. Her face showed extensive lesions of the soft tissues which the medical examiner interpreted as vital scalping injuries. The total findings at the scene suggested at first a sexual offence. On autopsy the face injuries were identified as postmortem defects by animal scavenging with the teeth marks typical of rodents. In fact, the deceased had kept in her flat a free-range golden hamster whose burrow contained numerous fingernail-sized pieces of skin, fatty and muscular tissue. By means of DNA typing it was proved that these pieces of tissue belonged to the deceased.

Excoriations and contusions of the skin as artefacts in fictitious sexual offences.

Some women reporting a sexual offence to the police inflict injuries on themselves, usually with the help of pointed and/or cutting tools, in order to make fictitious assaults more credible. If there is a classical pattern of the findings, such fictitious assaults are easier to identify. However, it is more difficult to distinguish genuine from fictitious injuries, if blunt force is used. This paper reports on 4 such cases, and the characteristics of self-inflicted excoriations and skin contusions are discussed. In the cases described, the excoriations were produced by scratching with the person's own fingernails or by rubbing the skin against a rough surface; contusion bleeding was caused by pinching (lifting and compressing skin folds with the fingers). The pattern of the injuries showed striking parallels to classical patterns: multiple lesions in grouped, parallel and symmetrical arrangement and of evenly minor intensity; preference of body regions within easy reach; no damage of the clothing, or if any, damage inconsistent with the injuries as described.

[Morphologic and morphometric aspects of contusion ring ("abrasion seam") of gunshot wounds].

Test shots (cartridges .22 lr and 9 mm Parabellum) were fired on skin preparations taken from the lumbar region. As anticipated, with caliber .22 the outer diameter and the area of the abrasion ring were smaller than with the 9 mm caliber. This was true of both orthograde and inclined angle shots. In the case of vertical projectile impact, the outer diameter of the epidermis-free area was either smaller or only slightly larger than the cross-section of the projectile. The area of the abrasion ring exceeded the area of the entrance hole in the proportion of 3:1 (.22 lr) and 2.45:1 (9 mm Para), respectively. The epidermis bordering immediately on the abrasion ring was lifted off, with irregular and torn-in edges. Progressive drying up of this border zone simulated a time-dependent increase of the abrasion ring. If the bullet penetrated at an oblique angle, the eccentrically enlarged drying-up collar (on the side from which the bullet came) showed a tongue-shaped area of unaffected epidermis with an intact stratum corneum. This phenomenon can be explained by the wound-ballistic behaviour of the skin in inclined angle shots. Skin covered with a layer of water showed no abrasion collar around the entrance hole.

[Fatal shotgun use in peace time].

On the suggestion of Conzelmann et al. (1977) and Lerch (1982), 176 shooting fatalities with 249 shot wounds in the South Baden region were evaluated in a multi-dimensional analysis of such data as place of death, autopsy findings, forensic examinations, and police and court files. Data were collected on incident, victim, culprit, weapon used, motive and the special circumstances of each case. The greatest number of deaths are suicides of inebriated men using handguns of medium-size caliber (7.65 and 9 mm), followed by homicidal shooting deaths, some involving multiple shots at longer range, though one-third were at close range. 78% of the shots fired from rifles and shotguns were in connection with suicides, 50% involved small-caliber weapon, 86% of the victims were men. Only 7% of the 131 shooting suicides, but 36% of the 39 homicides, involved women. 58% of the women who died a shooting death were victims (not seldom followed by their mates committing suicide), men were victims in only 17% of the cases. 98% of the suicidal and one-third of the homicidal shots were fired at close range. Rifles or shotguns were used in 12% of the homicides, but also in 27% of the suicides. Yet according to witnesses, every 10th victim, though mortally wounded in the head, chest, or abdomen, was still capable of action for some time. Every 7th suicides was still clutching the weapon after death. Half of the suicides involving unusual shots were committed by women. In 50% of the homicides, the killer was either the mate or a relative of the victim, was between 40 and 60 years of age, and committed suicide after shooting his mate. In 78% of the cases, killer and victim knew each other. Two-thirds of all the suicides occurred in the home, with the bedroom being the preferred scene in 25% of the cases.

[Fatal intoxication following rectal instillation of alcohol].

A 62-year old man who had been living on a remote farm was found dead in his bed. Inserted in his rectum was a synthetic flexible tube filled with brandy (43 vol. %) and connected to a bicycle pump. Samples taken during autopsy from 3 different parts of the body showed ethanol concentrations from 4.87% to 5.35% in the blood and 6.73% in the urine. The ethanol concentrations in the small and large intestine were considerably higher (more than 29%) than in the stomach (9%). The decreased had a tumor as large as a tennis ball on the base of his tongue, almost completely filling the oropharynx, making swallowing very difficult, which probably was the reason of the rectal instillation of alcohol. The report deals with this unusual case of rectal alcohol instillation, and with the different ethanol and congener alcohol concentrations in the body fluids.

[Injuries caused by frontal and side collisions].

The injuries of 100 passengers killed in automobile accidents and technical accident reconstruction data were recorded in a databank and analyzed according to the victim's age, survival time, extent of injuries, seat-belt protection, position of the seat, contusions, intrusions and Delta-v as a measure of passenger stress. 25 in-town, 39 rural road and 36 highway accidents were recorded, involving 74 drivers, 22 front-seat and 4 back-seat passengers as victims. The accidents were head-on in 79 cases, 17 side and 4 rear-end collisions. Fatal injuries were observed starting at Delta-v 40 km/h. Half of the victims were less than 30 years of age, 2/3 not wearing seat-belts and 1/3 of the drivers had blood alcohol concentrations of 1.0-2.5 g%. The most frequent injuries involved the arms, chest and skull. 88% of the victims died within 2 hours of the accident, and 82% were very severely poly-traumatized with 2-5 serious individual injuries and 48% with multiple injuries resulting in death. The chest was the body area subjected to the greatest biomechanical impact in the passengers wearing seat-belts.

[A new formula for the measurement of rigor mortis: the determination of the FRR-index (author's transl)].

The manual examination of rigor mortis as currently used and its often subjective evaluation frequently produced highly incorrect deductions. It is therefore desirable that such inaccuracies should be replaced by the objective measuring of rigor mortis at the extremities. To that purpose a method is described which can also be applied in on-the-spot investigations and a new formula for the determination of rigor mortis--indices (FRR) is introduced.

[Discovery of externally unremarkable case of homicide of an infant].

The court-ordered autopsy of an infant who had died 26 days after delivery unexpectedly revealed a fatal head injury. As external inspection of the body had not shown any traces of violence sudden infant death had at first been assumed. At autopsy a linear fracture of the skull and a subdural haematoma was found. Subsequent investigations showed that the father had thrown the baby into the air while playing with it, that he had failed to catch it and that it had fallen downwards heavily hitting the wooden armrest of a couch with the parietal region. As an example of a manslaughter without any external signs of trauma the case illustrates the importance of forensic autopsies in all cases of unexpected death.

[Morphologic patterns in suicidal stab injuries of the neck].

Stabs into the neck are seen very seldom in connection with suicide. If there are features typical of suicide, the medico-legal analysis of the pathomorphological findings essentially may contribute to the differentiation from homicidal injuries. On the basis of the observations already published and three own cases an attempt is made to derive criteria of self-infliction for differential diagnosis from the autopsy findings. Concomitant injuries and combination with another method of suicide have turned out to be important indications of self-infliction.

[Suicidal stab wounds through clothing].

Most authors of medico-legal textbooks emphasize that suicides bare the chest before self-infliction of stab wounds and vice versa that cuts in the clothing indicate homicide. The paper presents three cases of suicidal stabs to the chest and/or abdomen with concomitant damage to the clothing.
