A prospective randomized study comparing intramuscular progesterone and 17alpha-hydroxyprogesterone caproate in patients undergoing in vitro fertilization-embryo transfer cycles.

OBJECTIVE: To compare the effectiveness of i.m. P and 17alpha-hydroxyprogesterone caproate (17-HPC) for luteal phase support, in patients undergoing IVF-ET cycles. DESIGN: Prospective, randomized study. SETTING: Patients undergoing IVF-ET in our Centers. PATIENT(S): The inclusion criteria were the use of GnRH down-regulation and aged <40 years. INTERVENTION(S): A total of 300 cycles were randomly treated with either 17-HPC (341 mg every 3 days) or P (50 mg daily). MAIN OUTCOME MEASURE(S): The outcomes of IVF in both study groups were evaluated for biochemical pregnancy, miscarriage, clinical pregnancy, and ongoing pregnancy. RESULT(S): No difference was found in the main outcome parameters considered. CONCLUSION(S): Although the results of the study encourage the use of 17-HPC for luteal phase support in patients undergoing IVF-ET program, more studies are necessary to support the hypothesis that it can replace i.m. P-in-oil.

Modeling the Evolution and Rupture of Pendular Liquid Bridges in the Presence of Large Wetting Hysteresis.

A model has been developed to predict the shape evolution, rupture distance and postrupture liquid distribution of a pendular liquid bridge between two unequally sized spherical particles in the presence of wetting hysteresis. Two different simplifications of the bridge geometry were considered: a toroidal and a parabolic approximation. The liquid bridge was assumed to rupture through its thinnest neck leaving liquid distributed on each sphere. Experimental measurements showed that the rupture distance was well predicted by both profile approximations by assuming that rupture occurred when the liquid-vapor interfacial area of the bridge and the postrupture droplets was equal. Both bridge profile approximations only correctly predicted the evolution of the apparent contact angle and the extent of postrupture liquid distribution when the solid-liquid interfacial area measured throughout the separation was included in the calculations. This is because during the pendular liquid bridge elongation, the three-phase contact line usually begins to slip on at least one of the spheres. The parabolic profile approximation was slightly more accurate than the toroidal one. The toroidal approximation is more difficult to use because one of the parameters passes through infinity as the bridge changes from convex to concave in shape. In some cases the toroidal approximation was also unable to generate a solution. Copyright 2000 Academic Press.

Modeling Pendular Liquid Bridges with a Reducing Solid-Liquid Interface.

Liquid bridges formed between particles of dissimilar surface properties are important in many processes involving the handling of powders in mixtures. For instance, growth kinetic models for wet granulation frequently incorporate the evolution and resistance to breakage of individual liquid bridges between particles in a statistical form. These models generally propose a confusing definition of liquid-to-solid contact angles. Taken as a single thermodynamic value, they typically neglect the influence of wetting hysteresis on the liquid bridge. In this paper, a simple model based on the interfacial energies is proposed for the evolution of liquid bridges when one solid-liquid interface reduces. This receding process is well described by a balance between the adhesion energy of the bridge liquid on the particle surface and the capillary energy stored by the liquid free surface. The extent of solid-liquid interfacial area reduction can hence be predicted from the initial liquid bridge configuration. The liquid bridge shape is approximated by a parabolic curve, which is validated from the good agreement between measured and calculated contact angles or liquid-vapor interfacial area. Use of intramuscular progesterone versus intravenous albumin for the prevention of ovarian hyperstimulation syndrome.

This study was designed to compare the effectiveness of intramuscular progesterone with that of intravenous albumin in the prevention of ovarian hyperstimulation syndrome (OHSS). Ninety-six patients at high risk to develop OHSS (estradiol concentration >9, 000 pmol/l on the day of hCG administration and over 20 follicles of a diameter larger than 14 mm observed by transvaginal ultrasonography) and undergoing in vitro fertilization-embryo transfer were enrolled. They were randomly treated with intramuscular progesterone (200 mg/day) or 100 ml of 20% intravenous albumin in order to estimate the difference in the incidence of OHSS. A significant difference in the incidence of moderate OHSS and no cases of severe OHSS were observed between the groups. Our data show the effectiveness in preventing OHSS with high doses of progesterone.

[Solitary metastasis of the appendicular bones: role of orthopedic surgery].

The authors report their experience in the treatment of solitary metastasis of the appendicular skeletal system with reference to a group of 59 cases treated. The importance of orthopaedic surgery, involving medical oncology, radiotherapy and physiotherapy is emphasised. The surgical methods used to obtain stable synthesis and facilitate assistance to the patients are discussed. In circumscribed lesions the use of cryotherapy with azote fluid may be useful, and in some cases skeletal reconstruction with autoplastic bone transplant is indicated.

Current trends in the treatment of Ewing's sarcoma.

The authors analyse the results obtained in the treatment of Ewing's sarcoma by comparing the period previous to the use of chemotherapy with the more recent one involving the use of antiblastic drugs. Thirty-one patients with Ewing's sarcoma were divided into three groups according to the type of treatment used. The first group included 11 patients treated either by surgery or high dosage radiation (7000-8000 rads); the second group included 9 patients treated by radiation plus polychemotherapy; finally, the third group included 11 patients treated by surgery (excision in 5 localizations and segmental resection in the remaining 6), associated with polychemotherapy and low dosage radiotherapy. The results in the first group were discouraging as none of the patients survived five years; there was a low survival rate (33%) for the second group, partly due to the fact that there were 4 localizations in the pelvis, a site with a notoriously unfavourable prognosis; the third group had the highest percentage (55%) of long-term disease-free patients. Thus, in agreement with what is reported in the literature, the most radical surgery possible, associated with polychemotherapy and low-dose radiotherapy, currently seems to be the best treatment.

Analysis of the globins from fast human haemoglobins by isoelectrofocusing on polyacrylamide gel rods.

The globins from all fast haemoglobin (Hb) components obtainable by Bio-Rex 70 cation-exchange chromatography were examined by isoelectrofocusing on polyacrylamide gel rods with 8.0 mol/l urea. From this analysis HbA1a1 and HbA1a2 seem to be very heterogeneous components. HbA1b is separable into two components, one of which is varied in both the beta chains. Between HbA1b2 and the well-known HbA1c components two chromatographic peaks are separated, one with a noticeable percentage of glucosylated beta chain and one that probably contains HbF. HbA1c has both beta chains glucosylated, while HbA1x seems to be a beta monoglucosylated Hb form. Finally, the early part of the HbAo peak has a large amount of glucosylation on both alpha and beta chains.

Investigation of the heterogeneity of hemoglobin by cation-exchange chromatography on Bio-REX 70.

The use of Bio-Rex 70 cation-exchange resin for chromatography of normal and diabetic hemoglobin provides a reproducible pattern of the "fast components". Particular attention to the choice of sample preparation, pH of elution, and the increase of ionic strength by sodium chloride linear gradients results in the separation of Hb-A1b into two components and in the isolation of a new component eluting between Hb-A1c and Hb-A0. Experiments with [3H]glucose and the colorimetric test (thiobarbituric acid) normally used to determine the extent of non-enzymatic glycosylation, as well as an increase of this component in diabetic samples compared with normoglycemic ones and a significant linear correlation with Hb-A1c, indicate that this component should be a part of the hemoglobins glucosylated on the epsilon-NH2 group of the lysines of both chains and/or the hemoglobin glucosylated on the alpha-NH2 of the valine of the alpha-chain. We propose to call this component Hb-A1x, pending confirmation of its identity. Normally Hb-A1x accounts for about 3% of Hb-A, but up to 5-7% of glucosylated hemoglobins should be confined to the early part of Hb-A0. In diabetics, the percentage of Hb-A1x rises to 4-5% and that of the other glucosylated hemoglobins increases to 12-16%.

The chromatographic separation of glycosylated haemoglobins: a comparison between macro- and micromethods.

The microchromatographic determination of "fast haemoglobins" is currently used in long-term control of diabetes: its use is made easier by commercially available kits, that are impaired by some analytical variables. For a good standardization of these kits a comparison with an accurate method of analysis is necessary. The macrochromatographic column allows the determination of each fast component with a good precision, and reveals the great complexity of human haemoglobin. Comparison of the two methods shows that the most appropriate temperature of elution for the kit is 20 degrees C and that the increase in the fraction of "fast haemoglobins" in diabetic samples is only related to the HbA1c component.

Atypical squamous cells of undetermined significance (ASCUS), low-grade squamous intraepithelial lesion (LSIL), high-grade squamous intraepithelial lesion (HSIL) and histology.

OBJECTIVE: A new terminology for cytologic diagnosis of cervical lesions has been introduced by Bethesda System. This includes: 1) Atypical squamous cells of undetermined significance (ASCUS), 2) Low-grade squamous intraepithelial lesion (LSIL), 3) High-grade squamous intraepithelial lesion (HSIL), 4) squamo-cellular carcinoma. The aim of this study was to assess the correlation between the colpocytologic test (Pap psmear), the histologic response and colposcopy. METHODS: We re-examined the cytologic results of 447 patients who underwent routine cytologic tests, with a diagnosis of various grades of atypia, from ASCUS to HSIL. A histologic test was carried out on a colposcopic basis in 210 cases and cytologic results were correlated with the histologic and colposcopic pictures. RESULTS: For ASCUS the histology was positive in 19.1% (31/163) of cases and negative in 10.4% of the colposcopically positive cases (17/163), while in the remaining 70.5% (115/163) the colposcopy resulted negative. In LSIL with the presence of human papillomavirus (HPV) the histologic findings, confirmed the cytologic result in 35.8% (48/134) while in the group of cervical intraepithelial neoplasia (CIN I) results corresponded in 40.6% (41/101). In high squamous intraepithelial lesions there was an histologic confirmation in 59.2% (29/49). CONCLUSIONS: The data obtained indicate an increase in positive histology results from the types of low grade (ASCUS-LSIL) to those of a high grade (HSIL). Our results indicate that the presence of ASCUS should be assessed colposcopically and histologically, where indicated.
