We have described a patient with community-acquired, methicillin-resistant Staphylococcus epidermidis endocarditis that occurred on a prolapsing mitral valve. The patient was successfully treated with vancomycin and rifampin for six weeks and had no signs of relapse four months later.

A 38-year-old black woman presented with a multisystem disease characterized by malaise, fever, sweats, and diffuse hyperpigmentation. Laboratory examinations showed anemia, elevated alkaline phosphatase, granulomas in the liver and bone marrow, and elevated porphyrins in the urine and feces, characteristic of porphyria cutanea tarda (PCT). A workup for an etiology of the granulomas was nonproductive, and the patient responded to systemic steroids.
A 60-year-old black woman had a large hyperkeratotic lesion and multiple smaller hyperkeratotic papules and plaques on the lower part of her legs in areas of erythema ab igne. Histologic examination of the largest lesion showed hyperplastic carcinoma in situ and the multiple smaller lesions showed varying degrees of squamous cell atypia and dermal elastosis. Histologically, these lesions were identical to solar-induced atypia, indicating that squamous cell carcinoma arising in erythema ab igne may be biologically similar to actinic carcinoma. Discussed here are clinical and histologic features of the thermal-induced lesions and other types of thermal-induced carcinomas.
A 26-year-old man had mal perforans pedis as the principal manifestation of spina bifida occulta with an associated intradural lipoma. Two lumbosacral dimples and a tuft of hair overlying the spina bifida led to the diagnosis. We include a review of the causes of mal perforans and a review of spina bifida occulta and associated abnormalities.
