Nuclear pseudoinclusions in melanocytic naevi and melanomas.

Melanocytes in melanocytic naevi and melanomas can display great variation. The presence of nuclear pseudoinclusions (NPI) is said to be useful in the histological and cytological differential diagnosis of malignant melanoma. The prevalence and characteristics of NPI in a series of 493 naevi and 50 melanomas are described. NPI were found in 31% of adult naevi, 30% of congenital naevi from children, 42% of Spitz naevi, 20% of dysplastic naevi, and 56% of melanomas. The presence of NPI is not a reliable criterion for differentiating melanoma from benign melanocytic lesions, although it is useful in distinguishing melanocytic from non-melanocytic tumours.

Post-irradiation carcinosarcoma of the parotid gland.

Carcinosarcoma of the salivary glands are rare tumours, often associated with a history of pleomorphic adenoma. A case of carcinosarcoma of the parotid arising following irradiation to the resection site of a pleomorphic adenoma is presented. The clinical and histological features are discussed and the literature reviewed.

Multiblock slides: a useful technique for teaching.

Data on the quality of a modified method for producing multiblocks from conventional blocks of paraffin wax embedded histological material are reported. Originally devised as a way of assessing antibodies for immunohistology, the technique is particularly suited to producing a set of reference and teaching material for use by trainee and senior pathologists.

Which proliferation markers for routine immunohistology? A comparison of five antibodies.

AIMS: To determine the best of five antibodies for immunohistochemical assessment of growth fraction in formalin fixed, paraffin wax embedded tissues. METHODS: Sections from 100 recent, and 17 ten year old or over wax embedded blocks of normal and malignant tissues were immunostained with monoclonal Ki67, polyclonal Ki67, PC10, MIB1, and JC1. The antibodies were evaluated for specificity of nuclear versus cytoplasmic staining, cleanliness of background, and compared with the expected pattern of staining in normal tissues, defined immunohistochemically by monoclonal Ki67 antibody in frozen tissues or by tritiated thymidine uptake. RESULTS: No marker was ideal, but best results were obtained with MIB1 and polyclonal Ki67, followed by JC1, PC10, and monoclonal Ki67. CONCLUSIONS: For routine use, MIB1 or polyclonal Ki67 are the best proliferation markers in conventional histological preparations. The other markers tested cannot be recommended.

Primary pericardial mesothelioma presenting as tuberculous pericarditis.

Ante-mortem diagnosis of primary pericardial mesothelioma is very rare. We report a case which presented clinically as tuberculous constrictive pericarditis. The patient underwent pericardial resection with an immediate haemodynamic benefit, although the malignant process progressed, and he died 14 weeks later.

Focal nodular hyperplasia of the liver associated with intracranial vascular malformations.

Focal nodular hyperplasia of the liver, neoplasia of the brain, and intracranial vascular malformations have recently been described as a new syndrome. We report three cases of focal nodular hyperplasia of the liver with intracranial vascular anomalies. Two patients died from ruptured berry aneurysm. It is important to consider intracranial pathology in patients with focal nodular hyperplasia of the liver.

Role of calcium in the secretomotor response of the thyroid: effects of calcium ionophore A23187 on radioiodine turnover, membrane potential and fluid transport in cultured porcine thyroid cells.

The calcium ionophore A23187 (0.1-1 mumol/l) inhibited membrane electrical polarization, uptake of 125I, fluid transport and TSH-stimulated release of radioiodine from the organic pool in follicular cultures of porcine thyroid cells. At higher concentrations (1-30 mumol/l), A23187 promoted release of radioiodine from the organic pool. Stimulation of release of radioiodine from the organic pool by veratridine (a sodium channel agonist, 0.4-1 mmol/l) and A23187 was dependent on the calcium concentration of the medium, while TSH action was independent. Incubation in medium of very low calcium concentration (0.0177 mmol/l) resulted in enhanced release from the organic pool, which was inhibited by TSH (256 microU/ml), A23187 (25 mumol/l) or veratridine (0.5 mmol/l). These data therefore do not support the hypothesis that calcium acts as a mediator of the secretomotor action of TSH, but suggest the possibility of a TSH-induced increase in intracellular calcium as a regulatory negative-feedback mechanism.

A filamentous distribution for the herpes simplex virus type 2-encoded major DNA-binding protein.

Monoclonal antibodies reacting with the herpes simplex virus (HSV)-encoded major DNA-binding protein defined an intracellular filamentous network. This network was associated predominantly with the infected cell nucleus and occurred in cells infected with HSV type 2. It did not co-distribute with microfilaments, microtubules or intermediate filaments, and DNA synthesis was required for its formation. We suggest explanations for the occurrence and function of this novel filamentous network structure.

"Worse than death": psychodynamics of rape victims and the need for psychotherapy.

Victims of rape routinely are acknowledged to need counseling but not psychotherapy; if psychotherapy is provided, the major focus is on preexisting psychopathology. The author examines these assumptions and presents a different formulation based solely on an in-depth examination of the many levels of the trauma. Case material reveals the typical symptoms, conflicts, and defenses found in rape victims. The author makes recommendations for psychotherapeutic technique, especially in regard to difficulties in forming a therapeutic alliance.
